Transscleral infrared laser for retinal ablation without retinal visualization in an experimental model.

PURPOSE: To investigate whether transscleral diode laser can create retinal photocoagulation reliably without creating retinal holes under conditions simulating opaque media. METHODS: In New Zealand pigmented rabbits, optimal infrared diode laser power settings were determined, and transscleral retinal photocoagulation was then applied 4 mm and 6 mm from the limbus without retinal visualization. Transscleral testing was done using retina and cyclophotocoagulation probes placed directly on the sclera, on conjunctiva, and on silicone scleral buckles. RESULTS: A retina probe placed on the scler

Incremental cost-effectiveness of laser therapy for choroidal neovascularization associated with histoplasmosis.

PURPOSE: Laser photocoagulation has been shown in a large clinical trial to be efficacious in reducing the degree of vision loss occurring secondary to choroidal neovascularization (CNV) associated with ocular histoplasmosis. Nevertheless, data are lacking concerning the impact of the therapy on quality of life and its value to stakeholders in health care. Recently, information concerning the utility value of visual states has become available. Accordingly, the authors undertook to ascertain the cost-effectiveness of laser photocoagulation for the treatment of extrafoveal CNV occurring in eyes

Choroidal melanoma microcirculation with confocal indocyanine green angiography before and 1 year after radiation brachytherapy.

PURPOSE: The authors previously demonstrated that confocal indocyanine green (ICG) angiography is capable of imaging the microcirculation of choroidal melanomas. The purpose of this study is to report their observations regarding the response of the microcirculation using confocal ICG at 1 year after radiation brachytherapy. METHODS: Thirteen patients with unilateral choroidal melanoma were examined once before and several times up to 1 year after radiation brachytherapy (Iodine-125 or Ruthenium-106 plaque). At each visit, a complete ophthalmologic examination including standardized echograph

Aqueous humor nitric oxide levels in patients with central retinal artery occlusion.

BACKGROUND/PURPOSE: There is increasing evidence suggesting that nitric oxide (NO) is implicated in the pathogenic process of ischemic disease. The purpose of this study was to quantify the aqueous humor NO levels in patients with central retinal artery occlusion (CRAO) and in age-matched controls. METHODS: Aqueous humor specimens were obtained from seven patients who had been diagnosed with CRAO; the sampling was done within 12 to 24 hours of diagnosis. All specimens were obtained by paracentesis, which was performed as a therapeutic intervention. Samples of aqueous humor were also collected

Ultrastructural and immunohistochemical findings in five patients with vitreomacular traction syndrome.

OBJECTIVE: To evaluate pathologic features of vitreomacular traction syndrome (VMT). METHODS: Preretinal membranes removed from five patients during vitreous surgery for VMT syndrome were evaluated by electron microscopy (n = 4) and immunohistochemistry (n = 1). RESULTS: Electron microscopic examination revealed large segments of internal limiting lamina (ILL) in three of the four cases. Other extracellular features included two types of abnormal collagen fibrils, determined to be type I and fibrous long-spacing collagen. The myofibrocyte was the predominant cell type in all cases. Collagen

Factors related to subretinal proliferation in patients with primary rhegmatogenous retinal detachment.

PURPOSE: To evaluate factors related to subretinal proliferation in patients with primary rhegmatogenous retinal detachment (RD). METHODS: A total of 262 consecutive patients (267 eyes) with rhegmatogenous RD were evaluated retrospectively to determine factors associated with subretinal proliferation. RESULTS: Logistic regression analysis was used to identify the following factors associated with subretinal proliferation that caused retinal traction: number of quadrants detached (adjusted odds ratio [AOR] = 2.21), longer duration of RD (AOR = 1.52), younger patient age (AOR = 0.51), and pres

Surgical treatment for severe diabetic macular edema with massive hard exudates.

PURPOSE: Massive diabetic macular exudates respond poorly to conventional laser treatment. The purpose of this study was to analyze the surgical results of eyes with massive hard exudates secondary to diabetic macular edema treated with combined pars plana vitrectomy, posterior hyaloid removal, focal endolaser treatment, and panretinal photocoagulation. METHODS: The author retrospectively analyzed the surgical outcome of 13 consecutive eyes (11 patients) with massive diabetic macular exudates. All patients had had at least one session of focal and/or grid laser treatment without any effect. P
