Fresh frozen human cadaveric spinal specimens (T8-S1) were subjected to pure flexion extension bending moment and pure axial torque loadings while intervertebral rotations were recorded at the L3-L4, L2-L3, and Ll-L2 discs. We analysed the short-term outcome after varus osteotomy for Perthes' disease in 48 older children from south-west India, comparing them with 30 historical controls. The children were between 7 and 12 years of age at the onset of the disease. All had stage-I or stage-II disease, with half or more of the epiphysis involved. The impact of head-neck A-O cancellous screw transfixation on subcapital growth plate in 20 children operated for slipped capital femoral epiphysis is presented. It was found, that early fusion of the growth plate may be attributed to the compression by A-O screw used for transfixation (p-0.0001). Resulting shortening of the femoral neck and relative overgrowth of the great trochanter did not influence the final outcome of treatment.

Variable screw placement (VSP) plates and pedicle screw fixation were used to stabilize eleven lumbar neoplasms. Blood loss and complications were comparable to other methods of posterior segmental fixation, although operative times were longer. Fewer levels were fused than for systems using sublaminar hooks or wires, with 8/11 patients treated with two level fixation. We have analysed 31 malunited ankle fractures treated by reconstructive osteotomies. In all cases the malunited fibula has been corrected; this has been combined with other osteotomies and with fixation of ununited fragments as indicated. Our results show that, unless function was severely impaired, reconstruction was always worth while, no matter how long ago the injury had occurred and even if there were already arthritic changes.

The results of ten acute, displaced proximal femoral fractures in patients 14 years and under are reported. These high-risk fractures were managed with urgent open reduction and pin or screw fixation with supplemental spica casting. The exception to this protocol was in two Delbet's type IV (intertrochanteric) fractures which were managed by closed reduction and spica casting. There was a case of partial avascular necrosis of the femoral head in a type I transepiphyseal fracture. At a minimum followup of 2 years the patients were asymptomatic with no significant limitation of hip motion.
