Neuropsychological driving fitness tests for brain-damaged subjects.

OBJECTIVES: The study was carried out to evaluate and develop simple tools to assess the driving fitness of brain-damaged patients. DESIGN: Four neuropsychological tests (predictors) were validated against an open-road driving test (criterion standard), which is used by the Department of Adaptations of the Dutch Driving Licensing Agency for assessing disabled drivers. SETTINGS: The predictor tests were conducted in the involved scientific institute; the criterion standard involved participation in normal traffic. PATIENTS: The subject group consisted of 33 men and 5 women who had recovered …

[Effect of (+/-)12-chloroscoulerine on brain dopamine receptors].

AIM: To assess potencies of tetrahydroprotoberberines (THPB) and hydrobenzyltetrahydroisoquinolines (HBTI) on DA receptors. METHODS: The receptor binding assay with calf striatum to D1 and D2 receptors, and the animal behavior tests were used. RESULTS: (+/-) 12-Chloroscoulerine (CSL) was the most potent one among the THPB and HBTI. The affinities of CSL to D1 and D2 receptors were 13 and 51 nmol.L-1, respectively. In rats, CSL showed an antagonistic effect on the stereotypy and induced catalepsy. In the 6-OHDA lesioned rats, however, CSL exerted the agonistic effect to DA receptors. CONCLUS

A new brain region for coordinating speech articulation.

Human speech requires complex planning and coordination of mouth and tongue movements. Certain types of brain injury can lead to a condition known as apraxia of speech, in which patients are impaired in their ability to coordinate speech movements but their ability to perceive speech sounds, including their own errors, is unaffected. The brain regions involved in coordinating speech, however, remain largely unknown. In this study, brain lesions of 25 stroke patients with a disorder in the motor planning of articulatory movements were compared with lesions of 19 patients without such deficits. …

Persistent poliovirus infection of human fetal brain cells.

It has been suggested that poliovirus (PV), the causative agent of poliomyelitis, could persist in surviving patients. We have previously shown that PV can persistently infect some human cell lines in vitro, particularly neuroblastoma cell lines. We report here an ex vivo model in which PV can persistently infect primary cultures of human fetal brain cells. Two mutations involving capsid residues 142 of VP2 and 95 of VP1 were repeatedly selected during the persistent infections. These residues are located in capsid regions known to be involved in interactions between PV and its receptor. Durin…

[Reversible changes in brain volume in anorexia nervosa].

Cranial computed tomography (CT) was performed in three different samples of young women with anorexia nervosa before and after treatment. The biggest changes in cerebrospinal fluid volume were observed in the supratentorial and extracerebral-subarachnoid area, with smaller changes in the ventricles. These changes, as assessed by different methods of volumetric and planimetric measurement, were statistically significant and were correlated with the changes in body weight. Two patient groups could be distinguished on the basis of the presence or absence of cerebellar pseudoatrophy in the initia

Brain size and morphology in miniaturized plethodontid salamanders.

In six miniaturized salamanders of the family Plethodontidae, including one of the smallest tetrapod vertebrates. Thorius pennatulus, the anatomical consequences of miniaturization for the brain were investigated. We determined (1) absolute and relative size of the brain, major parts of the brain, the tectum and tectal gray matter, (2) nerve cell size and density, and (3) the number of cells within the visual and visuomotor centers (thalamus, tectum/praetectum and tegmentum). No common compensatory strategy for the brain among the miniaturized salamanders was found. Except for the smallest spe…

A valid experimental brain death organ donor model.

BACKGROUND: This study was designed to establish a validated canine brain death model. Ten consecutive dogs were studied to investigate the effects of brain death on hemodynamic, metabolic, and hormonal function. METHODS: Brain death was induced by inflation of a subdurally placed balloon and was validated neuropathologically. Functional data and blood samples were collected before and 15, 45, 90, 240, 360, and 420 minutes after the induction of brain death. No inotropic or vasoactive support was given. The results are expressed as mean +/- standard error of the mean. RESULTS: The Cushing re…

Crossed cerebellar diaschisis and brain recovery after stroke.

BACKGROUND AND PURPOSE: Although crossed cerebellar diaschisis is well recognized after stroke, there is controversy concerning its clinical correlations and serial changes, and little is known about its prognostic value. METHODS: We studied crossed cerebellar diaschisis and cerebral hypoperfusion in 47 patients with acute middle cerebral cortical infarction using 99mTc-hexamethylpropyleneamine oxime and single-photon emission computed tomography within 72 hours of stroke onset. Thirty-one of these patients had outcome studies at 3 months; 15 of the 31 underwent an additional scan after aceta

Critical care of children with acute brain injury.

Severe neurologic illness and injury in children may occur in a wide range of clinical and environmental settings. The majority of children who sustain traumatic brain injury will achieve a good outcome if intensive care is directed toward preventing secondary injury. The most important aspect of care is ensuring adequate oxygenation, ventilation, and perfusion. Together with standard supportive care, the aggressive use of intraventricular pressure monitoring and CSF drainage to treat intracranial hypertension can attenuate or prevent continuing brain injury. Sustained hyperventilation, aggres…

[Corticobasal degeneration: symptomatological, brain-imaging and electrophysiological studies].

Eight cases of clinically diagnosed corticobasal degeneration (CBD) were studied with reference to their symptomatology, brain-imagings and electrophysiological findings. The diagnosis was based on the combination of limb-kinetic apraxia (cortical sign), akinetic-rigid sign (extrapyramidal) and their unilateral predominance. Magnetic resonance imaging (MRI) and 123I-IMP or 99mTc-HMPAO SPECT findings were used to reinforce the diagnosis. The age at onset of 8 cases (4 males, 4 females) was 61 to 80 years (mean 66). Other common symptoms on admission consisted of dysequilibrium (8 cases), dysart…

Post-traumatic brain abscess: experience of 36 patients.

Thirty-six patients with post-traumatic brain abscess were managed over 18 years. They constituted 9.3% of all brain abscesses encountered during the same period. The head injury was associated with an external compound fracture in 20, internal compounding in three and was closed in 13 patients. The mean interval between the time of injury to presentation with an abscess was 113 days. This did not differ significantly in patients with closed and compound head injury, and amongst patients who had wound sepsis and with clean wounds after the injury. The occurrence of focal neurological deficit w…

Clinical aspects of perinatal hypoxic-ischemic brain injury.

Perinatal hypoxic-ischemic cerebral injury is a significant cause of neurological morbidity and mortality in childhood. It is often difficult to determine the precise timing of such injury. Data from epidemiological and neuropathological studies have identified several maternal and fetal risk factors that seem to be associated with suspected prenatal brain injury. Hypoxic-ischemic cerebral injury that originates earlier in gestation may result in few, if any, clinical abnormalities during the newborn period. Clinical abnormalities may also be difficult to recognize in the premature newborn, an…

Q-modification of tRNAs in human brain tumors.

Queuosine (Q nucleoside) is a highly modified guanosine analog and is present only in the first position of the anticodons of tRNA(Tyr), tRNA(His), tRNA(Asn) and tRNA(Asp). The levels of Q in normal human brain and two different types of human brain tumors (meningiomas and astrocytomas) were determined by using an enzymatic assay method. tRNAs isolated from tumor tissues contained decrease amounts of Q when compared to tRNAs from normal (i.e. non-tumor) brain tissue. There was a relationship between the levels of Q nucleoside and the tumor malignancy in the sense that the deficiency was greate…

Peritumoral brain edema and cortical damage by meningioma.

Forty supratentorial meningiomas were analyzed to identify factors causing peritumoral brain edema. Parasagittal, sphenoid ridge, and olfactory groove meningiomas induced edema more frequently than those in other locations. Meningothelial meningiomas were more invasive than other types and were associated with more peritumoral edema. Brain edema correlated significantly with tumor size and histological evidence of leptomeningeal and cortical damage from the tumor. Larger tumors destroy the leptomeninges and cerebral cortex, allowing direct transmission of edema fluid into the white matter, res…

Blood-brain barrier damage in experimental African trypanosomiasis.

African sleeping sickness is characterized by progressive central nervous system (CNS) involvement, leading to the so-called secondary or late stage in which there are widespread inflammatory changes with lymphoplasmocytic infiltration. A study was made of blood-brain barrier (BBB) integrity in the late stages of a rodent model by assessing the uptake of the fluorescent fluid-phase marker sulphorhodamine B into the brain tissue. Brain oedema was estimated from brain weight, density and electrolyte concentrations. Trypanosome distribution was studied by light and electron microscopy. At 35 days…
