Genetic and phenotypic associations of social isolation and loneliness with sarcopenia.

Social isolation and loneliness are recognized risk factors for adverse aging outcomes, but their effects on sarcopenia remain understudied. This study investigated the independent and combined effects of social isolation and loneliness on the risk of incident sarcopenia in two large prospective cohorts: CHARLS (n = 8,856) and ELSA (n = 5,561), including participants aged 50 years and older free of sarcopenia at baseline. Sarcopenia was defined using AWGS2019 and the Revised European Consensus criteria. During mean follow-ups of 3.7 years (CHARLS) and 3.9 years (ELSA), 1,431 and 331 incident s

Expanding the phenotypic and genotypic spectrum of KCNT1-related epilepsies.

The KCNT1 gene encodes for a sodium-activated potassium channel involved in neuronal excitability. Since its initial description in 2012 in patients with Epilepsy of Infancy with Migrating Focal Seizures (EIMFS) and in Sleep-related Hypermotor Epilepsy (SHE), the associated phenotypic spectrum has broadened-encompassing other focal epilepsies and Developmental and Epileptic Encephalopathies (DEEs)-and has included extra-neurological features. We aimed to characterize the neurological outcomes, extra-neurological features, mortality and genotype-phenotype correlations expanding the follow-up of…

Clinicopathological phenotypes of singleton stillbirth: A retrospective cohort study.

INTRODUCTION: Interpreting the histopathology report after stillbirth and applying it to care in a subsequent pregnancy can be challenging. … CONCLUSIONS: Understanding major patterns of placental injury and their correlation with clinical phenotypes can help counseling after stillbirth. Stillbirths with placental MVM often had clinical signs suggesting a high-risk pregnancy. However, term stillbirths with placental VUE or FVM and umbilical cord at risk were commonly without recognized risk fac …

Dysferlinopathies: phenotypic study of a Moroccan series of 28 cases.

INTRODUCTION: Dysferlinopathies are a spectrum of autosomal recessive muscular dystrophies caused by mutations in the dysferlin gene (DYSF), leading to heterogeneous phenotypes, mainly limb-girdle muscular dystrophy type 2R (LGMDR2) and Miyoshi distal myopathy (MMD1). In Morocco, dysferlinopathies accounted for 18% of all limb-girdle muscular dystrophies, ranking second after sarcoglycanopathies. METHODS: We retrospectively analyzed 28 confirmed cases, diagnosed either by dysferlin deficiency or DYSF mutation. RESULTS: The median age at examination was 25,5 years (21-33). Twelve patients (43…

Inactive waitlist phenotypes and failure to survive to lung transplantation.

BACKGROUND: Inactive waitlist status is common in lung transplantation, but the extent to which distinct inactivity phenotypes identify candidates at risk of failing to survive to transplantation is uncertain. METHODS: We analyzed adult U.S. lung transplant candidates listed from January 1, 2005, through March 31, 2026, using OPTN STAR files. Inactivity was characterized by cumulative inactive days, first inactivation reason, and cycling. Logistic models evaluated death or removal for being too sick; sequential severity adjustment, multiple imputation, 90- and 180-day landmark competing-risk …

Phenotypic Spectrum of HNF4α-Associated Fanconi Renotubular Syndrome.

INTRODUCTION: Fanconi renal tubular syndrome 4 (FRTS4) is a very rare, autosomal dominant disease caused by heterozygous variants affecting the arginine residue at position 85 of the HNF4A gene. METHODS: We conducted an international, physician-based, anonymous data collection within the European Rare Kidney Disease Reference Network and beyond, focusing on clinical features, treatment, and the progression of chronic kidney disease (CKD) in patients with FRTS4. RESULTS: Overall, we collected data on 25 patients. Twenty-four of these carried the classic p.R85W variant, and 1 carried a new p.R

Insights on molecular drivers of phenotypic plasticity in Candidozyma auris.

Candidozyma auris (or Candida auris) is the first fungal species declared as a global health threat by the U.S. Center for Disease Control a … s that mannan-mediated masking of pathogen-associated molecular patterns (PAMPs) in the aggregative phenotype may contribute to persistence and reduced virulence in the host. Overall, C. auris can remodel its cell wall and elicit distinct host immune responses based on its form, opening avenues for novel therapeutic strategies which further require deeper e …

CUMS alters behavioral phenotypes and gut mycobiota in mice.

OBJECTIVE: To investigate the effects of chronic unpredictable mild stress (CUMS) on the gut fungal community in mice and its association wi … rgillus, Scopulariopsis and Wallemia may maintain intestinal homeostasis and alleviate anxiety-like phenotypes. These findings challenge the conventional view that fungal community dysbiosis is merely reflected by reduced diversity; instead, monotypic fungal dominance better mirrors pathological status. This work provides novel theoretical perspectives and …

Phenotyping of Heart Failure in CKD Using Electrocardiography Features.

KEY POINTS: Electrocardiography features have incremental value in prediction of heart failure in CKD beyond clinical features. Comparison and integration in existing risk scores is needed. BACKGROUND: Tools for predicting heart failure in patients with CKD remain limited. We aimed to study whether standard electrocardiography (ECG) features or heart rate variability parameters predict de novo heart failure hospitalization in individuals with CKD. METHODS: Using a large New York University ECG database linked with electronic health records (2012-2021), we analyzed a cohort of patients with p

Broadening the phenotypic and molecular spectrum of PRS deficiency in females.

Phosphoribosylpyrophosphate synthetase (PRS) deficiency is a rare X-linked disorder caused by variants in the PRPS1 gene. While males typically exhibit severe phenotypes, heterozygous females may or may not be affected, most likely explained by skewed X chromosome inactivation and its impact on enzyme activity. In this study, we describe and study both unique and previously described variants in PRPS1 in female patients. We provide detailed molecular and phenotypic information for two pediatric patients who possess unique variants in PRPS1, one of whom presents with bilateral tongue fasciculat…

MOGAD update: new phenotypes in the expanding clinical spectrum.

Myelin oligodendrocyte glycoprotein antibody associated disease (MOGAD) is often heralded by established clinical features, including optic neuritis (ON), transverse myelitis (TM), cerebral cortical encephalitis, acute disseminated encephalitis (ADEM), and cerebral, brainstem and/or cerebellar defects. Recent publications have revealed new clinical phenotypes that challenge perceptions regarding age of onset, disease progression, and neurologic recovery related to MOGAD relapses. In this review, we use a case-based format to highlight less typical disease manifestations of MOGAD that will like…
