The cardiovascular risk of metabolically healthy obesity: an inflammatory perspective.

Metabolically healthy obesity (MHO) is a heterogeneous phenotype characterized by preserved traditional metabolic markers despite excess adiposity. However, accumulating evidence suggests that MHO may not be uniformly benign and may confer increased long-term cardiovascular risk through inflammatory and adipose tissue-mediated mechanisms. This narrative review synthesizes contemporary evidence on cardiovascular risk in MHO, focusing on visceral adiposity, ectopic fat, and immune metabolic pathways. Literature published between January 2010 and December 2025 was identified through PubMed/MEDLIN

Structural insights into an aptainhibitor of the SARS-CoV-2 E protein using molecular docking, molecular dynamics, and biophysical analyses.

The envelope (E) protein is a highly conserved structural protein that plays key roles in viral assembly, membrane curvature, and pathogenesis. The E-protein transmembrane (TM) domain oligomerizes into a pentameric ion channel, which contributes to the activation of the NLRP3 inflammasome that is directly linked to acute respiratory distress syndrome (ARDS). Therefore, targeted disruption of E-protein oligomerization represents a powerful therapeutic strategy to attenuate viral virulence. In this study, we report the first DNA aptainhibitors to block E-protein oligomerization. The lead aptainh

The untapped potential of oral vitamin E in lichen sclerosus: from immunobiology to real-life clinical experience.

Lichen sclerosus (LS) is a chronic dermatosis characterized by persistent inflammation, progressive fibrosis, and an elevated risk of vulvar squamous cell carcinoma. While ultra-potent topical corticosteroids remain the gold standard of care, the necessity for long-term maintenance therapy raises concerns regarding cumulative toxicity, underscoring the critical need for adjunctive therapies that target underlying pathogenic mechanisms. Vitamin E, traditionally recognized for its antioxidant and membrane-protective properties, has emerged as a promising candidate due to its broad immunomodulato

Delphinidin modulates neuroinflammation and behavioral deficits in a Parkinson's disease mouse model.

Neuroinflammation is deeply intertwined with dopaminergic (DA) neurodegeneration in Parkinson's disease (PD). We tested whether delphinidin, an anthocyanidin with reported inflammasome/NF-κB modulatory activity, alters neuroinflammation and nigrostriatal integrity in a progressive AAV1/2-A53T α-synuclein (hαSYN) mouse model. Once-daily intraperitoneal delphinidin for nine weeks modestly ameliorated asymmetric forepaw use, attenuated the hαSYN-induced loss of striatal TH⁺ terminal density, and was associated with modest alterations in dopamine turnover, yet did not prevent the loss of DA neuron

Hyperbaric oxygen therapy in sepsis-associated encephalopathy: pathophysiological rationale, preclinical evidence, and translational challenges.

Sepsis-associated encephalopathy (SAE) is a serious neurological complication of sepsis that can cause acute cerebral dysfunction and long-term cognitive impairment. Its pathogenesis involves multiple pathological processes, including blood-brain barrier (BBB) disruption, neuroinflammation, microglial and astrocytic responses, oxidative stress, mitochondrial dysfunction, ferroptosis, and pyroptosis. Excessive reactive oxygen species (ROS) production and mitochondrial injury can aggravate cerebrovascular endothelial and BBB damage, promote glial inflammatory responses, and contribute to lipid p

Potential Roles of Escherichia coli in IgA Nephropathy: Evidence, Mechanistic Hypotheses and Microbiota-Targeted Strategies.

IgA nephropathy (IgAN), the most common primary glomerular disease, involves mucosa-kidney axis dysregulation, with gut dysbiosis increasingly implicated in its pathogenesis. Certain Escherichia coli (E. coli) strains harbour virulence-associated features that promote intestinal colonization, barrier disruption and immune activation. Recent microbiome and metagenomic studies have reported an enrichment of E. coli in patients with IgAN, suggesting a potential link to the mucosa-kidney axis. Nevertheless, E. coli may contribute to IgAN through two major pathways. First, sustained LPS exposure ma

The Role of TXNIP in Skeletal Muscle Physiology and Related Diseases: Current Perspectives.

Thioredoxin-interacting protein (TXNIP) is a multifunctional regulatory protein of the α-arrestin family that acts as a redox sensor, an energy-responsive molecule, and a transcription-associated regulator. TXNIP has emerged as a key regulator of skeletal muscle homeostasis and has been implicated in the pathogenesis of multiple muscle disorders. Skeletal muscle is a principal site of systemic glucose disposal, accounting for more than 70% of insulin-stimulated glucose uptake in the human body. TXNIP has been implicated in sarcopenia, obesity-related insulin resistance, limb ischemia-induced m

Establishment of a TGF-β-Induced Rodent Myopia Model Using Subconjunctival Injections.

Myopia is a major global public health concern characterized by excessive axial elongation and scleral extracellular matrix (ECM) remodeling. While experimental models are essential for investigating these mechanisms, traditional optical defocus methods in small rodents present technical limitations. Here, a standardizable pharmacological myopia model was established and evaluated via repeated subconjunctival injections of transforming growth factor-beta (TGF-β). To achieve targeted delivery, a dual-handed surgical stabilization technique was developed to evert the upper eyelid, allowing a 33-

The immunoproteasome in Alzheimer's disease: a dual regulator of tau pathology and microglial senescence.

Alzheimer's disease (AD) is characterized by progressive cognitive decline driven by the convergence of tauopathy, impaired proteostasis, and chronic neuroinflammation. Emerging evidence identifies the immunoproteasome (iP), particularly the β5i/LMP7 subunit, as a central regulator linking these pathological processes. Unlike the constitutive proteasome, the iP is markedly induced in microglia, neurons, and astrocytes under inflammatory conditions through interferon-γ and NF-κB signaling. This review discusses the dual and context-dependent role of the iP in AD pathogenesis. In microglia, chro

Recent advances of GPR40 agonists as potential anti-Alzheimer's agents.

Alzheimer's disease (AD) is a progressive and multifactorial neurodegenerative disorder involving metabolic dysfunction, neuroinflammation, mitochondrial impairment, synaptic failure, and neuronal loss. Although amyloid-beta deposition and neurofibrillary tangles composed of hyperphosphorylated tau remain central pathological hallmarks, interconnected mechanisms contribute to disease progression and limit the effectiveness of single-target therapies. Current small-molecule drugs mainly provide symptomatic relief, whereas recently approved monoclonal antibodies have broadened the therapeutic la

Mitochondrial Dysfunction as a Driver of Meta-Inflammation in Aging: The Emerging Role of PDK4 in Bioenergetic Reprogramming and Inflammatory Amplification.

Aging is accompanied by a progressive decline in mitochondrial quality, bioenergetic flexibility, and stress resilience. Aging mitochondria are increasingly recognized as active inflammatory signaling platforms rather than passive targets of cellular damage. Excess mtROS, leaked mtDNA, defective mitophagy, altered NAD+ metabolism, and impaired pyruvate oxidation together create a cellular environment that favors persistent inflammatory activation. These signals engage NF-κB, NLRP3 inflammasome, cGAS-STING, and SASP pathways, allowing mitochondrial stress to spread from organelle dysfunction to

Immunological Reprogramming in Cardiomyopathies: From Cardiomyocyte Injury to Disease of the Cardiac Immune Ecosystem.

Cardiomyopathies have traditionally been regarded as disorders driven primarily by cardiomyocyte injury resulting from genetic defects, infection, metabolic stress, or toxic exposure. This paradigm has substantially advanced diagnosis and treatment. Still, it does not fully account for the marked heterogeneity in disease progression, persistent fibrosis, or variable therapeutic responses among patients with similar phenotypes. Increasing evidence indicates that immune remodeling is not merely a secondary consequence of myocardial injury but a dynamic process that actively shapes disease evolut

Inhibition of galanin receptor 3 slows down retina degeneration in retinitis pigmentosa through modulation of inflammatory and oxidative stress response.

Photoreceptors require a finely regulated balance of oxygen, nutrients, and waste removal to sustain visual function. In inherited retinopathies like rhodopsin (RHO)-associated retinitis pigmentosa (RP), disruption in retinal homeostasis leads to neurodegeneration. The most common mutation in RHO, P23H, causes protein misfolding, endoplasmic reticulum (ER) stress, and activation of inflammatory and oxidative stress pathways, ultimately leading to photoreceptor death. Upregulation of the NLRP3 inflammasome and NF-κB signaling in RHO mutant models, highlight inflammation as a key contributor to

Polydopamine-functionalized 3D-printed scaffold co-loaded with resveratrol and NGF for enhanced neuronal differentiation under inflammatory stress.

To address core issues in spinal cord injury repair such as oxidative stress, excessive inflammation and impaired nerve regeneration, this study developed a 3D-printed multifunctional PCP-Res-NGF scaffold. The scaffold achieves stable loading of resveratrol and nerve growth factor by sequentially coating chitosan and polydopamine on the polylactic acid skeleton. Experiments show that the scaffold has pH/ROS dual-responsive drug release characteristics, fitting the S-type Weibull model in simulated injury environments and following a first-order kinetic model under physiological conditions; it

Microglial ferroptosis mediated neuroinflammation in central nervous system diseases.

Microglial ferroptosis has become an important pathological mechanism in studies of central nervous system (CNS) diseases. Rather than being viewed only as an endpoint of cell death, ferroptosis in microglia is increasingly recognized as a process that links iron dyshomeostasis, lipid peroxidation, oxidative stress, and immune-inflammatory activation, thereby contributing to the sustained amplification of neuroinflammation. In this review, we summarize the molecular mechanisms by which microglial ferroptosis mediates neuroinflammatory responses, with a focus on iron homeostasis disruption, lip

Potential Relationship Between Ferroptosis and Pyroptosis in Myocardial Ischemia/Reperfusion Injury: Molecular Mechanisms and Therapeutic Targets.

While restoring blood flow is the primary treatment for acute myocardial infarction (AMI), this process often triggers additional damage known as myocardial ischemia/reperfusion injury (MIRI). Recent studies have suggested that regulated cell death is a major contributor to cardiomyocyte injury during ischemia and subsequent reperfusion. Notably, emerging evidence has demonstrated that both ferroptosis and pyroptosis play important roles in the pathogenesis of MIRI. Ferroptosis is primarily driven by iron-dependent lipid peroxidation, whereas pyroptosis is characterized by inflammasome-mediate

Berberine inhibits muramyl dipeptide-induced oxidative stress and pyroptosis in canine corneal epithelial cells via the NOD2 signaling pathway.

Bacterial keratitis poses a significant threat to ocular health in companion animals, particularly dogs, often leading to corneal ulceration and visual impairment. Muramyl dipeptide (MDP), a conserved bacterial peptidoglycan motif, activates nucleotide-binding oligomerization domain-containing protein 2 (NOD2), yet its contribution to corneal pathogenesis remains incompletely defined. This study demonstrated that MDP induced the NOD2-dependent activation of the RIPK2/NF-κB pathway, triggering pro-inflammatory cytokine production and suppressing the Nrf2-mediated antioxidant response, thereby e

An electrochemical enzyme-linked immunosorbent assay for interleukin 18 quantification in 3D skin models derived from ALS patients.

Amyotrophic lateral sclerosis (ALS) is a devastating neurodegenerative disorder that currently lacks validated molecular biomarkers for early diagnosis and prognosis, severely delaying personalized care. Interleukin 18 (IL-18), a proinflammatory cytokine linked to NLRP-3 inflammasome activation, has emerged as a promising biomarker for ALS. However, traditional colorimetric Enzyme-Linked Immunosorbent Assays (ELISAs) lack the sensitivity to distinguish IL-18 levels between Fast- and Slow-progressing ALS patients. To overcome this, we developed a highly sensitive electrochemical ELISA (e-ELISA)

HSPA8 lactylation attenuates neuronal pyroptosis via E3 ligase-mediated NLRP3 degradation after ischemic stroke.

BACKGROUND: Pyroptosis contributes significantly to neuronal death following ischemic stroke. This study investigated whether lactylation of heat shock protein family A member 8 (HSPA8) modulates neuronal pyroptosis after ischemic/reperfusion injury. RESEARCH DESIGN AND METHODS: Both in vitro oxygen-glucose deprivation/reperfusion (OGD/R) models and in vivo transient middle cerebral artery occlusion (tMCAO) mouse models were established to assess the functional significance of HSPA8 lactylation at lysine 524 (K524). RESULTS: HSPA8 expression and lactylation increased following ischemic/reper

Unraveling the role of non-coding RNAs in Parkinson's disease: Molecular mechanisms and therapeutic insights.

Parkinson disease (PD) is a progressive neurodegenerative disorder characterized by the selective loss of dopaminergic neurons in the substantia nigra pars compacta and pathological accumulation of α-synuclein in Lewy bodies. In this process, a set of non-coding RNAs including miRNAs, lncRNAs, and circRNAs form key regulatory layers in the pathogenesis of the disease and directly affect α-synuclein homeostasis, mitochondrial function, oxidative stress, neuroinflammation, autophagy, and proteostasis. Dysregulation of miRNAs targets neurosensitive pathways; miR-7 and miR-153 inhibit SNCA transla
