Transplant Renal Artery Stenosis Presenting as Posterior Reversible Encephalopathy Syndrome in a Post-Kidney Transplant Patient: A Case Report.

INTRODUCTION: Transplant renal artery stenosis (TRAS) is a reversible cause of severe hypertension and graft dysfunction. Posterior reversible encephalopathy syndrome (PRES) in kidney transplant recipients is commonly attributed to calcineurin inhibitor (CNI) toxicity; however, the biochemical signatures of CNI toxicity and TRAS differ markedly. CASE PRESENTATION: We report a 32-year-old woman, 5 months post-transplant, presenting with seizures, severe hypertension, and MRI-confirmed PRES. She exhibited hypokalaemia (2.6 mmol/L) and metabolic alkalosis (pH 7.50, bicarbonate 34 mmol/L) - findi

Allogeneic Hematopoietic Stem Cell Transplantation Despite Severe Fusarium solani Infection in a Lung Transplanted Patient-A Case Report.

Solid-organ transplant patients require prolonged immunosuppression, increasing their risk of hematologic disorders. For these conditions, allogeneic hematopoietic stem cell transplantation (HSCT) is a potential treatment, but it carries significant risk of treatment-related mortality due to the high possibility of developing rare infectious complications. We report a case of a 55-years-old male with a history of bilateral lung transplantation for extrinsic allergic alveolitis in 2015, who developed acute myeloid leukemia/myelodysplastic syndrome (AML/MDS) with TP53 mutation seven years later.

Association between pre-transplant CONUT score and pneumonia risk after allogeneic stem cell transplantation in acute leukemia.

BACKGROUND: The Controlling Nutritional Status (CONUT) score has been proposed as a simple tool for assessing nutritional and immunological status and has been associated with prognosis in various malignancies. This study aimed to assess the relationship between pre-transplant CONUT scores and pneumonia development within one year after allogeneic stem cell transplantation (allo-SCT) in patients with acute leukemia. METHODS: In this retrospective single-center study, 158 patients who underwent allo-SCT for acute leukemia between 2013 and 2023 were included. Patients who developed pneumonia dur

The Impact of Isolated HLA-DQB1 Mismatch and Donor Age in Unrelated Donor Hematopoietic Cell Transplantation with Post-Transplant Cyclophosphamide.

In unrelated donor hematopoietic cell transplantation (HCT) with post-transplantation cyclophosphamide (PTCy), the clinical relevance and interplay between an isolated HLA-DQB1 mismatch and donor age remain unclear. We conducted a single-center retrospective analysis of 988 consecutive patients with hematologic malignancies undergoing a first unrelated donor HCT with PTCy prophylaxis between 2017 and 2024. We compared outcomes among recipients of 10/10 matched unrelated donors (MUD) (10/10-MUD; n = 854), 8/8 MUD with an isolated HLA-DQB1 mismatch (n = 47), and 7/8 mismatched unrelated donors (

Outcomes of urgent lung transplantation in critically ill patients versus standard lung transplantation: A systematic review and meta-analysis.

BACKGROUND: Whether survival differs between urgent lung transplantation (ULTx) and standard lung transplantation (LTx) remains unclear. This systematic review and meta-analysis aimed to evaluate survival and other post-transplant outcomes between ULTx and standard LTx. METHODS: PubMed, Embase, and Cochrane Library were searched up to July 31, 2024 for relevant studies. A meta-analysis of baseline characteristics and postoperative outcomes was then performed, with subgroup analyses by study designs and indications. Overall survival (OS) was set as the primary outcome in this study. Risk ratio

Clinical outcomes of renal transplant recipients with and without post-transplant post-infectious glomerulonephritis: a propensity score-matched analysis.

BACKGROUND: Post-infectious glomerulonephritis (PIGN) is a rare but potentially serious cause of kidney injury following infection. This study aimed to compare long-term renal outcomes and comorbidities between kidney transplant recipients who underwent allograft biopsy with and without a diagnosis of PIGN. METHODS: We retrospectively identified patients from a large global multicenter database. De novo PIGN patients were matched 1:1 with non-PIGN patients using propensity score matching based on age, sex, body mass index, systolic blood pressure, comorbidities, and baseline kidney function.

The impact of second allogeneic hematopoietic stem cell transplantation as salvage therapy for hematologic diseases after a first allogeneic transplantation.

BACKGROUND AND OBJECTIVES: Acute leukemia patients who relapse after the first allogeneic stem-cell transplantation (HSCT1) have a poor prognosis. Second allogeneic hematopoietic stem-cell transplantation (HSCT2) is a therapeutic option for patients with acute myeloid leukemia (AML)/acute lymphoblastic leukemia (ALL) relapsing after HSCT1. Our aim is to evaluate the efficacy of HSCT2 in acute leukemia patients who relapsed after HSCT1. MATERIAL AND METHODS: In the current study, we retrospectively analyzed the data of 72 patients who underwent HSCT2. Forty-six patients with AML and 26 patient

A novel subcutaneous islet transplantation method using a bioabsorbable medical device to facilitate the creation of a highly vascularized transplantation site.

Subcutaneous transplantation is garnering attention as a potential transplantation site for pluripotent stem cell-derived islet cells to address the shortage of pancreatic islet transplant donors. However, subcutaneous transplantation of cells presents challenges related to angiogenesis, which is necessary for successful islet bioproduction. This study aimed to investigate a novel method for enhancing vascularization at the transplant site and thereby promote islet engraftment using a clinically available bioabsorbable medical device. A nonabsorbable device (agarose) or a bioabsorbable device

Evaluation of Immunosuppression Levels and Risk of Graft-Versus-Host Disease in Allogeneic Blood or Marrow Transplantation With Post-Transplantation Cyclophosphamide.

Post-transplantation cyclophosphamide (PTCy) is standard graft-versus-host disease (GVHD) prophylaxis for allogeneic blood or marrow transplantation (alloBMT), although optimal therapeutic levels of immunosuppression (IS) therapy combined with PTCy remain contested. Previously, with tacrolimus and methotrexate GVHD prophylaxis, week 1 tacrolimus levels >12 ng/mL were associated with a decreased incidence of grade 2 to 4 acute GVHD (aGVHD). We evaluated if associations between aGVHD and early IS levels were observed amongst patients receiving PTCy. This retrospective single-center study consist

Redefining Engraftment Syndrome After Post-Transplant Cyclophosphamide Allogeneic Hematopoietic Cell Transplantation: A Novel Classification and Impact on Outcomes.

Engraftment syndrome (ES) is a non-infectious febrile complication of hematopoietic cell transplantation (HCT), with diagnostic challenges, particularly in the allogeneic setting. The increasing use of post-transplant cyclophosphamide (PTCy) for graft-versus-host disease prophylaxis highlights the need for a re-examination of ES in this contemporary context. To evaluate the incidence and clinical presentation of ES, as well as its impact on transplant outcomes in the era of PTCy-based prophylaxis. We retrospectively analyzed 552 allogeneic HCT patients receiving PTCy, sirolimus, and mycophenol

Bridging transplantation and immunotherapy: Clinical promise of autologous stem cell transplantation with chimeric antigen receptor T-cell therapy.

Autologous stem cell transplantation (ASCT) and chimeric antigen receptor T-cell (CAR-T) therapy represent pivotal treatments for hematologic malignancies, each with distinct strengths and limitations. ASCT reduces tumor burden through myeloablative conditioning but remains susceptible to relapse, while CAR-T therapy precisely targets malignant cells but encounters challenges, including cytokine release syndrome (CRS), immune effector cell-associated neurotoxicity syndrome (ICANS), and limited persistence. Emerging evidence suggests that combining ASCT with CAR-T therapy yields synergistic eff

Acquired right-sided diaphragmatic hernia after pediatric living donor transplantation: a guide for the non-transplant pediatric surgeon.

BACKGROUND: Pediatric living donor liver transplantation (pLDLT) has risen to standard of care for children with liver failure. Strong policies of centralization have led to exceptional results, reducing the risk of morbidity and mortality. As a counterpart, long term follow-up often occurs in centers with no dedicated transplant unit, that should familiarize themselves with the exceptional anatomy of the liver-transplanted child and their unique postoperative complications. METHODS: We reviewed all cases of acquired diaphragmatic hernia (DH) after pLDLT in the period following the establishm

Waitlist and Post-Transplant Outcomes in Liver Transplantation for Alcohol-Associated Liver Disease: An Individual Patient Data Meta-Analysis.

BACKGROUND AND AIMS: Alcohol-associated liver disease (ALD) is a rapidly rising indication for liver transplantation. Early studies showed comparable post-transplant outcomes but emerging data suggest long-term survival benefits in ALD, thus warranting an analysis of waitlist and post-transplant outcomes. METHODS: Medline and Embase were searched from inception to 16 May 2025. Kaplan-Meier curves were extracted to reconstruct individual participant data for pooled survival analysis of time-to-event outcomes using a random effects model. Cox regression-generated hazard ratios (HRs) for surviva

Successful Unmanipulated Haploidentical Hematopoietic Stem Cell Transplant With Post-transplant Cyclophosphamide in a Child With Down Syndrome and Myelodysplastic Syndrome.

Allogeneic hematopoietic stem cell transplant (HSCT) in children with Down syndrome and hematologic malignancies is challenging and is reserved for those who develop relapsed/refractory disease due to concerns regarding transplant-related mortality. Haploidentical HSCT, although performed in limited cases using graft manipulation methods to prevent graft-versus-host disease (GVHD), often results in dismal outcomes. Herein, we report a case of a 12-year-old boy with Down syndrome and myelodysplastic syndrome who underwent unmanipulated haploidentical HSCT using a reduced toxicity treosulfan-bas
