Iatrogenic arteriovenous fistula: a complication of percutaneous subclavian vein puncture.

A case of iatrogenic subclavian arteriovenous fistula is reported. An intracath had been inserted percutaneously into the left subclavian vein to monitor the central venous pressure during aortic valve surgery. On the second postoperative day a thrill and continuous murmur were detected at the left subclavian region. Left subclavian arteriogram and catheterization studies confirmed the diagnosis of subclavian arteriovenous fistula. The lesion was treated surgically with satisfactory results.

Influence on clinical practice of routine intra-partum fetal monitoring.

An attempt has been made to monitor by continuous fetal heart rate according all women admitted in labour. Altogether 85% of the 1070 patients delivered at one hospital were monitored in 1973 and 92% in 1974. Perinatal mortality fell significantly from levels in preceding years to 15-8 and 11-7 per 1000 births, respectively, in 1973 and 1974. The fall was primarily due to the elimination of intra-partum stillbirths and a significant reduction in neonatal mortality. The incidence of caesarean sections also fell from 9-7% in 1973 to 5-8% in 1974. All patients should be monitored because it is im

Fluid replacement monitoring: effect of dextran overload, norepinephrine drip, and positive pressure ventilation on systemic arterial, right atrial pulmonary wedge, and left atrial pressures in dogs.

The effects of dextran overload, norepinephrine drip and positive pressure ventilation upon right atrial, pulmonary wedge, left atrial and systemic arterial pressures were studied in 15 dogs. Rapid intravenous infusion of Dextran 70 invariably produced a marked and statistically significant (p < .001) rise in right atrial, pulmonary wedge and left atrial pressures. The rise in left atrial pressure invariably exceeded the rise in right atrial pressure, and the difference in maximum pressures averaged 10.8 mm Hg (p < .001). Thus acute fluid overload and pulmonary edema can be produced by rapid i

Implanted intracranial pressure sensors: mechanisms, fabrication, and system integration.

Intracranial pressure (ICP) is a key physiological parameter for the diagnosis and management of traumatic brain injury, hydrocephalus, stroke, and other neurological disorders. ICP sensors are typically required to provide accurate pressure measurement at low pressure while maintaining long-term stability in biofluid environments. Although conventional liquid-based ICP systems remain the clinical standard, their bulky and tethered configurations increase the risk of infection and tissue injury and restrict patient mobility. Advances in miniaturized implantable sensors have created new opportu

Occupational Physical Activity and Hypertension: A Narrative Review.

PURPOSE OF REVIEW: To summarize the prospective and experimental evidence examining occupational physical activity (OPA) and hypertension-related outcomes, discuss potential physiological mechanisms, and identify methodological challenges contributing to heterogeneity across studies. RECENT FINDINGS: Prospective evidence linking OPA with hypertension and resting blood pressure remains limited and inconsistent, with studies reporting predominantly null or modestly inverse associations. Some evidence suggests very high OPA may be unfavorable in specific subgroups, particularly when combined wit

Post-lung transplant surveillance in 2026: current practice, variability, and the need for standardization.

Post-lung transplant surveillance remains highly heterogeneous, with no universally accepted standard guiding organisation of care or the use of physiological testing, imaging, bronchoscopy, laboratory monitoring, and emerging biomarkers. This narrative review synthesises current surveillance practices across these domains and addresses key limitations, sources of inter-centre variability, and evidence gaps that hinder timely detection of allograft dysfunction. We summarize established and evolving approaches to organisation of care, lung function monitoring, radiological assessment, invasive

In Vivo Real-Time Detection of Phenylephrine in Mice Using Boron-Doped Diamond Microelectrodes with Simultaneous Video-Based Assessment of Pupil Size.

Precise quantification of ocular pharmacokinetics remains a critical bottleneck in ophthalmic drug development, primarily constrained by the inability to monitor intraocular concentrations in real time. Current methods typically rely on destructive sampling, which precludes longitudinal assessment in individual subjects. Here, we report an electrochemical platform for time-resolved quantification of phenylephrine in the anterior chamber of living mice using needle-shaped boron-doped diamond microelectrodes. An optimized multistep potential protocol mitigated biofouling and enabled stable monit

A Near-Infrared Fluorescent Probe with a Large Stokes Shift for Detecting Hydrogen Sulfide in Environmental Waters, Wine Samples, and Living Systems.

Hydrogen sulfide (H2S) is closely related to physiological diseases, environmental pollution, food safety, and plant growth. Herein, a new nopinone-based near-infrared fluorescent probe has been developed to detect H2S with superior performance, including a large Stokes shift, low detection limit, good selectivity, and fast response time. NPD-H2S was used to detect H2S in environmental water and wine samples quantitatively. In addition, NPD-H2S could be used to fabricate nanofiber membranes as a portable tool for sensitively detecting H2S vapor using a smartphone. Moreover, NPD-H2S has been ef

A Wearable Multimodal NanoTracker for Noninvasive, Simultaneous Pharmacokinetic and Pharmacodynamic Monitoring.

Inter- and intrasubject physiological variability makes it essential to track the real-time interplay between therapeutic medications and the human body for precision medicine in both daily life and special contexts such as spacefaring. This creates an urgent need for noninvasive wearable sensors capable of real-time, simultaneous monitoring of individualized pharmacokinetic (PK) and pharmacodynamic (PD) responses. Yet, their development remains largely unexplored and technically challenging. Here, we present an integrated multimodal wearable NanoTracker (P2NanoTrek) that enables skin-on, noni

Unique Challenge of Saving Two Lives in Pregnancy with Severe Traumatic Brain Injury: A Narrative Review.

A pregnant patient is a body with two lives. Saving the mother and baby after severe head injuries during pregnancy requires intensive monitoring and multidisciplinary team based management. There are many neurosurgical challenges in cases of pregnancy with severe traumatic brain injury (TBI) during pregnancy and the postpartum period due to changes in physiological and anatomical changes due to pregnancy. The strategy to treat post traumatic hydrocephalus using a ventriculoperitoneal shunt is also risky as it tends to malfunction due to a gravid uterus. The challenges and issues faced by the

Gitelman's and Bartter's syndromes in pregnancy - a systematic review.

BACKGROUND: : Gitelman's syndrome (GS) and Bartter's syndrome (BS) are rare renal disorders causing electrolyte disturbances, with physiological changes of pregnancy potentially worsening symptoms and electrolyte derangement. METHODS: A literature search of electronic databases was conducted using terms describing (1) GS or BS and (2) pregnancy, and the quality of studies was graded using the NIH/NHLBI quality assessment tool. Data collected included measures of maternal and neonatal outcomes, pregnancy information, and details of treatment, diagnosis, and electrolyte monitoring. RESULTS: Fi

Mechanical modeling of mechanosensitive insect strain sensors as a tool to investigate exoskeletal interfaces.

During walking, sensory information is measured and monitored by sensory organs that can be found on and within various limb segments. Strain can be monitored by insect load sensors, campaniform sensilla (CS), which have components embedded within the exoskeleton. CS vary in eccentricity, size, and orientation, which can affect their sensitivity to specific strains. Directly investigating the mechanical interfaces that these sensors utilize to encode changes in load bears various obstacles, such as modeling of viscoelastic properties. To circumvent the difficulties of modeling and performing b
