Computed tomographic evaluation of the role of craniofacial and upper airway morphology in obstructive sleep apnea in Chinese.

OBJECTIVES: To evaluate the relationship between cephalometric parameters, upper airway morphological factors and obstructive sleep apnea (OSA) in Chinese subjects. DESIGN: Polysomnogram (PSG) were performed and scored using standard criteria. Supine lateral cephalometric parameters and pharyngeal cross-sectional areas at the level of velopharynx (VA) and hypopharynx (HA) were measured from computed tomographic scans. The roles of these parameters and other anthropometric/demographic characteristics in OSA (apnea hypopnea index, AHI > or = 5) and their relationship with severity of OSA were e…

Autobiographical memory predicts cognitive but not somatic change in sleep apnea patients vulnerable for affective disorder.

BACKGROUND: Autobiographical memory style (AM) can serve as a marker for determining susceptibility for depression. Currently and formerly depressed persons show reduced specific memory for autobiographical events. AM also has proven to be a reliable predictor for the remissive course of depression. Recently the issue arose as to whether this predictive power depends on particular dimensions of depression (cognitive, somatic). METHODS: To replicate the predictability in vulnerable patients, we tested two groups of obstructive sleep apnea patients (OSAS) before and 2 months after initiation of…

Recurrent hypoxemia in young children with obstructive sleep apnea is associated with reduced opioid requirement for analgesia.

BACKGROUND: Obstructive sleep apnea (OSA) in children is often associated with recurrent hypoxemia during sleep. In developing animals, central opioid neuropeptide content is high, and opioid receptors are up-regulated after recurrent hypoxia. The authors hypothesized that children with recurrent hypoxemia due to OSA might have altered central opioid functionality that could affect their responsiveness to opioid drugs. Using a retrospective database, we assessed the relation of age and preoperative oxygen saturation to the cumulative postoperative morphine dose administered for analgesia in ch…

[Evaluation of cerebral hemodynamics in patients with syndrome of obstructive sleep apnea by long term registration of rheoencephalogram].

AIM: To assess cerebral hemodynamics during sleep in patients with syndrome of obstructive sleep apnea. METHODS: Long term monitoring of rheoencephalogram. Data processing consisted in construction of trends (curves of changes of rheographical parameters during any selected time interval). RESULTS AND CONCLUSION: Cyclical periods of apnea are associated with negative changes of cerebral hemodynamics. At the end of period of apnea despite compensatory vasodilatation cerebral blood flow decreases. This occurs during pronounced hypoxemia and can predispose to ischemic disturbances. In ventilati…

Effects of treatment by laser-assisted uvuloplasty on sleep energy expenditure in obstructive sleep apnea patients.

The purpose of this study was to evaluate the effect of successful laser-assisted uvulopalatoplasty (LAUP) on sleep energy expenditure (EE) in obstructive sleep apnea syndrome (OSAS) patients. Fifteen healthy subjects (group I) and 25 patients with moderately severe or severe OSAS (group II) proven by overnight sleep study and who wanted LAUP were enrolled. During the night of the sleep studies, EE was measured with a metabolic cart (indirect calorimetry with canopy), including basal metabolic rate (BMR), mean sleep EE, lowest sleep EE, ratios of mean sleep EE/BMR, and lowest sleep EE/BMR. For…

Incidence of sleep apnea in a presumably healthy working population: a significant relationship with excessive daytime sleepiness.

Seventy-eight workers, drawn from a population of 1502 presumably healthy working men who were interviewed about sleep habits and sleep disorders, underwent polygraphic recordings for at least 1 night. A significant association was found between the complaint of excessive daytime sleepiness and the incidence of sleep apnea. Workers with more than 10 apneas per hour of sleep complained significantly more about loud snoring, hypermotility in sleep, and frequent headaches. They had significantly more ENT findings and hypertension.

Obstructive sleep apnea: a canonical correlation of cephalometric and selected demographic variables in obese and nonobese patients.

One hundred male obstructive sleep apnea (OSA) patients were classified into 2 groups on the basis of body mass index (BMI): 43 nonobese (BMI < 30 kg/m2) and 57 obese (BMI > or = 30 kg/m2) patients. A comprehensive cephalometric analysis with a multivariate statistical method was performed in order to define the different principal components (PCs) of cervico-craniofacial skeletal and upper airway soft tissue morphology in each group and how they contributed to selected elements of the patient demographic data, ie, apnea-hypopnea index (AHI), nocturnal oxyhemoglobin saturation, and BMI. Thirty…

Patterns of EEG frequency, movement, heart rate, and oxygenation after isolated short apneas in infants.

Patterns of events occurring at the end of apneas have rarely been reported in infants. No previous studies have compared these patterns to those of spontaneous events during sleep. We examined 163 isolated apneas in 17 infants (47 +/- 4 wk postconceptional age) who underwent polysomnography for suspected upper airway problems. Mean apnea duration was 6.5+/-1.5 s (range, 5 to 11.5 s), 78% of apneas occurred in active sleep, and 67% of apneas were obstructive. We recorded the occurrence of body movement or augmented breath and analyzed changes in EEG frequency > or = 1 s, heart rate, and oxygen…

R-zacopride, a 5-HT3 antagonist/5-HT4 agonist, reduces sleep apneas in rats.

The effects of R-zacopride, a benzamide with potent 5-HT3 receptor antagonist and 5-HT4 receptor agonist properties, on spontaneous apneas were studied in 10 Sprague-Dawley rats by monitoring respiration and sleep for 6 h. R-zacopride (0.5, 1.0 and 10.0 mg/kg) suppressed spontaneous central apneas during non-rapid-eye-movement (NREM) sleep by 50% (P=.05 for 0.5 mg/kg, P=.02 for 1.0 mg/kg and P=.001 for 10.0 mg/kg dose vs. control), and during rapid-eye-movement (REM) sleep by 80% by all doses tested (P<.0007) for at least 2 h after intraperitoneal injection. We conclude that R-zacopride, over …

[Obstructive sleep apnea syndrome in a patient with superior vena cava syndrome caused by lung cancer].

A 48-year-old man was treated with chemotherapy and irradiation therapy for superior vena cava syndrome (SVCS) caused by adenocarcinoma of the lung, and his symptoms subsided temporarily. However, the same symptoms recurred, and severe snoring during sleep and daytime hypersomnolence developed. Obstructive sleep apnea syndrome (OSAS) was diagnosed by respiratory inductive plethysmography. After chemotherapy, the symptoms of edema of the face and arms, snoring, and daytime somnolence tendency were alleviated, and the patient's apnea hypopnea index decreased remarkably. In addition, lateral ceph…
