Voice outcomes after radiotherapy treatment for early glottic cancer: assessment using multidimensional tools.

BACKGROUND: This is the first prospective study to use instrumental and both clinician- and client-rated auditory-perceptual measures to examine voice and voice-related quality of life changes in patients after curative radiotherapy for early glottic cancer. METHOD: Thirty patients undergoing curative radiotherapy treatment for early glottic cancer completed the following: 3 voice tasks for acoustic, aerodynamic, and auditory-perceptual voice measures (therapist-rated); a patient self-report rating of voice quality; and a voice-related quality of life assessment before and 12 months after rad

An investigation of voice quality in individuals with inherited elastin gene abnormalities.

The human elastin gene (ELN) is responsible for the generation of elastic fibres in the extracellular matrix of connective tissue throughout the body, including the vocal folds. Individuals with Supravalvular aortic stenosis (SVAS) and Williams syndrome (WS) lack one normal ELN allele due to heterozygous ELN abnormalities, resulting in a haploinsufficiency. We measured perceptual and acoustic characteristics of voice quality in individuals with SVAS and WS to investigate the consequences to vocal function secondary to ELN haploinsufficiency. Results indicated that the voice quality of individu

Auditory processing of sine tones before, during and after ECT in depressed patients by fMRI.

Our goal was to assess treatment effects of electroconvulsive therapy (ECT) on acoustic processing in major depression. We hypothesized that (1) depression is related to functional alterations in auditory networks, and that (2) pre-treatment alterations in auditory networks are reversible through treatment with ECT. Acoustic perception of 20 severely depressed and 20 age and gender matched healthy controls was investigated by 3 T functional magnetic resonance imaging employing repeated stimulation by sine tones. Prior to ECT, depressed patients presented a multimodal recruitment of additional

Activation of middle-ear muscles by transcranial magnetic stimulation.

OBJECTIVES: To evaluate the reliability of transcranial magnetic stimulation in eliciting admittance changes due to activation of middle-ear muscles. METHODS: Admittance changes induced by transcranial magnetic stimulation at the inion were evaluated in eight normal subjects, two subjects with prelingual deafness and 22 patients suffering from other otological disorders characterised by absence of acoustic reflex. RESULTS: Responses showed a predominant negative peak in normal ears. Two small positive components, one preceding and the other following the negative deflection, were less consis

[Mild head trauma: complications and acousticovestibular sequelae].

Head injury with fracture of the petrous bone is frequent and directly related to the development of human activity. Head trauma is associated with nearly 75% of traffic accidents; 5% of them with petrous bone fracture. Based on a clinical case, the acute injuries and secondary acoustic or vestibular sequelae after low-energy cranio-cerebral trauma are discussed here. The subject was a victim of a low-energy head trauma with brief loss of consciousness and translabyrinthine fracture of the petrous bone. This fracture caused invalidating equilibrium disorders related to a perilymphatic fistula.

Bilateral profound hearing loss due to meningeal carcinomatosis.

Meningeal carcinomatosis (MC) is an uncommon form of metastasis of solid tumors. Hearing loss as the presenting symptom of MC is very uncommon. A patient with an esophageal signet ring cell carcinoma 3 years previously presented with sudden onset of profound hearing loss affecting both ears. He had no evidence of local tumor recurrence. Brain magnetic resonance imaging (MRI) showed swelling and increased signal intensity on T2 weighted images of both acoustic nerves and the right trigeminal nerve. After gadolinium administration, enhancement of both acoustic and trigeminal nerves was seen. He

Categorical perception of speech stimuli in children at risk for reading difficulty.

Children determined to be at risk (n = 24) or not at risk (n = 13) for reading difficulty listened to tokens from a voice onset time (VOT) (/ga/-/ka/) or tone series played in a continuous unbroken rhythm. Changes between tokens occurred at random intervals and children were asked to press a button as soon as they detected a change. For the VOT series, at-risk children were less sensitive than not-at-risk children to changes between tokens that crossed the phonetic boundary. Maps of group stimulus space produced using multidimensional scaling of reaction times for the VOT series indicated that

[Soft palate muscles electrostimulation in treatment of children at different stages of exudative otitis media].

We studied efficacy of electrical stimulation of soft palate muscles in 180 patients (292 ears) with various stages of exudative otitis media (EOM). The analysis of the results covered 172 patients. 8 patients withdraw because of the 4th (fibrous) stage of the process (n=4) and incomplete treatment course (n=4). Normalization of audibility thresholds and tympanometric parameters was observed after 10 sessions of electrostimulation in 129 (75.0%) patients with early EOM stages. The effect persisted for 1 year. 3 (1.7%) patients with EOM stage II and 40 (23.3%) patients with stage III failed the

Dehiscence of bone overlying the superior semicircular canal as a cause of an air-bone gap on audiometry: a case study.

Dehiscence of bone overlying the superior semicircular canal can result in a syndrome of vertigo and oscillopsia induced by loud noises or by maneuvers that change middle ear or intracranial pressure. Patients with this disorder can also experience a heightened sensitivity to bone-conducted sounds in the presence of normal middle ear function. High-resolution CT scans of the temporal bones demonstrate the dehiscence. The authors describe a patient with bilateral superior canal dehiscence who had bilateral low-frequency conductive hearing loss, normal middle ear function, intact acoustic reflex

Neural conservation in skull base surgery.

Surgical treatment of lesions of the skull base carries significant risk to the functioning of the cerebral hemispheres, the brain stem and the cranial nerves. This risk is due both, to problems associated with maintaining an adequate blood flow while exposing and removing the tumor and to direct or indirect trauma to the brain, perineural tissues and cranial nerves. These risks may be reduced if information about possible implications of surgical maneuvers on the cerebral blood flow and on the function of the patients central nervous system and cranial nerves is available and can be monitored

Intraoperative transesophageal echocardiographic assessment of acute prosthetic aortic valve regurgitation after mitral valve replacement: value of the deep transgastric long-axis view.

The transesophageal echocardiographic assessment of prosthetic aortic valve function is made more difficult by the presence of a mechanical mitral valve prosthesis because echocardiographic views conventionally used to assess the aortic valve function are obscured by acoustic shadowing and artifacts. We report the use of intraoperative transesophageal echocardiography in a patient who developed severe prosthetic aortic valve regurgitation after implantation of a mechanical mitral valve, in whom conventional multiplane midesophageal views failed to reveal aortic regurgitation owing to acoustic

Objective voice analysis after autologous fat injection for unilateral vocal fold paralysis.

This study was designed to objectively compare a patient's voice after onset of unilateral vocal fold paralysis (UVFP) to his or her own normal voice, and to compare the results after treatment by intrafold injection of autologous fat. Acoustic recordings were obtained for 2 male patients before thoracic surgery and after the onset of iatrogenic left UVFP. Vocal fold augmentation was performed 10 days after UVFP. The acoustic recordings were repeated within 3 days and at 1 month. The phonation quotient, pitch perturbation quotient, amplitude perturbation quotient, harmonics-to-noise ratio, cep

Electrophysiological responses of cochlear root neurons.

Cochlear root neurons (CRNs) are second-order neurons interspersed among the fibers of the cochlear nerve in certain rodents. They project, among other nuclei, mainly to the pontine reticular nucleus, and participate in the acoustic startle response (ASR), a short-latency motor reflex initiated by sudden intense sounds. The sound-evoked activity of CRNs has not previously been described. Here we describe extracellular responses of CRNs located in the infranuclear portion of the cochlear nerve root. CRNs exhibited secure responses to tone bursts, with first-spike latencies of approximately 2.2

Mechanically evoked shortening of outer hair cells isolated from the guinea pig organ of Corti.

Outer hair cells isolated from the mammalian hearing organ have been shown to respond to mechanical stimuli at acoustic frequencies by expressing a change in cell length (e.g. Canlon et al., 1988). The acoustically evoked response is characterised by both a tonic length change following the envelope of the stimulus, and a frequency-dependent phasic component. We show here that mechanical stimulation at much lower frequencies directed at the cell body also elicits length changes of the outer hair cells. When the apical pole of isolated outer hair cells was compressed with a quartz fibre, a shor

Effectiveness of classical chordectomy in the treatment of cancer of the glottis.

From 1980 to 1992, 85 patients diagnosed with tumour of the glottis, localized exclusively in the vocal cord area, had undergone surgery. A 90% 5-year survival rate without recurrences had been obtained. Taking into account the cases of life-saving surgery, the percentage of the 5-year survivals amounted to 94%. Considerably better results were obtained by the excision of the entire vocal cord and not just one of its sections (1/2, 2/3 etc.). The degree of deformation of the voice following chordectomy was assessed by means of acoustic and laryngographic methods (Laryngograph Processor PCLX).

Cochlear implantation as a successful rehabilitation for radiation-induced deafness.

Radiotherapy of the head and neck can be associated with conductive and/or sensori-neural hearing loss. We report the case of a 67-year-old man who developed complete bilateral deafness caused by labyrinthitis and radiation-induced neuritis of the acoustic nerve after postoperative radiotherapy of a nasopharyngeal carcinoma. Two years postoperatively extensive clinical workup including computed tomography and magnetic resonance imaging showed no recurrence or secondary brain tumors. To facilitate sound perception a Combi 40 cochlear implant was implanted. Because of fibrosis the insertion dept

The use of the startle reflex measurement in patients with various types of fixation of the ossicular chain.

The results of startle and acoustic stapedius reflex measurements can be used to subclassify various types of fixation of the ossicular chain. Reflex measurements were performed on a group of subjects with normal hearing and on a group of patients with surgically verified ossicular chain fixation. In 97% of the subjects with normal hearing (n = 30), reproducible impedance changes were observed as part of a startle reaction and four different reaction patterns could be distinguished. In a subgroup of subjects with normal hearing who had a relatively, statistically significant, high acoustic ref
