Increasing complexity and plateauing outcomes in AO type C distal humerus fractures: a 50-year institutional case series.

BACKGROUND: Distal humerus fractures in adults are rare but complex injuries, particularly AO type C3 fractures, often involving comminution, instability, and osteoporotic bone. Demographic trends indicate rising incidence, especially among elderly patients, with increasing functional demands challenging modern osteosynthesis. Open reduction and internal fixation (ORIF) with bicolumnar plating remains the preferred reconstructive approach, although complications remain substantial. This study aimed to evaluate trends in fracture complexity, patient demographics, and functional outcomes over fi

Systematic review of shoulder arthroplasty outcomes: what sample size is meaningful?

BACKGROUND: Shoulder arthroplasty is increasingly performed for shoulder conditions such as arthritis, rotator cuff arthropathy, and traumatic injuries. Registries and other compilations of patient data provide the opportunity to detect meaningful differences in outcomes between alternative techniques and implants. A wide range of outcome measurements are reported after shoulder arthroplasty, but the sample sizes needed to identify meaningful differences have not been studied systematically. This review systematically analyzes common clinical outcomes reported for shoulder arthroplasty and rep

The relationship between subjective shoulder discomfort and quantitative humeral head translation in baseball players; cross-sectional study.

BACKGROUND: High shoulder external rotation range of motion (ROM) in baseball players is associated with a higher risk of shoulder and elbow injuries. Subjective shoulder discomfort can impair performance in these athletes. While shoulder stabilization surgery is effective in non-throwing athletes, including those in collision sports, its efficacy is limited in throwing athletes. Understanding the appropriate degree of anterior and posterior shoulder structural translation is crucial for treating shoulder instability in baseball players. This study aimed to determine whether subjective shoulde…

Musculocutaneous nerve: histotopographic study and clinical implications.

Surgical reconstruction of severe brachial plexus injuries includes nerve grafting and neurotization techniques of the musculocutaneous nerve (MCN) to recover elbow flexion. In treating recurrent anterior shoulder instability, knowledge of the topography of the MCN is important for the margin of safety available during dissection. The present study evaluates the origin and course of the MCN and its branches, and their relationships to bone landmarks. Twelve unembalmed cadavers (50-82 years old) were dissected. A histological study of the MCN and the coracobrachialis muscle (CB) was also carrie

Safety and Effectiveness of Placental Connective Tissue Matrix Allograft Versus Corticosteroid Injection in Adhesive Capsulitis: A Double-Blinded Prospective Randomized Study.

BACKGROUND: Adhesive capsulitis causes significant pain and functional limitation, with limited effective treatment options. Current standard-of-care treatments, such as corticosteroid injections, may offer short-term symptom relief but are associated with limited long-term efficacy and potential side effects. Regenerative therapies using extracellular matrix-based products derived from placental tissue have shown promise in treating musculoskeletal injuries due to their anti-inflammatory and tissue-healing properties. Clinical data supporting their use in adhesive capsulitis remains limited.

Reconstructive Options for Pressure Ulcers in Pediatric Patients.

BACKGROUND: Pressure ulcers pose significant challenges in terms of treatment, often exhibiting a low success rate and a propensity for recurrence. Children with neurological impairments such as myelomeningocele and those with spinal injuries are particularly vulnerable to developing pressure ulcers. Despite advancements, achieving successful reconstruction remains a formidable task. Common sites prone to pressure ulcer formation include the sacral and ischial regions, as well as areas over bony prominences. Additionally, pressure ulcers attributable to medical devices facilitating ambulation

Biceps Tenodesis Versus Superior Labral Anterior and Posterior (SLAP) Lesion Repair for the Treatment of SLAP Lesion in Overhead Athletes: A Systematic Review and Meta-analysis.

BACKGROUND: Superior labral anterior and posterior (SLAP) lesions are common injuries in overhead athletes. As a surgical treatment for SLAP lesions, SLAP repair has been traditionally performed. Recently, biceps tenodesis has been performed as an alternative treatment option; however, there is no review for outcome comparisons between these 2 procedures in overhead athletes. PURPOSE: The aim of this study was to compare the outcomes of SLAP repair versus biceps tenodesis, especially in overhead athletes with SLAP lesions. STUDY DESIGN: Systematic review and meta-analysis; Level of evidence,

Percutaneous pin fixation of intercondylar fractures in young children.

T-condylar fractures of the distal humerus are very rare in children. When they do occur they tend to affect those approaching skeletal maturity. Most agree that the undisplaced fracture can be managed conservatively. With increasing displacement and comminution the opinions tend to differ. As an alternative to open reduction and internal fixation we report the use of closed reduction and percutaneous pin fixation for displaced and comminuted T-condylar fractures in a series of three young children. In addition to the classical medial and lateral column fixation we report the use of a transcon

[Reconstruction of the loss of bone substance of the forearm by cubitalization of the radius (one bone forearm). Apropos of 6 cases].

Extensive forearm bone loss, whatever its etiology, presents a difficult reconstruction problem. This is mainly the case in the presence of lesions of the interosseous membrane associated with the radio-ulnar joint. When preservation of forearm rotation is not possible, cubitalization of the radius and reconstruction of the forearm by creation of a "one bone forearm" seems to be an excellent salvage technique both functionally and cosmetically. Our experience concerns six clinical cases; two of these cases are original and give the authors the opportunity to describe a new reconstructive techn

Anatomic Reconstruction of Acute Posterior Shoulder Instability With Reverse Hill-Sachs Lesions: A Report of Two Cases.

Posterior shoulder dislocation associated with a reverse Hill-Sachs lesion is an uncommon injury that may result in persistent instability when the humeral head defect is engaged. Management depends on lesion size, engagement, chronicity, and reconstructibility; both non-anatomic procedures and anatomic reconstruction strategies have been described for acute lesions within the joint-preserving range. We present two cases of acute posterior shoulder instability with engaging reverse Hill-Sachs lesions treated with a combined arthroscopic-open approach. In both patients, arthroscopic posterior l

Structural Augmentation in Rotator Cuff Repair Decreases the Risk of Retear: A Systematic Review and Meta-analysis.

BACKGROUND: Rotator cuff tears are a common orthopaedic injury often resulting in shoulder pain and functional impairment. Despite the prevalence of rotator cuff tears, the incidence of retear after rotator cuff repair (RCR) remains high. There is growing interest in the use of structural augmentation in RCR to decrease retear rates and improve patient outcomes. PURPOSE: The purpose of this systematic review and meta-analysis was to compare the clinical and imaging outcomes of structural augmentation versus standard RCR in patients with full-thickness rotator cuff tears. The effectiveness of

Nonoperative management of olecranon fractures in the working age patient: A case series.

BACKGROUND: Operative treatment is the standard of care for the majority of olecranon fractures in working-age patients. However, nonoperative management may be indicated for non-injury reasons in this population. Current literature on nonoperative management for olecranon fractures is limited to the low-demand geriatric population. This study aimed to report outcomes following primary nonoperative management of olecranon fractures in working-age patients. METHODS: All consecutive patients aged 18-65 years managed nonoperatively for an olecranon fracture at two Level 1 Trauma Centers between

Intramedullary nailing using K-wires for high-energy distal humeral metaphyseal-diaphyseal fractures accompanying radial nerve palsy in a 2-year-old toddler: A case report.

RATIONALE: Pediatric distal humeral diaphyseal fractures are rare and challenging to manage due to anatomical and biomechanical complexities. In addition, humeral shaft fractures in adults are often accompanied by radial nerve paralysis (RNP), but there are no studies on the incidence or treatment of pediatric humeral shaft fractures accompanied by RNP in toddlers. The authors present the outcomes of closed reduction and intramedullary nailing using Kirschner wires (K-wires) for high-energy distal humeral metaphyseal-diaphyseal open fractures accompanied by RNP in a toddler. Mindful attention is inversely associated with pain via mediation by psychological distress in orthopedic patients.

BACKGROUND: Orthopedic patients report pain as their main symptom complaint. Subjective pain experience is correlated with self-reported psychological state, such as distress. PURPOSE: This study tests whether scores from a measure of mindful attention are associated with subjective pain levels and whether psychological distress scores function as a mediation path. METHODS: During routine visits to a single orthopedic clinic in East Los Angeles, California, 525 patients were recruited to participate in the study. Participants reported on measures of pain (Universal Pain Assessment Tool [UPAT
