Editorial Commentary: Endoscopic and Open Techniques Can Achieve Successful Outcomes With Hip Abductor Tendon Repair but Retracted Tears With High-Grade Fatty Atrophy May Benefit From an Open Approach.

The gluteus medius and minimus tendon unit are commonly referred to as the "rotator cuff of the hip." Tears of the gluteus medius and/or minimus can range from interstitial, partial-thickness tears to retracted, full-thickness tears, and may result from trauma but are more commonly associated with chronic degeneration. Patients commonly present with lateral hip pain exacerbated by weight-bearing and sleeping on the affected side. In patients with persistent lateral hip pain, despite nonoperative management, both open and endoscopic gluteal tendon repair techniques have demonstrated excellent s

Regeneration of full thickness common extensor tendon tear after percutaneous microfragmented adipose graft.

Tennis elbow, also commonly known as lateral epicondylitis or common extensor tendinosis, is a common musculoskeletal injury in the adult population. Currently, the standard treatment regimen prescribed for this injury involves a combination of rest, physical therapy, bracing and anti-inflammatory medications. If refractory to these conservative measures, platelet-rich plasma has been shown effective. However, in the case of full thickness tears, surgery has remained the only treatment option until now. We present a case report of a 56-year-old man with a diagnosis of a left large full thickne

Serial application of meshed collagen-chondroitin silicone bilayer matrix to obtain full coverage over bone and tendon in challenging situations and medically compromised patients: a small case series.

INTRODUCTION: Soft tissue defects in medically compromised patients present significant challenges to the reconstructive surgeon, particularly when vital structures are exposed. This case series reports clinical outcomes of 5 adult patients with challenging medical problems whose wounds were managed using a meshed collagen-chondroitin silicone bilayer matrix to obtain coverage over bone and tendon. MATERIALS AND METHODS: Patient 1 had significant degloving of the scalp. Patient 2, who had a giant neglected tumor, had a defect comprising the entire occipital skull. Patients 3 and 4 had necroti

A Simple and Cost-effective Approach to Make a Modified Suture Anchor in Orthopaedics.

The purpose of this study is to describe a technique of using an affordable suture anchor for various soft tissue repairs of both upper and lower limb surgeries in our series of patients. Eighteen patients with minimum 1-year follow-up after various upper limb surgeries using modified suture anchor were assessed both clinically and radiographically. In all 18 cases, the purpose of the suture anchor was served in terms of attachment of ligament or the tendon. There was no incidence of breakage of suture material or loosening of the implant seen during the insertion. There were no complications

Methods to analyse the long head of the biceps in the management of distal ruptures of the supraspinatus tendon. Part 1: the concept of the "biceps box": dynamic rotator interval approach. Incidence of lesions of the long head of the biceps tendon.

INTRODUCTION: The area encompassing the long head of the biceps (LHB) can be represented as a rectangular parallelepiped. This geometric view can be likened to a box, the "biceps box", where the sides are the extrinsic structures and the LHB is the intrinsic structure. Since these structures are mobile in relation to each other, a dynamic "biceps box" model can modify assessments of the LHB, in its healthy or pathological state, and make the therapeutic approach to treating LHB lesions less arbitrary. MATERIAL AND METHOD: In order to describe the different sides of the "biceps box", and to un

Transarterial Embolization for Refractory Overuse Sports Injury: Pictorial Case Reports.

Transarterial embolization for the treatment of chronic musculoskeletal diseases is gaining increasing interest in the field of interventional radiology. Overuse sports injury is defined as an injury occurring in the absence of a single, identifiable traumatic cause. In the treatment of this condition, there is a need for reliable results and a quick return to activity. Minimally invasive treatments with short periods of missed practice are required. Intra-arterial embolization has the potential to meet this need. In this article, we describe cases of embolization for refractory overuse sports

Arthroscopic repair of massive rotator cuff tear. The role of the LHB distal tenotomy.

PURPOSE: The aim of the study was to evaluate the results of an all arthroscopic technique for the treatment of massive rotator cuff tears using the long head of biceps as a graft to reconstruct the superior capsule and to reinforce the cuff. METHODS: A retrospective review of a consecutive series of arthroscopic repair of massive rotator cuff tears using the long head of biceps tendon was conducted. Twenty-five patients underwent surgery, and none were lost at follow-up. Minimum follow-up period was more than 12 months. Constant, UCLA and VAS scores as clinical outcome were analyzed. Time fo

Tendon transfer for trapezius palsy.

The trapezius muscle produces upward scapular rotation that in turn allows complete lateral elevation (abduction) by maintaining the acromiohumeral distance and the deltoideus resting length. Loss of trapezius function leads to shoulder drooping, loss of scapular external rotation with secondary loss of abduction. When conservative treatment has failed and in cases where nerve surgery is not indicated, the most common procedure for treating this condition is the Eden-Lange (EL) procedure. This procedure entails transferring the levator scapulae (LS) to the lateral part of the scapular spine, a

Chronic Disorders of the Peroneal Tendons: Current Concepts Review of the Literature.

Chronic disorders of the peroneal tendons are a common cause of posterolateral ankle pain, including tendinopathy, tendon instability, and chronic tendon tears. They are often preceded by ligamentous instability or predisposing anatomic abnormalities such as a shallow fibular groove or a cavovarus foot deformity. Given the substantial disability associated with chronic peroneal tendon disorders, it is important for orthopaedic surgeons to optimize the diagnostic and treatment strategies of these entities based on contemporary studies. This article reviews both classic and recent scientific evi

The effectiveness of manipulation of fingers with Dupuytren's contracture 7 days after collagenase clostridial histolyticum injection.

Timing of manipulation of digits after collagenase injection for Dupuytren's disease varies and often takes place within the first few days post-injection. We prospectively investigated the effectiveness of performing manipulation under local anaesthesia 7 days after injection in 100 patients. Demographic data, passive extension deficit, and patient-reported outcome measures were recorded before collagenase injection. Four to 7 weeks after manipulation, passive extension deficit and patient-reported outcome measures improved significantly without the development of any tendon ruptures. Clinica

Editorial Commentary: Finally, Something Positive About the Long Head of the Biceps Tendon?!-Shoulder Superior Capsular Reconstruction.

Massive irreparable rotator cuff tears without glenohumeral degeneration in young and active patients remain a challenging clinical problem for shoulder surgeons. Superior capsular reconstruction with fascia lata autograft or dermal allograft has recently become a popular treatment option. The long head of the biceps tendon, which has otherwise widely fallen into disfavor with shoulder surgeons, has recently emerged as an interesting alternative graft choice for superior capsular reconstruction because of its local availability. However, the literature on techniques using the biceps tendon for

Rupture of the extensor hood of the fifth toe: a rare injury.

Closed injuries of the extensor hood of the lesser toes are rare and seldom reported in the literature. We present the case of a woman aged 25 years who presented to the orthopaedic fracture clinic with a 2-week history of pain in the left fifth toe and inability to extend following a ballet dancing session. Investigations showed no fracture on plain radiographs, but an ultrasound scan demonstrated rupture to the extensor hood of the little toe. Successful surgical repair of the extensor hood was performed, and the patient made a good recovery with return to dancing activities.

Biomechanical comparison between the modified rolling-hitch and the modified finger-trap suture techniques.

PURPOSE: The purpose of this study was to characterize the biomechanical effect of two grasping suture techniques used during ligament reconstruction: the modified rolling-hitch (MRH) and the modified finger-trap (MFT). METHODS: Flexor profundus tendons were harvested from fresh pig hind-leg trotters. Each specimen was mounted on an electro-mechanic universal testing machine (Instron 3367). In half of all tendons (15 specimens), the suture was passed around the tendon following the MRH knot (Group 1). In the remaining half of all tendons (15 specimens), the suture was passed over a distance o

Efficacy of eccentric exercise in lower limb tendinopathies in athletes.

Tendinopathies are among the most frequent sport injuries, therefore their correct treatment is a crucial issue in sports medicine practice. In most of the cases, these multifactorial conditions are related to overuse and characterized by activity-induced pain, local tenderness and swelling. Although tendinopathies are common, their treatment is not easy. Currently, it is generally accepted that their management should include early functional exercises. Eccentric exercise (EE) is considered a fundamental therapeutic resource, especially for the treatment of Achilles and patellar tendinopathie
