[Systemic complications in intramedullary nailing].

Today intramedullary nailing is the treatment of choice in stabilizing femoral and tibial diaphysial fractures because of its superior bone healing compared to other forms of osteosyntheses. By interlocking, the indication can be extended to all fractures in which interlocking bolts can be fixed in the proximal and distal main fragment. Küntscher's principle of elastic clamp has changed to intramedullary splinting. With that method reaming is limited to a few reaming processes, and unreamed nailing has become possible. Today implants start at a diameter of 9 mm. The diameter of implants of all

Intraarticular atlantooccipital fusion for the treatment of traumatic occipitocervical dislocation in a child: a new technique for selective stabilization with nine years follow-up.

STUDY DESIGN: A case report of traumatic atlantooccipital dislocation (AOD) managed by intraarticular-posterior fusion from a posterior approach at the C0-C1 level with preservation of C1-C2 motion. OBJECTIVE: To present a new technique for atlantooccipital fusion with long-term follow-up. SUMMARY OF BACKGROUND DATA: There is an increasing number of patients with AOD who have preservation of neurologic function. The most frequent method used to treat this condition is occipitocervical fusion. There has been a tendency in recent years to minimize the extent of stabilization, performing occipi

Motion versus fixed distraction of the joint in the treatment of ankle osteoarthritis: a prospective randomized controlled trial.

BACKGROUND: Initial reports have shown the efficacy of fixed distraction for the treatment of ankle osteoarthritis. We hypothesized that allowing ankle motion during distraction would result in significant improvements in outcomes compared with distraction without ankle motion. METHODS: We conducted a prospective randomized controlled trial comparing the outcomes for patients with advanced ankle osteoarthritis who were managed with anterior osteophyte removal and either (1) fixed ankle distraction or (2) ankle distraction permitting joint motion. Thirty-six patients were randomized to treatme

Trans-tragal incision for improved exposure of diacapitular and condylar neck fractures.

The authors present a modification of the preauricular approach that improves the exposure of the condyle whilst reducing diacapitular and condylar neck fractures. The incision is a combination of the hockey-stick and endaural incisions. Its inferior part runs within the ear on the posterior face of the tragus; the tragal cartilage is transected together with the retrotragal skin and included in the anterior skin flap. Between May 2009 and December 2010, 16 patients with diacapitular or condylar neck fractures were treated with this approach. All patients showed good occlusion postoperatively

[Nonunions or malunions of pelvic fractures].

PURPOSE OF THE STUDY Surgery for the nonunion or malunion of pelvic fractures is not common even at specialised departments. This article presents the authors experience with this procedure, completed with case studies and a review of relevant literature. MATERIAL AND METHODS From 1998 to 2007, a total of 359 patients underwent surgery for pelvic trauma. In the same period, eight surgeries on nonunions or malunions of the pelvis were performed, three in men and five in women. The average age of the patients was 37.3 years (range from 15 to 68). The primary treatment included conservative thera

Radiological assessments and clinical results of intra-articular osteotomy for traumatic osteoarthritis of the ankle.

INTRODUCTION: Traumatic osteoarthritis of the ankle joint caused after malleolar fractures of the ankle and tibial plafond fractures are frequently observed in comparatively young and highly active patients. Since the ankle movement in these patients is in general, comparatively favorable, orthopedists may sometimes have difficulty in deciding on a treatment policy. In our department, when treating traumatic osteoarthritis patients having a movable range within their ankle joints, we proactively applied distal tibial oblique osteotomy (DTOO) developed by Dr. Teramoto in 1994 or intra-articular

[A prospective study of the outcome of anterior laxity of the knee after anterior cruciate ligament reconstruction with procedures using two different patellar tendon grafting methods].

PURPOSE OF THE STUDY: The aim of this study was to evaluate postoperative evolution of anterior laxity, in order to know at what time the elongation of the reconstructed anterior cruciate ligament (ACL) begins to develop after surgery, and if there were any correlations with recuperation of knee flexion or extension. The second purpose of this study was to compare two series of patients operated on by two different procedures for a long standing ACL rupture. MATERIAL AND METHODS: Two series of patients were compared: group I (60 cases), had an ACL reconstruction using a free autologous patell

Distraction lengthening by callotasis of traumatically shortened bones of the hand.

BACKGROUND: Callotasis of the hand has several advantages: it is less invasive than other techniques as bone grafting is unnecessary, gradual distraction is possible, joint mobilization can be performed during treatment, and sensation is maintained. Disadvantages include longer period of treatment and perhaps the need for complicated and bulky instrumentation. METHODS: We reported results of the lengthening of eight traumatically shortened metacarpals or phalanges (in six patients). There were two men and four women, with a mean age of 17.5 years. There were one thumb and seven fingers. There

Relapse following frontofacial advancement using the rigid external distractor.

Multisutural synostosis may result in frontofacial hypoplasia. The aesthetic and function problems arising from this can be corrected by frontofacial advancement, either by monobloc or bipartition osteotomy. Significantly larger, safer advancements can be achieved using distraction osteogenesis when compared to conventional osteotomy. However, the stability of this technique has been questioned. A retrospective study of 21 patients with craniofacial dysostosis who underwent frontofacial advancement osteotomies using the rigid external distractor system was undertaken. Twelve were distracted on

Mandibular distraction in the absence of an ascending ramus and condyle.

The introduction of distraction osteogenesis (DO) as a method of mandibular elongation has provided the craniofacial surgeon with a valuable tool in the approach to patients with mandibular deficiencies. A growing number of case reports and clinical series have proven the efficacy of mandibular DO as an augmentative technique in patients with hemifacial microsomia (HFM). Although DO has become a part of the treatment algorithm for many patients with HFM, surgeons have been reluctant to apply the technique in patients with complete agenesis of the ascending mandibular ramus and condyle (grade I

Surgery versus conservative management of stable thoracolumbar fracture: the PRESTO feasibility RCT.

BACKGROUND: There is informal consensus that simple compression fractures of the body of the thoracolumbar vertebrae between the 10th thoracic vertebra and the second lumbar vertebra without neurological complications can be managed conservatively and that obvious unstable fractures require surgical fixation. However, there is a zone of uncertainty about whether surgical or conservative management is best for stable fractures. OBJECTIVES: To assess the feasibility of a definitive randomised controlled trial comparing surgical fixation with initial conservative management of stable thoracolumb

Three-dimensional computed tomographic evaluation of Le Fort III distraction osteogenesis with an external device in syndromic craniosynostosis.

There is little anteroposterior growth of the midface in patients with syndromic craniosynostosis who are followed up over time without intervention. A Le Fort III with distraction osteogenesis can be done to correct this. This is a controlled way in which to achieve appreciable stable advancement of the midface without the need for bone grafting, but the vector of the movement is not always predictable. The purpose of this study was to evaluate the 3-dimensional effect of Le Fort III distraction osteogenesis with an external frame. Ten patients (aged 7-19 years) who had the procedure were inc
