Autonomic markers of arousal during sleep in patients undergoing investigation for obstructive sleep apnoea, their relationship to EEG arousals, respiratory events and subjective sleepiness.

Estimating the degree of sleep fragmentation is an important part of a respiratory sleep study and is conventionally measured using EEG micro arousals or is inferred indirectly from respiratory abnormalities such as apnoeas and desaturations. There is a need for less labour-intensive measures of sleep fragmentation, and transient rises in blood pressure and heart rate may fulfil this role. Forty unselected sleep clinic referrals undergoing investigation for possible obstructive sleep apnoea (OSA) were studied with one night of polysomnography. Three conventional indices of sleep fragmentation …

Nocturnal assisted ventilation using bilevel positive airway pressure: the effect of expiratory positive airway pressure.

Increasing expiratory positive airway pressure (EPAP) has theoretical advantages during overnight nasal ventilation. We wanted to evaluate t … e (COPD) (FEV1 0.46 +/- 0.14 l, PaO2 6.2 +/- 0.6 kPa, and PaCO2 8.4 +/- 1.1 kPa) all underwent full polysomnography on two nights during bilevel positive airway pressure (BiPAP) ventilation, with and without the addition of expiratory positive airway pressure, which was matched to the level of dynamic positive end-expiratory pressure (PEEP) or set at a minimum va …

Role of hyperventilation in the pathogenesis of central sleep apneas in patients with congestive heart failure.

Periodic breathing with central apneas during sleep is typically triggered by hypocapnia resulting from hyperventilation. We therefore hypothesized that hypocapnia would be an important determinant of Cheyne-Stokes respiration with central sleep apnea (CSR-CSA) in patients with congestive heart failure (CHF). To test this hypothesis, 24 male patients with CHF underwent overnight polysomnography during which transcutaneous PCO2 (PtcCO2) was measured. Lung to ear circulation time (LECT), derived from an ear oximeter as an estimate of circulatory delay, and CSR-CSA cycle length were determined. P…

Dynamic thalamocortical functional connectivity disruptions in Parkinson's disease with probable REM sleep behavior disorder.

As the symptom of rapid eye movement sleep behavior disorder (RBD) is paroxysmal during sleeping, the disruptions of dynamic functional connectivity (FC) between thalamic subnuclei and the cortex may play a critical role in Parkinson's disease (PD). A total of 35 PD patients with probable RBD (PD-pRBD) and 40 PD patients without probable RBD (PD-npRBD) and 41 healthy controls were enrolled. All participants underwent functional magnetic resonance imaging scan and clinical assessment. Altered dynamic FC between bilateral 14 thalamic nuclei and cortex was calculated. Although the RBDSQ demonstra

Effectiveness of supervised multilevel oropharyngeal rehabilitation via videoconference on tongue function and apnea severity in obstructive sleep apnea: Quasi-experimental study.

INTRODUCTION: Obstructive sleep apnea (OSA) is marked by recurrent upper airway collapse during sleep and is associated with upper airway muscle dysfunction. Oropharyngeal rehabilitation (OPR) targets this dysfunction through structured muscle training. While supervised OPR improves adherence, its time and logistical demands may hinder access and increase dropout. OBJECTIVE: This study evaluated the effectiveness of a multilevel OPR program delivered via video conferencing on muscle function and the apnea-hypopnea index (AHI) in individuals with OSA. METHODS: This quasi-experimental study in

Kalinix®, an innovative, intelligent, non-invasive, and self-adaptive medical device for effective OSA management.

PURPOSE: Obstructive sleep apnea (OSA) is a rising prevalent disease mediated, in part, by the excessive relaxation of pharyngeal muscles that increases the vulnerability for pharyngeal collapse. At the therapeutic level, the reduced patient’s compliance of continuous positive airway pressure (CPAP) linked with the limitations of alternative OSA treatments, emphasizes the need for novel non-invasive and effective treatment options. METHODS: This real-world interventional clinical evaluation aims to explore the viability and short-term performance of Kalinix® in a clinically relevant OSA popula

SApneaNet: Adaptive Squeeze-and-Excitation-Based CNN-Transformer Network with AGFF for Sleep Apnea Event Detection Using ECG Images Under IoMT.

Background and Objective: Obstructive sleep apnea (OSA) is a fatal widespread sleep-related breathing disorder and a major risk factor for cardiovascular and cerebrovascular diseases, significantly impacting older adults' health worldwide. Due to the risk of such complications, timely and accurate identification of OSA is crucial. Polysomnography is considered the most accurate technique for detecting OSA; however, it is limited by its complexity and multi-channel requirements. A promising alternative is electrocardiogram (ECG)-based diagnosis, which continuously monitors heart rhythm and capt…

High-resolution sleep fragmentation assessment in narcolepsy type 1 and their non-narcoleptic siblings: a 5-s mini-epoch study.

STUDY OBJECTIVES: Narcolepsy type 1 (NT1) is characterized by fragmented sleep, yet brief transitions may be concealed in traditional 30-s epochs. We therefore aimed to assess sleep fragmentation in NT1 patients and their non-narcoleptic siblings using 5-s mini-epochs. METHODS: We analyzed sleep stages from human-scored 30-s epochs and automatically (U-Sleep) scored 5-s mini-epochs in polysomnographies from 125 NT1 patients and their 100 non-narcoleptic siblings. Sleep fragmentation was quantified using transition indices and probabilities in epochs and mini-epochs, furthermore, stratified by
