A 24-year-old woman sustained a blow-out fracture of the orbital floor with prolapse of the globe into the maxillary sinus. This was treated by replacing the globe in the orbit surgically and reconstructing the orbital floor. Postoperatively, the eye retained normal visual acuity.

Detection and localization of known orbital foreign bodies with computed tomography was evaluated using a model that simulates as closely as possible in vivo conditions. The GE 8800 scanner proved to be an excellent instrument for detection and localization of most orbital or intraocular foreign bodies above certain minimum levels of detectability. The minimum detectable size varied according to the material, for example, 0.06 mm3 for steel, 1.82 mm3 for auto window glass in intraocular position, and slightly larger size for extraocular location. Small wood fragments were not detected.

Sudden unilateral proptosis occurred in a patient with a history of chronic sinusitis. The radiologic description of chronic sinusitis associated with the computerized tomography scan picture of a retro-orbital mass justified surgical exploration and evacuation of a hematoma. The proximity of the orbital cavity and the sinuses, plus their common venous drainage system, can account for this disease.

Three types of craniostenoses, plagiocephalies, trigonocephalies, and oxy-brachycephalies provoke modifications in the fronto-orbital region of the face, for which fronto-orbital remodelling osteostomies are proposed. A case of unilateral exophthalmos with ophthalmoplegia and papilledema which developed 1 week after culture-proven Shigella sonnei dysentery is described. Of 200 patients undergoing bilateral transantral-ethmoidal orbital decompression for Graves' ophthalmopathy, nine suffered symptomatic postoperative obstruction of the lower nasolacrimal excretory system. Five of the nine patients had persistent symptoms of epiphora or repeated episodes of dacryocystitis that necessitated dacryocystorhinostomy. Dacryocystorhinostomy relieved symptoms and objective evidence of obstruction in these five patients. We believe that nasolacrimal duct obstruction occurred in these patients as a complication of nasoantral window construction.

Localized necrotizing inflammation of the Harderian gland was a sequela to obtaining blood samples from rats by the orbital bleeding technique. The lesion affected a few adjoining lobules that were adjacent to ocular muscles, in contrast to the widespread inflammation of the Harderian gland which results from sialodacryoadenitis virus infection. Lesions resolved in 4 weeks by atrophy of acini in affected lobules with associated mononuclear cell inflammation and fibrosis.

The ultrastructural study of three malignant orbital tumors, only one of which was correctly diagnosed as embryonal rhabdomyosarcoma upon histologic examination, is reported. In all 3 cases electron microscopic investigation revealed the presence of rhabdomyoblasts. An unusual complication of repair of orbital floor blow-out fracture is reported. The supramid plate eroded partially through the orbital floor into the maxillary sinus creating a springboard-like mechanism. When the pressure in the maxillary sinus increased, the posterior portion of the supramid plate was pushed upward. This motion of the plate displaced the eye forward and somewhat outward as well as upward. Following the resection of the portion of the plate protruding into the orbit, the symptoms disappeared.

The orbital bones may show nearly all the pathologic changes observed in the skull and in the face. The congenital anomalies in this area are numerous and involve various forms of craniostenoses. Among the benign osseous tumors the osteoma is most frequently encountered in the orbit. Fibrous dysplasia is a tumefaction of indeterminate behavior that often involves the orbit. Osteosarcoma or other malignant neoplasms are rarely seen in this area. Eosinophilic granuloma and Hand-Schüller-Christian disease are tumor-like lesions that may involve the orbit.
