Black-white differences in factors influencing mammography use among employed female health maintenance organization members.

OBJECTIVE: This study examined racial differences in knowledge, attitudes and practices related to breast cancer screening of black and white women health maintenance organization members over age 40 who are employed at 75 worksites in Pennsylvania and New Jersey in the US. DESIGN: Data are from telephone interviews of 1677 women (20% black). The interviews queried background factors and concepts from the Health Belief Model. RESULTS: Compared to whites blacks were younger and less likely to be married or to have family history of breast cancer. They were also more likely to underestimate th

Metastatic intracranial chordoma in a child with massive pulmonary tumor emboli.

A 27-month-old boy of Hispanic background developed multiple cranial nerve palsies, difficulty swallowing, bloody nasal discharge, and irritability. Radiographic evaluations showed extensive destruction of the clivus by a large tumor that invaded the sphenoid bone, left cavernous sinus, ethmoid sinus, nasal cavity, and left orbit. Multiple pulmonary nodules were also noted. The bone marrow and spinal fluid showed no evident tumor cells. Transnasal biopsy revealed a chordoma. Treatment was initiated with a combination of ifosfamide, mesna, and etoposide along with radiation therapy to the crani

A fully flexible joint lattice position and dose optimization method for LATTICE therapy.

BACKGROUND: Lattice radiotherapy (LATTICE) is a technique of spatially fractionated radiation therapy (SFRT) that delivers high radiation doses to specific regions (vertices) within a large tumor, forming a spatially modulated "lattice" pattern, while surrounding areas receive lower doses to minimize damage to healthy tissues. In LATTICE, vertices (regions of high dose) are typically placed at regular intervals within tumors, such as simple cubic or hexagonal arrangement, which limits the flexibility needed to adapt to irregular tumor shapes and the proximity of critical organs, potentially le

Cost-effectiveness analysis of cemiplimab vs pembrolizumab for treatment of advanced cutaneous squamous cell carcinoma.

BACKGROUND: Most cutaneous squamous cell carcinomas (CSCCs) can be treated with surgical excision or radiation; however, approximately 1% of patients develop advanced disease. In 2018, the FDA approved cemiplimab-rwlc as the first programmed cell death-1 (PD-1) monoclonal antibody for the treatment of patients with metastatic CSCC or locally advanced CSCC who are not candidates for curative surgery or curative radiation. In June 2020, pembrolizumab, another PD-1 monoclonal antibody, was approved for the treatment of patients with recurrent or metastatic CSCC who are not candidates for curative

Considerations of previous augmentation in subsequent breast reconstruction.

BACKGROUND: The number of patients receiving augmentation mammaplasty is rapidly growing. Breast cancer will develop in a significant number of these women. OBJECTIVE: The authors report on a series (expanding the size of a previous study by the senior author, S.L.S.), encompassing one surgeon's experience performing breast reconstruction in women who had undergone previous augmentation. They compare the stage and detection method in this series with other published studies and also review their experience with regard to the benefit of subpectoral versus subglandular implant placement. [Risk factors associated with neonatal tumors. Experience of a pediatric environmental health specialty unit in Valencia (Spain)].

BACKGROUND: Neonatal tumors (NT) result from a variable combination of constitutional and environmental determinants. Multiple risk factors (RF) are involved in their development, although most are unknown. OBJECTIVE: To document the constitutional, environmental, preconceptional and transplacental RF associated with the development of NT with greater or lesser scientific evidence. PATIENTS AND METHODS: We investigated known RF in NT diagnosed at La Fe University Children's Hospital from January 1990 to December 1999, using a questionnaire completed by parents in a personal interview, either

The necessity of CT/CTA scans on pediatric cerebrovascular injury after blunt trauma - a single center study.

BACKGROUND: Blunt cerebrovascular injury (BCVI) is a rare but potentially devastating complication of pediatric blunt trauma. Delayed or missed diagnosis may result in ischemic stroke, permanent neurological deficits, or death. To avoid missed injuries, many trauma centers apply liberal computed tomography angiography (CTA) screening strategies, although this may expose children to substantial and potentially unnecessary radiation. Several adult- and pediatric-derived screening tools have been proposed, but their applicability in pediatric trauma populations remains controversial. This study a

The Application of the Balloon Pre-Inflating Technique in Percutaneous Balloon Compression for Primary Trigeminal Neuralgia: A Retrospective Study.

BACKGROUND: Percutaneous balloon compression (PBC) is a safe and effective surgical technique for treating trigeminal neuralgia. The filling balloon can compress the trigeminal ganglion in Meckel's cave to achieve the interruption of pain signals. The core of the operation is the successful placement of a balloon catheter into Meckel's cave so that effective compression of the trigeminal ganglion can be achieved after the balloon is inflated. OBJECTIVE: In this article, we try to determine the application value of the balloon pre-inflating technique (using contrast agents as guides during pos

Two-Stage Endovascular Aneurysm Repair with Preemptive Embolization: A Retrospective Study.

BACKGROUND: Type II endoleak is the most common complication of endovascular aneurysm repair. Retrograde perfusion from the aneurysmal sac side branch to the aneurysmal sac, including the inferior mesenteric artery and lumbar arteries, is associated with adverse events after endovascular aneurysm repair, such as aneurysm sac enlargement, reintervention, rupture, and abdominal aortic aneurysm-related death. Preemptive embolization of the aneurysmal sac side branch before endovascular aneurysm repair is an effective and safe procedure for preventing type II endoleak and reducing the size of the

Hyperbaric oxygen therapy for late radiation tissue injury.

BACKGROUND: Cancer is a significant global health problem. Radiotherapy is a treatment for many cancers and about 50% of patients having radiotherapy with be long-term survivors. Some will experience late radiation tissue injury (LRTI) developing months or years later. Hyperbaric oxygen therapy (HBOT) has been suggested as a treatment for LRTI based upon the ability to improve the blood supply to these tissues. It is postulated that HBOT may result in both healing of tissues and the prevention of problems following surgery. OBJECTIVES: To assess the benefits and harms of HBOT for treating or

Changes in air quality and tropospheric composition due to depletion of stratospheric ozone and interactions with climate.

Air pollution will be directly influenced by future changes in emissions of pollutants, climate, and stratospheric ozone, and will have significant consequences for human health and the environment. UV radiation is one of the controlling factors for the formation of photochemical smog, which includes tropospheric ozone (O(3)) and aerosols; it also initiates the production of hydroxyl radicals (˙OH), which control the amount of many climate- and ozone-relevant gases (e.g., methane and HCFCs) in the atmosphere. Numerical models predict that future changes in UV radiation and climate will modify

Investigating the impact of margins on the management of oropharyngeal cancer treated with transoral robotic surgery: a survey of head and neck oncologists' treatment patterns.

BACKGROUND: There remains heterogeneity in the management of intermediate-risk factors following transoral robotic surgery (TORS) for oropharyngeal cancer (OPC). METHODS: A 23-item case-based survey assessing postoperative-management across risk factors following TORS for OPC was distributed to members of the American Head and Neck Society (AHNS). RESULTS: Sixty-four responses (4.5 % of members) were received, predominantly from U.S-based surgical oncologists (96.2 %), with most reporting ≥ 10 years in practice. Without additional pathological risk factors, observation was recommended for 98
