Aortic valve replacement with a 17 mm Björk-Shiley prosthetic valve, and the closure of left ventricular and right atrial fistulae which were connected with the aortic root were successfully performed in a 10-year-old boy. He was admitted because of high fever and severe heart failure, and diagnosed aortic regurgitation, and left ventricular and right atrial fistulae due to active infective endocarditis. The annular defect was repaired with annular enlargement by a pericardial patch and annulo-right atrial fistula was directly closed from the right atrium. The patient is doing well 1 year afte

The management of young patients with valvular heart disease is a difficult problem in developing countries. At our Institution, 765 valves were operated upon in 523 patients over a period of two years. Rheumatic etiology was seen in 70% of the valves and 60% of all the valves were repaired. The hospital mortality and morbidity in the form of thromboembolic episodes were less in the repair group; 35 valves in 26 patients had to be reoperated upon for instability of the primary repair. Despite this high and early reoperation rate in the repair group and considering the higher mortality plus the

The authors report two operated cases of floating left atrial thrombus. Both patients had severe mitral stenosis, atrial fibrillation and dilatation of the left atrium. In the first case, the patient had no anticoagulant therapy; in the second case, the observance of anticoagulant therapy was irregular. Neither patient complained of syncope. The diagnosis was made in both cases by two-dimensional echocardiography which, in addition to confirming the severe mitral stenosis, showed a round, free-floating intra-atrial mass, rebounding from wall to wall and repeatedly engaging itself into the mitr

Tricuspid valve surgery inherently carries an increased peri-operative risk. In the redo setting, impaired right ventricular function and the frequent presence of hepatic dysfunction are further amplified by extensive adhesions. Conventional re-sternotomy, although well-established, requires substantial tissue trauma. Therefore, strategies that minimize surgical trauma-such as a minimally invasive approach on the beating heart with limited access-may be advantageous in this high-risk population. In this video tutorial, we present a minimally invasive (totally endoscopic) second re-operation in

We aim to describe the technical pitfalls and different strategies in repairing a regurgitant aortic valve due to two different mechanisms/pathologies in two different patients. The first was related to endocarditis and the second was post-balloon dilation.

Essential thrombocythemia (ET) is a rare chronic myeloproliferative disorder characterized by elevated platelet counts and increased risk of thrombosis and hemorrhage. We report the case of a 75-year-old woman with ET who underwent aortic valve replacement for severe aortic stenosis using a 21 mm Edwards INSPIRIS bioprosthesis. Her postoperative course was uneventful, without thrombotic or hemorrhagic complications. She was managed with aspirin and warfarin for antithrombotic therapy, and hydroxyurea was used to control the platelet count. Perioperative management in ET patients undergoing car

BACKGROUND: Despite the promising results of both MitraClip and PASCAL systems for the treatment of mitral regurgitation (MR), there is limited data on the comparison of both systems regarding their safety and efficacy. We aim to compare both systems for MR.
MATERIALS AND METHODS: Five databases were searched until October 2024. Original studies were only included and critically appraised using an adapted version of the Newcastle-Ottawa scale for observational cohort studies and the Cochrane risk of bias tool for randomized controlled trials. The risk ratio (RR) and mean difference (MD) with

BACKGROUND: Transcatheter edge-to-edge repair (TEER) is increasingly used for mitral regurgitation (MR), particularly in high-risk patients. However, when TEER fails, patients often require mitral valve surgery, which remains challenging because of patients' compromised condition. This study aims to evaluate short- and long-term surgical outcomes following failed TEER.
METHODS: This meta-analysis included observational studies with reported outcomes of mitral valve surgery after failed TEER published until September 2024. Multivariable meta-regressions were performed to predict short- and lon

OBJECTIVES: Transcatheter mitral valve interventions (TMVIs) provide less invasive alternatives for patients deemed ineligible for surgery. This manuscript reviews the evolving landscape of TMVI, limited to a comparative analysis of transcatheter edge-to-edge repair (TEER) and transcatheter mitral valve replacement (TMVR) in terms of safety and efficacy outcomes.
METHODS: A comprehensive review of the existing literature was conducted, encompassing data from initial device trials to the most recent real-world studies, ranging from 2009 through 2025. A narrative synthesis was conducted to inte

The prevalence of calcific mitral stenosis (MS), which is associated with mitral annular calcification (MAC), has been increasing, particularly in aging populations such as in Japan. Severe MAC interferes with the normal diastolic relaxation of the mitral annulus, and calcification can extend onto the mitral leaflets with reduced leaflet mobility, causing MS. As MAC frequently coexists with aortic stenosis, aortic stenosis is a strong risk factor for calcific MS among patients with MAC. The advent and expansion of transcatheter aortic valve implantation revealed attendant challenges of calcifi

In recent years, transcatheter edge-to-edge repair (TEER) has been widely adopted as an effective treatment for mitral regurgitation (MR). The aim of this study is to develop a personalized in silico model to predict the effect of edge-to-edge repair in advance to the procedure for each individual patient. For this purpose, we propose a combination of a valve deformation model for computing the mitral valve (MV) orifice area (MVOA) and a lumped parameter model for the hemodynamics, specifically mitral regurgitation volume (RVol). Although we cannot obtain detailed information on the three-dime
