Subacute Combined Degeneration of the Spinal Cord From Nitrous Oxide Abuse.

INTRODUCTION: The recreational use of nitrous oxide is surging in popularity, with nearly 13 million Americans aged 12 and older reported to have misused it, according to a 2019 survey by the Substance Abuse and Mental Health Services Administration. Prolonged use has been linked to significant neurological deficits, potentially leading to lifelong issues if not treated early. CASE PRESENTATION: We present a case of a 38-year-old male with significant neurologic deficits attributed to prolonged nitrous oxide abuse, resulting in subacute combined degeneration of the spinal cord. This condition

Agency, sex and drug education: Examining the response-ability of education responses to consumption, sex and harm.

This article examines how drug education professionals understand and respond to the relationship between alcohol and other drug consumption, sex and harm. While recent research examines how these issues are addressed in drug education curriculum, little is known about the perspectives of professionals involved in education design and delivery. Research suggests that agency is centrally important for understanding experiences of harmful, pleasurable or ambiguous sexual encounters in consumption settings. I analyse understandings of the relationship between agency, drug consumption, sex and har

Identifying barriers and facilitators to accessing harm reduction services in prisons: A systematic narrative synthesis.

BACKGROUND: Drug use is prevalent in carceral settings globally. Although a comprehensive harm reduction package for people in prison (PIP) is recommended by international agencies, its implementation is limited. The aim of this systematic narrative synthesis was to explore barriers and facilitators to accessing harm reduction services (HRS) in prisons. METHODS: We searched MEDLINE, PsycINFO, SCOPUS, and CINAHL for English and French language articles published before June 26, 2024. Studies evaluating any HRS defined as condoms, pre-exposure prophylaxis, post-exposure prophylaxis, opioid agon

Impact of alcohol consumption, substance use, and smoking on treatment outcomes in tuberculosis: a systematic review and meta-analysis.

BACKGROUND: This study aimed to elucidate the influence of alcohol, smoking, and substance use on tuberculosis (TB) treatment failure using a meta-analysis approach. METHOD: A comprehensive search strategy was developed and applied to three major databases: MEDLINE, Web of Science, and Scopus. Additionally, Google Scholar, and Google were used to locate grey literature. Studies were identified through title and abstract screening, followed by a full-text review for eligibility. The Newcastle-Ottawa Scale checklist was employed to assess the quality of included studies. Pooled odds ratios (OR)

Putting tendencies and trajectories to work: useful tools for engaging with accounts of change and recovery?

Dominant understandings of recovery emphasise personal responsibility for initiating and sustaining changes in people's subjectivities and relationships to alcohol and other drugs. However, this potentially obscures the complexities and temporalities of change processes and the range of socio-material elements involved. Addressing this gap, critical drug studies scholars have productively employed the concepts of tendencies and trajectories to analyse how past events of drug consumption flow into current and future consumption events. Critiquing notions of personal responsibility within recove

Effect of integrated hepatitis C virus treatment on psychological distress in people with substance use disorders.

People with substance use disorders (SUD) have a high prevalence of chronic hepatitis C virus (HCV) infection and mental health disorders. We aimed to assess the impact of integrated HCV treatment on psychological distress measured by Hopkins-symptom-checklist-10 (SCL-10). This multi-center randomized controlled trial evaluated psychological distress as a secondary outcome of integrated HCV treatment (INTRO-HCV trial). From 2017 to 2019, 289 participants were randomly assigned to receive either integrated or standard HCV treatment with direct-acting antiviral therapy. Integrated HCV treatment

A Comparison of Two Statewide Datasets to Understand Population Prevalence of Substance Use in Pregnancy: Findings and Considerations for Policy & Research.

Mental health conditions including substance use disorder are the leading cause of pregnancy-related deaths in the U.S. Unfortunately, fears of child protective services' involvement interfere with maternal self-disclosure of substance use in pregnancy. Seeking to identify more mothers with substance use disorder in pregnancy or at delivery, and responsive to changes to the federal Child Abuse Prevention and Treatment Act (CAPTA), Connecticut requires hospital personnel to submit a deidentified notification to CPS for all newborns with prenatal substance exposure. However, it is unknown whethe

The multiple faces of footshock punishment in animal research on addiction.

Continued drug use despite negative consequences is a hallmark of addiction commonly modelled in rodents using punished drug intake. Over the years, addiction research highlighted two subpopulations of punishment sensitive and resistant animals. While helpful to interrogate the neurobiology of drug-related behaviors, these procedures carry some weaknesses that need to be recognized and eventually defused. Mainly focusing on footshock-related work, we will first discuss the criteria used to define punishment-resistant animals and how their relative arbitrariness may impact our findings. With th

Recovery Coach Program Implementation Across an Integrated Health System.

Peer recovery coaches (PRCs) are increasingly playing a role in helping patients with substance use disorders engage with treatment. PRCs can support and motivate patients in meeting their self-defined recovery goals, engaging in addiction treatment, navigating the health care system, and overcoming barriers to recovery. This support increases patient engagement and is cost-effective. Little has been written about integrating PRCs in health care settings. In this column, the authors describe the implementation of a PRC program with 23 coaches serving 5,662 participants in diverse clinical sett

Addressing the needs for cultural adaptation of DARE-keepin' it REAL among Brazilian students: strategies to improve implementation.

BACKGROUND: The widely implemented prevention program in Brazil, PROERD (a translated version of the DARE-kiR program), showed no evidence of effect in preventing drug use in a recent trial. The lack of cultural adaptation and instructors' deviations from the curriculum were identified as potential reasons for its ineffectiveness. This study aims to identify points of inadequacy of the PROERD program for the Brazilian culture, suggesting strategies for revising the curriculum to improve implementation. METHODS: We triangulated data obtained from three different groups of study participants: s

Racism-related experiences and substance use: A systematic and meta-analytic review.

Examinations highlighting interpersonal racism-related experiences as risk factors for substance use are well documented, particularly for alcohol use. The associations between racism-related experiences across other levels of influence (e.g., historical trauma, online, internalized) and use of other types of substances, while emerging, have yielded mixed findings. The present systematic review and meta-analyses examined the associations between multilevel racism-related experiences and different types of substances including substance use overall, alcohol, binge drinking, tobacco/nicotine, ca

A scoping review of rural mental health and substance use nursing.

INTRODUCTION: Globally, nearly 50% of the population live in rural areas, while just 36% of nurses serve in these locations. Rural nurses face distinct challenges such as limited resources and geographical isolation, and often work with an expanded scope of practice that includes mental health and substance use (MHSU) care. The extent to which rural nurses engage in MHSU care, care barriers, and facilitators has not been previously well described. Thus, this scoping review explored the international research on rural MHSU nursing. The aim was to synthesize the rural MHSU nursing evidence and c

Disentangling developmental trajectories of childhood anxiety and depression symptoms in the prediction of adolescent substance use.

Adolescent substance use is comorbid with internalizing problems (e.g., anxious and depressive symptoms) and externalizing problems (EPs). Developmental researchers have linked early anxious and depressive symptoms to later substance use behaviours via an "internalizing pathway". Support for this pathway exists among young adults; however, evidence from prospective studies of adolescents has been inconsistent. Given the paucity of prospective studies in this area, the current study used a developmental framework to examine childhood anxiety and depression symptom trajectories as predictors of

Family-centred interventions for people with substance use disorders in low-income and middle-income country settings: a scoping review protocol.

INTRODUCTION: Substance use disorder (SUD) and problematic substance use are global public health concerns with significant multifaceted implications for physical health and psychosocial well-being. The impact of SUD extends beyond the individual to their family while imposing financial and social burdens on the community. Though family-centred interventions have shown promise in addressing SUD, their implementation and impact in low-income and middle-income countries (LMICs) remain underexplored. METHODS AND ANALYSIS: Per Joanna Briggs Institute's scoping review protocol, a systematic search

Strategies to improve delivery of equitable and evidence-informed care for pregnant and birthing people with a substance use disorder in acute care settings: A scoping review protocol.

This protocol outlines a proposed scoping review to characterize evidence on implementation and quality improvement (QI) strategies that aim to improve equitable, evidence-informed care delivery for pregnant and birthing people with substance use disorder (SUD) in acute care. Untreated SUD during pregnancy is associated with an increased risk of overdose and severe maternal morbidity. Acute care settings are one important place to deliver equitable, evidence-informed clinical care. While clinical practice guidelines for substance use treatment and care of pregnant and birthing people with SUD

'But … Would I Be Able to Toast With Friends?' When Service Users Ask for New Care Pathways.

INTRODUCTION: The WHO European Mental Health Action Plan (2013-2030) emphasises the need to generate services that are more inclusive and attentive to the co-construction of care practices. This exploratory research investigates the needs of young substance abusers shown during their stay in residential communities; in particular, it explores the idea that treatment may include a new phase focused on how to manage moderate or controlled alcohol intake during residential care. Interviews with young ex-users open up critical reflections on complete abstinence programmes from all substances, incl

Residential and inpatient treatment of substance use disorders in Sub-Saharan Africa: a scoping review.

BACKGROUND: With substance use rates increasing in Sub-Saharan Africa (SSA), an understanding of the accessibility and effectiveness of rehabilitative services for people who use alcohol and other drugs (AOD) is critical in the global efforts to diagnose and treat substance use disorders (SUD). This scoping review seeks to address the gaps in knowledge related to the types of research that have been conducted regarding inpatient or residential SUD treatment in SSA, the settings in which the research was conducted, and the study countries. METHODS: A search of three databases, PubMED, Scopus,

Patterns of cannabis use, perception of harm, and perceived impact of legislative change in an online sample of young adults from Lebanon: insight on recreational users versus dual motive users.

BACKGROUND: Lebanon remains as one of the major sources of cannabis worldwide. In 2020, its government passed a legislation enabling the cultivation of local medicinal cannabis. This first study following the legislative change examines the overlapping use of cannabis for recreational/medicinal purposes and characteristics of the distinct cannabis user types. METHODS: A total of 1230 young adults (18-24 years) filled an anonymous online survey in early 2020. RESULTS: Young adults in the sample were distributed as follows: 33% 18-20 years; 60% males; 94% Lebanese; 75% students; and 89% living

Identifying Three Psilocybin Use Patterns by Frequency and Quantity.

OBJECTIVE: Patterns of psilocybin use in nonclinical settings are not well described in the literature. Psilocybin use can involve infrequent, large (i.e., macro) doses that produce hallucinogenic effects. In addition, some people report psilocybin use at particularly small (i.e., micro), sub-perceptual doses. Given the heterogeneity in reported use metrics, we sought to determine whether there are identifiable patterns of psilocybin use based on participants' self-described typical use frequencies and quantities and to describe how demographic characteristics are associated with each pattern
