Blood pressure and cognitive decline in healthy old people.

Both hypertension and cognitive decline are common in old age. We sought to examine the effects of blood pressure (BP) on rates of cognitive decline in a longitudinal study of community-resident healthy old people. A total of 603 initially healthy old people aged over 69 years were visited at home. Subject's age, years of full-time education, Social Occupational Classification, health status and medication use were recorded. Sitting systolic and diastolic BP was measured, and the Mini-Mental State Examination (MMSE) and National Adult Reading Test (NART) administered. Follow-up was planned aft

Getting It Out There: Reflections on the Process and Impact of Public Engagement Activities in a Study on End-of-Life Care Planning With People With Intellectual Disabilities.

INTRODUCTION: Many tools and interventions developed from research projects are not properly implemented in health and social care practice. Engaging with stakeholders is an important part of the implementation process when developing, sharing and embedding new tools. The Victoria & Stuart Project was a 2-year project that co-designed a toolkit of resources and approaches to support end-of-life care planning for people with intellectual disabilities. It had an inclusively delivered dissemination and engagement workstream to raise awareness of the toolkit throughout the project. In this paper,

Co-Designing Person- and Family-Centered Care for Older Adults Living With HIV: Protocol for a Community-Based Participatory Study.

BACKGROUND: Advances in antiretroviral therapy have transformed HIV into a chronic condition, leading to a growing population of adults aged 50 years and older living with HIV in Canada and globally. These individuals experience higher rates of multimorbidity, frailty, cognitive changes, and polypharmacy than their HIV-negative peers, and many rely on caregivers for emotional and practical support. Caregiving often occurs within chosen families of partners, friends, and community members, yet these caregivers remain largely unrecognized in policies that prioritize bio-legal family structures.

Effectiveness of an online mindfulness based stress reduction intervention on psychological distress among patients with COVID19 after hospital discharge.

Given the high prevalence of psychological complications during pandemics such as COVID-19, it is imperative to identify effective psychological interventions that can improve the mental health of patients and community members. The effectiveness of mindfulness-based intervention in discharged patients with infectious respiratory diseases, particularly during pandemics, has been examined in a limited number of studies. This study aimed to investigate the effectiveness of an online Mindfulness-Based Stress Reduction (MBSR) training program in alleviating psychological distress, depression, anxi

A multicentred two-arm parallel single-blind superiority randomised controlled trial comparing psychological and emotional stabilisation with eye movement desensitisation and reprocessing and treatment-as-usual to treatment-as-usual with adults with intellectual disabilities who have post-traumatic stress disorder (the Trauma-AID trial): protocol.

INTRODUCTION: The primary objective of this clinical trial is to determine the clinical and cost-effectiveness of psychoeducation and emotional stabilisation (PES), together with eye movement desensitisation and reprocessing (EMDR) plus treatment-as-usual (TAU) in reducing symptoms of post-traumatic stress disorder (PTSD) among adults with intellectual disabilities compared with TAU. Secondary objectives include: (1) determining whether PES/EMDR plus TAU is superior to TAU in improving mental health problems and quality of life (QoL) among adults with intellectual disabilities who had a diagno

Use of Pre-operative Echocardiograms in Traumatic Neck of Femur Fracture Patients: A Two-Cycle Quality Improvement Project.

Background Hip fractures are among the most common causes of hospital admission in elderly patients. National Institute for Health and Care Excellence (NICE) recommends surgery within 24-48 hours in such patients; however, pre-operative transthoracic echocardiography (TTE) is occasionally requested inappropriately, risking delays in surgery without improving outcomes. Delays to hip fracture surgery are associated with increased morbidity and mortality, with each 24-hour delay linked to worse clinical outcomes, underscoring the importance of timely operative intervention. This quality improveme

Shared decision making - a review of its evaluation, efficacy, and applicability in asthma.

Over the years, the approach to medical decision-making has evolved significantly-from the traditional paternalistic model, in which decisions were made on behalf of the patient, to Shared Decision Making (SDM), which actively involves patients in the process. Given that a strong patient-healthcare provider relationship is a key factor in effective treatment, the literature increasingly highlights the importance of incorporating patient preferences. To achieve this, patients must receive clear explanations about their condition and treatment options, as well as care plans tailored to their ind

Association of Childhood Maltreatment With Suicide Behaviors Among Young People: A Systematic Review and Meta-analysis.

IMPORTANCE: Children and young people's reports of experiences of adverse childhood events have increased in recent years, and this trend has been associated with an elevated risk for suicide behaviors. However, a systematic review and meta-analysis is needed to confirm the significance of this association in young people. OBJECTIVE: To quantify the association between core types of childhood maltreatment, including sexual, physical, and emotional abuse and/or neglect and suicide behaviors in children and young adults. DATA SOURCES: Medline, PsychInfo, Embase, Web of Science, and CINAHL (Cum

Treatment effect modifiers of cognitive behaviour therapy in people with psychosis: an individual participant data meta-analysis of RCTs.

BACKGROUND: Cognitive-behavioural therapy is a recommended intervention for the treatment of schizophrenia and related psychoses, but there is considerable uncertainty on whether its effectiveness is moderated by patient characteristics and/or intervention characteristics. OBJECTIVE(S): To identify treatment effect modifiers of cognitive-behavioural therapy in people with schizophrenia spectrum diagnoses. DESIGN: An individual participant data meta-analysis of randomised controlled trials comparing cognitive-behavioural therapy to treatment as usual or control active psychosocial control int

Can a digital intervention 'Momentum' improve social functioning and transdiagnostic symptoms for Australian youth at ultrahigh risk for psychosis? Protocol for a superiority randomised controlled trial comparing treatment as usual with and without Momentum.

INTRODUCTION: Existing psychological and pharmacological interventions for young people at ultra-high risk (UHR) for psychosis have shown benefit in at least delaying the transition to psychosis, but they have limited benefit for comorbid disorders or social dysfunction, which are prominent for those at UHR. We developed a moderated online social therapy platform (named Momentum) including: (1) transdiagnostic therapeutic interventions targeting social functioning, depression, generalised anxiety and social anxiety; (2) a moderated, peer-led online community and (3) specialised human support f

Interventions for encouraging sexual behaviours intended to prevent cervical cancer.

BACKGROUND: Human papillomavirus (HPV) is the key risk factor for cervical cancer. Continuing high rates of HPV and other sexually transmitted infections (STIs) in young people demonstrate the need for effective behavioural interventions. OBJECTIVES: To assess the effectiveness of behavioural interventions for young women to encourage safer sexual behaviours to prevent transmission of STIs (including HPV) and cervical cancer. SEARCH STRATEGY: Systematic literature searches were performed on the following databases: Cochrane Central Register of Controlled Trials (CENTRAL Issue 4, 2009) Cochra

Variations in practice and outcomes in the Canadian NICU network: 1996-1997.

BACKGROUND: Previous reports of variations in outcomes among neonatal intensive care units (NICUs) examined only specific subpopulations of interest (eg, very low birth weight [VLBW] infants <1500 g of birth weight [BW]). OBJECTIVES: We report on current practice and outcomes variations in a population-based national study of Canadian NICUs from January 8, 1996 to October 31, 1997. METHOD: Information on 20 488 admissions to 17 tertiary level NICUs across Canada was prospectively collected by trained abstractors using a standard manual of operations and definitions. Evaluation of immediate and sustained effects of transitioning quality long-acting reversible contraceptives (LARCs) services to public sector health facilities in Ghana: a pre-post intervention study.

BACKGROUND: Long-acting reversible contraceptives (LARCs) are highly effective at preventing pregnancy and demonstrate favorable client satisfaction. However, limited knowledge, misconceptions, and concerns about side effects hinder LARC adoption. Marie Stopes International-Ghana collaborated with Ghana Health Service to implement a 5 year multifaceted intervention to transition quality LARC services from an outreach approach to being available in public sector health facilities. This study evaluates if the intervention resulted in immediate or sustained improvements in the provision of qualit

Drivers influencing antibiotics use in pediatric wards across five public hospitals in Zanzibar: a WHO multiple point prevalence survey.

BACKGROUND: Strengthening antimicrobial stewardship (AMS) programs is a major public health strategy to reduce inappropriate antibiotic use (AMU) and averting antibiotic resistance (AMR). Evidence on AMU and AMS program implementation is widely documented in Tanzania mainland, consistent with previous studies done and other low-and middle-income countries. However, there is limited information on AMU and AMS programs to guide the implementation of the Zanzibar Action Plan on AMR. The objective of this study was to determine the prevalence of AMU, identify factors associated with AMU, and to as

Social Transfers for Exclusive Breastfeeding in Brazil: Protocol for a Randomized Controlled Trial.

BACKGROUND: According to the World Health Organization's infant and young child feeding guidelines, infants should be exclusively breastfed for the first 6 months of life. Despite public health campaigns to increase exclusive breastfeeding (EBF) rates, socioeconomic inequities persist among low-income breastfeeding mothers, especially in countries with large wealth and health gaps, such as Brazil. Social transfer programs are initiatives that provide financial support to individuals or households to improve their well-being and reduce financial burdens. These may be conditional, requiring reci

Pooled prevalence and multilevel determinants of stillbirths in sub-Saharan African countries: implications for achieving sustainable development goal.

BACKGROUND: Despite being included in the Millennium Development Goals (MDGs) and now the Sustainable Development Goals (SDGs), stillbirths remain overlooked with limited regional research, highlighting an ongoing gap in addressing this issue. However, a staggering 2 million stillbirths occur each year, equivalent to one every 16 s. Furthermore, approximately 98% of these stillbirths take place in developing countries, particularly in sub-Saharan Africa (SSA). In light of these statistics and the need to address the lack of data, methodological approaches, and population gaps, this study aims

Efficacy of mindfulness and goal setting interventions for increasing resilience and reducing smoking in lower socio-economic groups: randomised controlled trial protocol.

BACKGROUND: Smoking and resulting health problems disproportionately impact low socioeconomic status (SES) individuals. Building resilience presents an approach to 'closing the gap'. Mindfulness-based interventions and setting realistic goals are preferred in low socioeconomic communities. We aim to test if these interventions, delivered online and consolidated with peer support offered via ex-smokers, are successful in promoting smoking cessation and resilience. Our conceptualisation of resilience encompasses the inner capacity/skills and external resources (e.g., social support) which smoker

Multinational dietary changes and anxiety during the coronavirus pandemic-findings from Israel.

BACKGROUND: Increased anxiety was frequently reported during the 2020 global COVID-19 pandemic. An association between anxiety and increased body weight has been documented. Identifying associations between diet quality and anxiety may facilitate the development of preventive dietary policy, particularly relevant since obesity appears to increase the risk of adverse COVID-19 outcomes. In this study we aim to examine associations between changes in diet pattern and body weight and anxiety levels during the COVID-19 pandemic among Israeli respondents to an international online survey. METHODS:

Age, ageing, ageism and "age-itation" in the Age of COVID-19: rights and obligations relating to older persons in Israel as observed through the lens of medical ethics.

COVID-19, the illness caused by the SARS-CoV-2 virus, has reached pandemic proportions. Although the virus can cause disease in anyone, it is particularly dangerous for those with various "co-morbidities" such as heart disease, hypertension, diabetes, obesity and others. Furthermore, advancing age (from about 60 on), even in those older persons without any accompanying illnesses, is a strong and independent risk factor for pneumonia, need for an ICU bed and death from the virus. It is therefore essential to find ways to protect all at-risk persons (old or young) from the virus but at the same
