Tension-type headache and sleep apnea in the general population.

The main objective of this study is to investigate the relationship between tension-type headache and obstructive sleep apnea in the general population. The method involves a cross-sectional population-based study. A random age and gender stratified sample of 40,000 persons aged 20-80 years residing in Akershus, Hedmark or Oppland County, Norway were drawn by the National Population Register. A postal questionnaire containing the Berlin Questionnaire was used to classify respondents to be of either high or low risk of obstructive sleep apnea. Included in this study were 297 persons with high r

Central sleep apnea.

Central sleep apnea (CSA) is characterized by the periodic occurrence of apnea caused by loss of ventilatory motor output. CSA is often discussed as a minor variant of obstructive sleep apnea. However, this view obscures the critical contribution of CSA as an important manifestation of breathing instability in a variety of conditions with diverse causes. Central apnea can also be a physiologic phenomenon in healthy people during sleep onset. Conversely, patients who have obstructive apnea may also develop episodes of apparent central apnea, and apneas that begin as central may become obstructiv

Influence of cardiac function and failure on sleep-disordered breathing: evidence for a causative role.

Heart failure is an increasingly common public health problem that is strongly linked to both central and obstructive sleep apnea, collectively referred to as sleep-disordered breathing. Much attention has been given to the deleterious effects of sleep-disordered breathing on the failing heart and potential mechanisms by which treatment of sleep-disordered breathing may result in improved cardiac performance and long-term outcomes. However, there is compelling evidence that cardiac dysfunction may contribute to sleep-disordered breathing. Although there is recognized overlap between pathophysi

Effect of combined use of tolterodine and continuous positive airway pressure vs continuous positive airway pressure only treatment on overactive bladder symptoms in women with moderate-to-severe obstructive sleep apnea syndrome: a randomized clinical trial.

INTRODUCTION AND HYPOTHESIS: Obstructive sleep apnea syndrome is associated with urological symptoms, including overactive bladder (OAB). This study aims to determine whether combined tolterodine and CPAP therapies are more effective for patients with OSAS than CPAP treatment only. METHODS: Women who underwent polysomnography test and were diagnosed with moderate-to-severe OSAS with apnea-hypopnea index (AHI) were included in the study. Data were collected on AHI, OAB awareness-8-item tool (OAB-V8), incontinence questionnaire-urinary incontinence short form (ICIQ-UI-SF), total daily urine vol

[Complex sleep apnea syndrome].

Some patients with obstructive sleep apnea (OSA) develop persistent central sleep apnea activity when started on continuous positive airway pressure (CPAP). This recently recognized syndrome, complex sleep apnea syndrome (CompSAS), is thought to be due to dysfunction of the respiratory center in a setting of an obstructive sleep apnea. We present the current knowledge on epidemiology, pathophysiology, and treatment options in CompSAS. We review the current literature on CompSAS, and present our own data on this interesting condition.

Polygraphy in hospitalized pediatric patients: A real-life practice.

INTRODUCTION: The gold standard test for diagnosing sleep-disordered breathing is polysomnography; however, its limited availability has led to the emergence of alternatives such as polygraphy, which is more accessible and cost-effective. OBJECTIVE: To analyze the association between underlying conditions and obstructive sleep apnea-hypopnea syndrome in children with suspected sleep-disordered breathing. METHODS: Retrospective cross-sectional study. Polygraphy studies of hospitalized children aged ≥1 year with suspected sleep-disordered breathing were included. Demographic, clinical, and pol

Sleep apnea in amyloidosis.

Obstructive sleep apnea (OSA) occurs in patients with narrowing of the pharyngeal airway due to a variety of pathologic conditions. We documented obstructive and mixed sleep apnea in a patient with narrowing of the airway caused by macroglossia associated with amyloidosis. Tracheostomy relieved the symptoms of OSA (hypersomnolence, confusion upon awakening, and hyperkinesis during sleep) despite the emergence of striking central sleep apnea following the procedure.

Treatment of sleep apnea with prosthetic mandibular advancement (PMA).

Nine males with sleep apnea DOES syndrome and three males with sleep apnea DIMS syndrome were treated with prosthetic mandibular advancement (PMA). The method uses a prosthesis, which is designed to advance the mandible 3-5 mm to prevent upper airway occlusion during sleep. The apnea index in the obstructive-type apnea and the percentage of time spent in obstructive apnea decreased significantly with PMA. Although the apnea index showed merely a tendency to decrease in central apnea (p < 0.1), the percentage of time spent in central apnea decreased significantly with PMA. A marked improvement

Sleep-Disordered Breathing During Congestive Heart Failure: To Intervene or Not to Intervene?

Sleep-disordered breathing is common in heart failure patients and is associated with increased morbidity and mortality. Central sleep apnea occurs more commonly in heart failure-reduced ejection fraction, and obstructive sleep apnea occurs more frequently in heart failure with preserved ejection fraction. Although the two types of sleep-disordered breathing have distinct pathophysiologic mechanisms, both contribute to abnormal cardiovascular consequences. Treatment with continuous positive airway pressure for obstructive sleep apnea in heart failure has been well defined, whereas treatment st

Impact of untreated sleep apnea on prognosis of patients with congestive heart failure.

128 congestive heart failure (CHF) patients with a median age of 55 years and median left ventricular ejection fraction of 35.4% were followed up for a median period of 35 months. 23 (18%) had no sleep apnea (CHF-N), 55 (43%) had obstructive sleep apnea (CHF-OSA), and 50 (39%) had central sleep apnea (CHF-CSA). At the end of follow-up, mortality was greater in the CHF-CSA group than in the CHF-N group (18.2 vs 6.7/100 person-years, p=0.017). However, after adjusting age and the New York Heart Association functional class central sleep apnea, obstructive sleep apnea, or the severity of sleep ap…

Sleep measures and morning plasma TNF-alpha levels in children with sleep-disordered breathing.

BACKGROUND: Sleep disordered breathing in children is associated with severity-dependent increases in excessive daytime sleepiness (EDS). TNF-alpha is an inflammatory cytokine that has been implicated in EDS. Since, at any given level of apnea-hypopnea index, there is significant variability in EDS, we hypothesized that morning tumor necrosis factor (TNF)-alpha plasma levels may provide a biologic correlate of EDS. METHODS: Children being evaluated for sleep disordered breathing underwent a blood draw after nocturnal polysomnography, and TNF-alpha plasma concentrations were assayed using ELIS

Sleep apnea risk in transmasculine individuals on testosterone therapy: A population-based cohort study.

Importance: Testosterone affects airway anatomy and respiratory control, potentially increasing the risk of obstructive and central sleep apnea. While findings in cisgender men are mixed, studies of women with hyperandrogenic conditions, such as polycystic ovary syndrome, consistently demonstrate elevated prevalence. Given that sleep apnea is a major public health concern, its relationship to testosterone in transmasculine individuals warrants investigation. Objective: This study aims to assess whether testosterone therapy is associated with increased risk of obstructive and central sleep apne

Images: Caffeine therapy for central sleep apnea, hypoxemia, and hypoventilation in a term neonate.

UNLABELLED: The evaluation of higher-risk infants with brief resolved unexplained events and term infants with central sleep apnea can be clinically challenging due to the multitude of potential etiologies. We report a 7-day-old term neonate hospitalized for evaluation of brief resolved unexplained events with oxygen desaturations during sleep. Polysomnography showed central sleep apnea, hypoxemia, hypoventilation, periodic breathing, and mild obstructive sleep apnea. Following initial evaluations and while awaiting genetic testing, primary central sleep apnea of infancy was suspected and caff

Prevalence and treatment of breathing disorders during sleep in patients with heart failure.

Heart failure is a highly prevalent disorder, with significant economic impact, and is associated with excess morbidity and mortality. One factor that may contribute to the progressively declining course of heart failure is the occurrence of recurrent episodes of apnea and hypopnea. There are two major kinds of sleep-related breathing disorders: obstructive and central sleep apnea. In patients with heart failure, in contrast to the general population, central sleep apnea is the most common form of sleep-related breathing disorder. Episodes of apnea, hypopnea, and the subsequent hyperpnea cause

Effects of age on sleep apnea in men: I. Prevalence and severity.

The effects of age on the prevalence of sleep apnea in the general population remain unclear, because previous studies have focused on specific populations. The effects of age on the severity of apnea are unknown. This study was based on a two-stage general random sample of men (aged 20 to 100 yr), consisting of a telephone survey (n = 4,364) and a sleep laboratory evaluation of a survey subsample (n = 741). Obstructive sleep apnea (OSA), based on both sleep laboratory and clinical criteria (apnea/hypopnea index [AHI] > or = 10 and the presence of daytime symptoms) was found in 3.3% of the sam
