The acardiac anomaly is a rare condition found only in monozygotic multiple pregnancies, usually twins. We present a review of the pathophysiology of acardia and a case report. Twin embryonic membrane formation is discussed, since a conceptual understanding of the subject is necessary for the early recognition of this and other severe anomalies. Emphasis is placed on close sonographic monitoring to insure the best chance of survival of the normal twin, who may die of heart failure if delivery is not properly timed.

Case report concerning a young man in whom exudative retinitis had occurred as a result of the A-V shunt of a retinal angioma, leading to sudden loss of vision. With argon laser and xenon arc photocoagulation a minor hemorrhage cleared up spontaneously, the retina remained attached and vision increased to 20/25 within 1 year. The authors believe that the combined photocoagulation insured an interruption of blood flow in feeder vessels and the breakdown of the intervening capillary bed.

Three infusions of vincristine-loaded platelets were carried out in a young man with acute idiopathic thrombocytopenic purpura (ITP) refractory to steroids. This procedure, used until now only for chronic refractory ITP, insured a remission without side effects. After 2 years of follow-up the patient has not had relapse. A potentially dangerous splenectomy was thus avoided in a phase of the disease in which a spontaneous remission was still very probable.

Keloids are medically benign, but often psychologically and cosmetically malignant lesions. They are most commonly located on the posterior aspect of the ear lobes. The author's treatment of choice is the combined intralesional steroid and surgical approach. Surgically, the best results are obtained when some of the skin overlying the keloid is used as a split thickness graft after all of the keloidal tissue has been removed. Explicit postoperative wound care instructions are important in insuring complication-free surgery.

PURPOSE OF REVIEW: Relative value units (RVUs) are a primary metric composed of several components including physician work, practice expenses, and insurance costs, that drive physician reimbursement. Policymakers practice priority-setting, as either implicit or explicit rationing, embedded in evidence-informed decision-making processes to guide funding decisions. Health technology assessment (HTA) is a method that may assist explicit evidence-informed priority setting. South Africa developed an official HTA methods guide in 2022, however before this, commissioning and performing economic evaluations was not standardized.
