Surgical management of chronic pulmonary embolism.

Recurrent pulmonary emboli ultimately may produce respiratory insufficiency, severe hypoxemia, and progressive pulmonary hypertension. In many patients this syndrome is silent in its initial phases, and when thrombophlebitis is present it is often unresponsive to anticoagulant therapy. Unless pulmonary embolectomy is undertaken, most of these patients characteristically succumb with severe respiratory insufficiency. Twenty-five patients with this syndrome have been evaluated at the Duke University Medical Center, and 14 were selected for elective pulmonary embolectomy for relief of severe and

First-in-Human Clinical Experience Using High-Definition Exoscope with Intraoperative Indocyanine Green for Clip Reconstruction of Unruptured Large Pediatric Aneurysm.

The operative exoscope is a novel tool that combines the benefits of surgical microscopes and endoscopes to yield excellent magnification and illumination while maintaining a comparatively small footprint and superior ergonomic features. Until recently, current exoscopes have been limited by 2-dimensional viewing; however, recently a 3-dimensional (3D), high-definition (4K-HD) exoscope has been developed (Sony-Olympus, Tokyo, Japan).1 Our group had previously described the first in-human experiences with this novel tool including microsurgical clipping of intracranial aneurysms. We have highli

The feasibility of spleen-preserving pancreatectomy for end-stage chronic pancreatitis.

BACKGROUND: It is considered difficult to preserve the spleen at the time of distal or total pancreas resection for chronic pancreatitis (CP). The aim of this study was to assess the feasibility of preserving the spleen in patients requiring total or completion pancreatectomy for CP. METHODS: All patients having total or completion pancreatectomy for CP were evaluated postoperatively in terms of morbidity, mortality, and pain relief. To assess splenic vascularity, all patients underwent abdominal ultrasound and power doppler imaging to assess splenic perfusion and the patency of the remaining

Endovascular Treatment of Abdominal Aorto-Caval Fistula With Occluder Devices: Case Report and Systematic Literature Review.

Aortocaval fistula (ACF) is a life-threatening condition secondary to abdominal aortic aneurysms (AAA) rupture or previous trauma/intervention. The treatment of ACF by an occluder device deployment is a rare but increasingly common approach. We report a case of ACF secondary to ruptured AAA treated with an occluder device after endograft deployment. A 66-year-old male was treated in an emergent setting for a ruptured AAA with ACF deploying aorto-bi-iliac endograft. At 3-month computed tomography angiography (CTA), the persistence of aorto-caval communication and the increased sac reperfusion (

Right non-bifurcating cervical carotid artery with a long-segment incomplete carotid bifurcation, a right accessory middle cerebral artery arising from a common trunk with a cortical branch at the distal A1 segment.

INTRODUCTION: Both non-bifurcating cervical carotid artery (NBCA) and accessory middle cerebral artery (aMCA) have been reported previously. Here, we report a rare case of right NBCA with a long-segment incomplete carotid bifurcation, associated with a right aMCA arising from a common trunk with a cortical branch at the distal A1 segment. METHODS: The patient was a 70-year-old woman who was transferred to our department from another hospital for further evaluation and treatment after an anterior communicating artery aneurysm had been identified. No obvious neurological abnormalities were found

A stochastic, physiology-based digital twin model of hemostasis and oxygenation in trauma resuscitation.

Hemorrhagic shock is a common emergency that accounts for more than 10% of global mortality and up to 40% of trauma-related mortality. The Advanced Trauma Life Support (ATLS) guidelines outline resuscitation strategies in patients with massive hemorrhage based on clinical trial and observational data. Early intervention with fluids and/or blood products is recommended during resuscitation as key to maintaining vascular patency, stabilizing a patient's blood pressure, maintaining tissue oxygenation, and limiting shock. From a transfusion perspective, there is wide heterogeneity in the quality o…

Circadian Control of Neutrophils Drives Collateral Perfusion and Stroke Outcome.

BACKGROUND: Stroke remains a leading cause of mortality and disability, driven by complex, time-dependent mechanisms that aggravate ischemic injury. Collateral perfusion dictates infarct size, expansion rate, and penumbral preservation, yet its regulation is poorly understood. Beyond structural/genetic factors such as aging or cardiovascular risk, functional influences like circadian immune activity may also affect vascular patency. Neutrophils, key mediators of ischemic injury, exhibit circadian oscillations in phenotype and function that could modulate collateral flow and stroke outcome. ME…

The CHA2DS2-VASc Score as an Early Predictor of Graft Failure After Coronary Artery Bypass Surgery.

Background and objective Graft patency is one of the major concerns after coronary artery bypass graft (CABG) surgery. The CHA2DS2-VASc [congestive heart failure, hypertension, age ≥75 years, diabetes mellitus (DM), stroke or transient ischemic attack (TIA), vascular disease, age 65 to 74 years, sex category] score is a tool that was developed to predict the risk of thrombotic events in patients with atrial fibrillation (AF). In this study, we evaluated the use of the CHA2DS2-VASc score as a simple tool for predicting graft failure (GF) among patients who underwent CABG surgery. Methods In thi

Angiosome-Guided Endovascular Revascularization for Treatment of Diabetic Foot Ulcers with Peripheral Artery Disease.

BACKGROUND: Because diabetic foot ulcers (DFUs) are difficult to heal and cause huge economic losses to the society, accelerating their healing has become extremely important. The purpose of this study was to evaluate the effect of revascularization based on the angiosome concept on DFU. METHODS: Between January 2018 and July 2020, 112 consecutive legs with DFUs, in 111 patients who were discharged from the vascular surgery department of our hospital were retrospectively evaluated. The legs were assigned to two groups depending on whether direct arterial flow to the foot ulcer based on the an

Risk Factors for Abdominal Compartment Syndrome After Endovascular Repair for Ruptured Abdominal Aortic Aneurysm: A Case Control Study.

OBJECTIVE: Ruptured abdominal aortic aneurysms (rAAA) are treated by endovascular aneurysm repair (rEVAR) increasingly often. Despite rEVAR being a minimally invasive method, abdominal compartment syndrome (ACS) remains a significant post-operative threat. The aim of this study was to investigate risk factors for ACS after rEVAR, including aortic morphological features. METHODS: The Swedish vascular registry (Swedvasc) was assessed for ACS after rEVAR in the period 2008 - 2015. All patients identified were compared with controls (i.e., patients who did not develop ACS after rEVAR), matched by

Effects of systemic hemodynamics on flow within vascular accesses used for hemodialysis.

Absolute value of access flow (QA) and change in flow (deltaQA) over time are major determinants of access patency. However, QA may change in response to variation in systemic hemodynamics among dialysis sessions. We examined the effect of mean arterial pressure (MAP), cardiac output (CO), and segmental resistances (R) on QA. Access flow and CO (L/min) were determined by Transonic ultrasound dilution. Static intra-access pressures (mm Hg) at the arterial segment (AS) and venous segment (VS) were determined with the access unoccluded. During access occlusion (O), the AS pressure was equated to
