Structural equation analyses of clinical subpopulation differences and comparative treatment outcomes: characterizing the daily lives of drug addicts.

The use of structural equation modeling (SEM) is illustrated for comparative treatment outcome research conducted with heterogeneous clinical subpopulations within large multimodality treatment settings. All analyses are accomplished with SEM analogs of more familiar classical multivariate techniques. The effect of the early period of treatment on the daily lives of 486 clients in two drug abuse treatment modalities (methadone maintenance and outpatient counseling) is evaluated. Structured means analysis is used to assess initial differences between modalities on the latent means of 6 latent c

Pharmacokinetic population parameters for aminoglycosides in cholecystitis patients.

We report the use of a new method to determine patient population pharmacokinetic parameters (nonparametric expected maximum or NPEM). Our purpose was to develop and then analyze the utility of these parameters, compared to a more traditional approach. Nineteen patients with acute cholecystitis made up the control group for defining the parameters via NPEM. The standard of practice was to use a model created from a different intraabdominal infection group (appendicitis), referred to as "surgical patient model." These two models were compared with a group of 23 patients receiving gentamicin for

Association between vaginal douching and acute pelvic inflammatory disease.

The vaginal douching habits of 100 consecutive municipal hospital patients with verified pelvic inflammatory disease (cases) were compared with those of 762 randomly selected controls (random controls) and 119 women thought to have pelvic inflammatory disease but in whom the diagnosis was not confirmed by laparoscopy and/or endometrial biopsy specimen (internal controls). Because patients had been symptomatic for no more than 3 weeks, current douching was arbitrarily defined as any douching during the previous 2 months. Current douching was more common among those with pelvic inflammatory dise

Abuse and neglect of the frail elderly at home: an exploration of theoretical relationships.

Using an ex post facto, correlational descriptive design, a purposive sample of 74 elderly individuals who were identified as either having a 'good relationship' or an 'abusive/neglectful relationship' with their related caregivers were interviewed in their own homes. The purpose of the study was to test two hypotheses which predicted the nature of differences between the two groups and four which predicted the magnitude and direction of theoretically deduced causal relationships. The data showed that demographic characteristics and certain theoretical variable such as anger, hostility and str

Study of klotho gene transfer for the protective effect of the coronary of diabetic rats.

UNLABELLED: The authors model diabetes in rats, and inject the rats with Klotho gene. Then the levels of serum lipoprotein are tested, measuring the thickness of coronary artery intima and the ratio of intima and media to see whether Klotho gene has protective effect of coronary artery in diabetes rats. METHODS: Extracting the Klotho gene from kidney tissue in the normal SD rats amplifies the target gene by PCR and uses adenovirus as carrier. Then SD rats were randomly divided into model group, control group and the treatment group for diabetes modeling. Transferred the Klotho gene into treat

Ultrasound scatteromics: A multimodal QUS-based solution for detecting ambulatory function deterioration in Duchenne muscular dystrophy.

Quantitative ultrasound (QUS) envelope statistics imaging has been investigated as a non-invasive method for evaluating Duchenne muscular dystrophy (DMD). This study introduces an ultrasound scatteromics approach to differentiate between early and late ambulatory stages of DMD. A total of 47 DMD subjects were divided into early (n = 23) and late (n = 24) ambulatory stages. Ultrasound images of the gastrocnemius muscle were acquired and processed to generate multimodal QUS envelope statistics images based on the Nakagami distribution parameter m, homodyned K-distribution parameters α and k, and

Validation of CSR model to predict stroke risk after transient ischemic attack.

It is essential to identify high risk transient ischemic attack (TIA) patients. The previous study reported that the CSR (comprehensive stroke recurrence) model, a neuroimaging model, had a high predictive ability of recurrent stroke. The aims of this study were to validate the predictive value of CSR model in TIA patients and compare the predictive ability with ABCD3-I score. Data were analyzed from the prospective hospital-based database of patients with TIA which defined by the World Health Organization time-based criteria. The predictive outcome was stroke occurrence at 90 days. The receiv

Enhancing early detection of cognitive decline in the elderly: a comparative study utilizing large language models in clinical notes.

BACKGROUND: Large language models (LLMs) have shown promising performance in various healthcare domains, but their effectiveness in identifying specific clinical conditions in real medical records is less explored. This study evaluates LLMs for detecting signs of cognitive decline in real electronic health record (EHR) clinical notes, comparing their error profiles with traditional models. The insights gained will inform strategies for performance enhancement. METHODS: This study, conducted at Mass General Brigham in Boston, MA, analysed clinical notes from the four years prior to a 2019 diag

Optimal time window for initiating cefuroxime surgical antimicrobial prophylaxis in spinal fusion surgery: a nested case-control study.

BACKGROUND CONTEXT: Surgical site infections (SSI) are one of the common complications following spinal fusion surgery. Unfortunately, several studies had shown conflicting results regarding optimal timing of surgical antimicrobial prophylaxis (SAP) administration. Due to limitations in population homogeneity and sample size, these studies have not provided significant statistical correlations or clear practical recommendations. PURPOSE: The purpose of the study was to investigate the impact of timing of cefuroxime SAP on the risk of SSI in patients undergoing spinal fusion surgery, and to de

Bridging the Gap: Development of frontal crash mode ATD Analogous human body models.

Anthropomorphic variation is an important factor in computational studies using Human Body Models (HBMs), particularly regarding how such differences can influence observed kinematics and loading. Currently, a gap exists between Anthropomorphic Test Devices (ATDs) and human body models (HBMs). By necessity, there are differences in constitutive behaviors at a material level, however segment mass distribution and anthropometry differences can make matched simulations of ATDs and HBMs difficult to interpret, which has real-world implications for current or future regulatory applications. A machine learning model to predict the risk factors causing feelings of burnout and emotional exhaustion amongst nursing staff in South Africa.

BACKGROUND: The demand for quality healthcare is rising worldwide, and nurses in South Africa are under pressure to provide care with limited resources. This demanding work environment leads to burnout and exhaustion among nurses. Understanding the specific factors leading to these issues is critical for adequately supporting nurses and informing policymakers. Currently, little is known about the unique factors associated with burnout and emotional exhaustion among nurses in South Africa. Furthermore, whether these factors can be predicted using demographic data alone is unclear. Machine learn

Machine learning models to predict disease progression among veterans with hepatitis C virus.

BACKGROUND: Machine learning (ML) algorithms provide effective ways to build prediction models using longitudinal information given their capacity to incorporate numerous predictor variables without compromising the accuracy of the risk prediction. Clinical risk prediction models in chronic hepatitis C virus (CHC) can be challenging due to non-linear nature of disease progression. We developed and compared two ML algorithms to predict cirrhosis development in a large CHC-infected cohort using longitudinal data. METHODS AND FINDINGS: We used national Veterans Health Administration (VHA) data t

Enhanced dynamic risk stratification of smoldering multiple myeloma.

Accurate prediction of risk of progression from smoldering multiple myeloma (SMM) to active multiple myeloma (MM) is paramount to individualized early therapeutic strategies with minimum risk of overtreatment. Current risk stratification models do not account for evolving biomarker trajectories. We assembled a cohort of 2,344 patients with SMM from seven international centers with longitudinal clinical and biological data to train and validate the Precursor Asymptomatic Neoplasms by Group Effort Analysis (PANGEA)-SMM risk models. Four evolving biomarkers were significantly associated with shor

A prognostic nomogram to predict overall survival in patients with platinum-sensitive recurrent ovarian cancer.

BACKGROUND: Patients with platinum-sensitive recurrent ovarian cancer have variable prognosis and survival. We extend previous work on prediction of progression-free survival by developing a nomogram to predict overall survival (OS) in these patients treated with platinum-based chemotherapy. PATIENTS AND METHODS: The nomogram was developed using data from the CAELYX in Platinum-Sensitive Ovarian Patients (CALYPSO) trial. Multivariate proportional hazards models were generated based on pre-treatment characteristics to develop a nomogram that classifies patient prognosis based on OS outcome. We

Prediction of stenosis behaviour in artery by neural network and multiple linear regressions.

Blood flow analysis in the artery is a paramount study in the field of arterial stenosis evaluation. Studies conducted so far have reported the analysis of blood flow parameters using different techniques, but the regression analysis is not adequately used. Artificial neural network is a nonlinear and nonparametric approach. It uses back-propagation algorithm for regression analysis, which is effective as compared to statistical model that requires a higher domain of statistics for prediction. In our manuscript, twofold analyses of data are done. First phase involves the determination of blood

The need for better analysis of observational studies in orthopedics. A retrospective study of elbow fractures in children.

BACKGROUND AND PURPOSE: The conventional statistical methods employed in observational studies in orthopedics require the fundamental assumption that the outcomes are independent. However, fractures treated by the same surgeon cannot be regarded as being independent of each other and should be nested in the statistical analysis. If the effect on outcome of early rather than delayed surgery depends on the severity of the fracture, we have a case of interaction. This is rarely considered in orthopedic research, but could affect the conclusions drawn. The aim of this paper is to describe the conc

Prediction of Native Liver Survival in Patients With Biliary Atresia 20 Years After the Kasai Procedure.

OBJECTIVE: To establish risk models for long-term native liver survival (NLS) in patients with biliary atresia. METHODS: In this retrospective study, we analyzed data from 1792 patients registered in the Japanese Biliary Atresia Registry. Using multivariate logistic regression, we created predictive models for NLS at 1, 5, 10, 15, and 20 years postoperatively. Variables at the first year of age and those at 1 and 5 years postoperatively were used. The variables used in this model were selected using a forward-backward stepwise selection method. The models were further assessed using C-statist
