Foetal Akinesia Deformation Sequence: A Rare Lethal Entity.

Foetal akinesia deformation sequence (FADS) represents a group of disorders resulting from absent or diminished in utero foetal mobility. The aetiology is multifactorial, including genetic, environmental, maternal, and foetal causes. The absence of foetal movements leading to multiple joint contractures, pulmonary hypoplasia, and intrauterine growth restriction are the key features of foetal akinesia deformation sequence. Herein we describe the case of a 30-year-old gravida 4 (para 2+1) who came for foetal ultrasound at 28 weeks of gestation due to decreased foetal movements. Ultrasound showed

A Pilot Study Comparing Newly Licensed Drivers With and Without Autism and Experienced Drivers in Simulated and On-Road Driving.

This study compared newly licensed drivers with and without autism spectrum disorder (ASD) and experienced drivers. Twenty new drivers (8 with ASD) and 16 experienced drivers completed the Driving Attitude Scale (DAS) and drove a simulator and an instrumented vehicle. Heart rate (HR), galvanic skin response (GSR), wrist movement, eye-gaze and driving performance were monitored. ASD drivers had more negative attitudes toward driving and greater change in HR, GSR and wrist movement. In a driving simulator, drivers with ASD scored lower than NT drivers and were rated less safe. There were fewer d

Vergence Eye Movements: From Basic Science to Clinical Application - Foreword to the Special Issue.

The abstract book of the last European Conference on Eye Movements [1] lists abstracts of 373 presentations, but less than five percent investigate vergence eye movements, i.e. the coordination of the right and left eye. Why then a special issue on this neglected issue? Human vision under natural conditions involves both eyes in coordination controlled by interacting processes subsumed under the concept of vergence.. Further, vergence is important for people in their daily lives since disorders of vergence can have serious consequences: ophthalmologists deal with squinting patients on the basi

Association of facial massage, dry needling, and laser therapy in Temporomandibular Disorder: case report.

This case report aims to evaluate the treatment of Temporomandibular Disorder (TMD) of muscular origin by associating facial massage (FM), dry needling (DN), and low-level laser therapy (LLLT). The pre- and post-treatment evaluations consisted of clinical examinations based on the Research Diagnostic Criteria (RDC) for TMD, pain intensity quantification by means of the Visual Analog Scale (VAS), mandibular movement measurement, and electromyographic (EMG) analysis of the masseter and temporal muscles. Post-therapy assessment indicated a decrease in pain sites of 58%, in pain intensity mean (1.

Screening of mutations in GNAL in sporadic dystonia patients.

BACKGROUND: GNAL mutations have been shown to cause adult-onset isolated dystonia, a disabling movement disorder characterized by involuntary muscle contractions causing twisting and repetitive movements or abnormal postures. METHODS: To test the frequency of GNAL mutations in a series of 137 German patients with sporadic dystonia patients we used next-generation sequencing of amplicon-derived barcoded NexteraXT libraries for the coding exons and adjacent intronic sequences of GNAL. RESULTS: In our cohort we identified 1 pathogenic nonsense mutation (c.733C>T, p.R245*) in a patient with cerv

Narcolepsy-cataplexy: is streptococcal infection a trigger?

Narcolepsy-cataplexy is an uncommon sleep disorder which may present in childhood. We report a case of an 8-year-old presenting with narcolepsy-cataplexy following a streptococcal infection. Autoimmune etiology for narcolepsy has been suggested. In our patient increased anti-streptolysin O and anti-DNAse B titers were noted. As suggested by recent cases, the streptococcal infection was likely a trigger for narcolepsy onset in this genetically predisposed child. The patient was initially diagnosed as having Sydenham chorea due to motor movements. However, these transient movements may be due to

Clonazepam in the treatment of essential palatal tremors.

Essential Palatal tremor (EPT) is a rare disorder presenting as unilateral or bilateral rhythmic involuntary movements of the soft palate. There is mention of the utility of benzodiazepines like clonazepam probably because of their gamma amino butyric acid (GABA) agonistic property. But no reports are available for the same. Here we report a 30-year old married female patient who presented with the complaints of pain in the lower part of face, behind the ears, back side of neck and clicking sound in her. General physical examination (GPE) revealed symmetrical rhythmic flapping movements of the

Extranodal marginal zone B-cell lymphoma involving superior rectus muscle: A clinicopathological case report.

A 66-year-old female had suffered from proptosis in the left eye (OS) and double vision for 1 month due to abnormality of the superior rectus muscle. Visual acuity was noted as 20/20 in both eyes (OU). Eye movement showed limited OS supraduction. Magnetic resonance imaging revealed an indistinct mass in the orbit involving the superior rectus muscle. A biopsy specimen of the orbital tumor led to the histological diagnosis of extranodal marginal zone B-cell lymphoma. Radiotherapy with a total dosage of 30 Gy was administered, which subsequently resolved the tumor. However, the supraduction limi

Functional magnetic resonance imaging of tics and tic suppression in Gilles de la Tourette syndrome.

Tics are defined as involuntary, quick, sudden, and stereotypical movements or phonic productions. Despite the fact that tic suppression plays an important role for the patient's ability to cope with tic disorders, investigations of the underlying neural correlates using functional imaging focused on tic generation rather than tic suppression. We examined a patient with Gilles de la Tourette syndrome with regard to neural mechanisms of tic generation and tic suppression using fMRI. Three different conditions were compared: "tics", "tics suppressed", and "tics imitated". The comparisons of "tic

Is this dystonia?

Torsion dystonia is characterized by sustained muscle contractions causing twisting and repetitive movements and abnormal postures. The diagnosis can be made difficult, delayed, and often misled by several factors: variability of dystonia presentation, uncertain recognition of the specific physical signs, lack of diagnostic tests, wide etiological spectrum, and coexistence of other movement disorders. Diagnostic tools are of limited assistance for the diagnosis of dystonia, which remains based on clinical diagnostic skills. We propose here, a new diagnostic algorithm to systematize the clinica…

Using FMRI and FNIRS for localization and monitoring of visual cortex activities.

The purpose of this study was to design a near infrared spectroscopy (NIRS) sensor head to continuously monitor visual cortex activation. Visual cortex activation regions as a result of eye movements were localized using functional magnetic resonance imaging (FMRI). Once the region was determined we placed the NIRS sensor head on that region and emulated the same task perfomed in the FMRI experiment. The eye movement chosen for our current validation study was saccades. One subject was instructed to move their eyes in a saccadic fashion for 30 seconds then fixate for 30 seconds. We were able t

Magnetic resonance imaging does not always confirm left ventricular noncompaction.

Left ventricular hypertrabeculation/noncompaction (LVHT) is diagnosed echocardiographically or by other imaging techniques, like cardiac magnetic resonance imaging (CMRI). LVHT was diagnosed echocardiographically in a 48-year-old woman with peripheric embolism and a neuromuscular disorder of unknown etiology. The spongiform trabecular meshwork was located in the ventricular lateral and posterior wall. CMRI failed to visualize LVHT and showed only a homogenously thickened structure. The reason for overlooking LVHT on cardiac magnetic resonance imaging might be poor image quality due to movement

Oscillopsia without nystagmus caused by head titubation in a patient with multiple sclerosis.

Oscillopsia in patients who have brain stem disorders but not nystagmus is attributed to a failure of the vestibular-ocular reflex (VOR) to compensate for head movements. We report a patient who had marked head titubation and oscillopsia in aggressive multiple sclerosis but no nystagmus. Her severe head titubation precluded our ability to measure a VOR accurately. Because oscillopsia has also been described after rapid voluntary head oscillations in normal subjects, we queried whether the oscillopsia in our patient could be ascribed to the head movement alone. Six normal control subjects did n

Lamotrigine in two cases of Rett syndrome.

Rett syndrome (RS) is a particular neurodevelopmental disorder in which symptoms develop in early infancy and manifests particular symptoms at certain ages. Two girls aged 4.5 and 2.5-years-old were admitted with complaints of seizures, regression of language, psychomotor development, and autistic behavior. They had fulfilled the criteria of classic RS. Lamotrigine was used in the treatment of the convulsions. After lamotrigine therapy, the convulsions were successfully controlled and the stereotyped hand movements and autistic behaviors markedly decreased. We would like to stress that, aside

Upper-arm elevation during office work.

The present aim was to measure and quantify upper-arm elevation and to find how changed work organization and work tasks influence arm movement during a working day. Sixteen female office workers participated in the study. Their main work was statistical data entry. Upper-arm elevation was measured on two occasions separated by 18 months, i.e., before and after a change of work organization. The measurements were performed during the whole of one ordinary working day. The differences between the two measurements were mostly non-significant. Arm elevation remained essentially below 30 degrees d

Congenital ocular motor apraxia with torsional oscillations: a case report.

Congenital ocular motor apraxia (C-OMA) is an infrequently reported eye movement disorder associated with malformation or delayed maturation of the brain. Patients with C-OMA are unable to initiate voluntary horizontal saccades. This results in characteristic head thrusts into the desired field of gaze to compensate for the lack of saccadic ability. Careful examination of an 8-year-old Hispanic male revealed uncoordinated eye movements, lack of voluntary saccades, head thrusts, and gross/fine motor delays all consistent with congenital ocular motor apraxia. Torsional oscillations that resemble

Tardive and chronically recurrent oculogyric crises.

Six patients with chronically recurrent oculogyric crises (OGC) are reported. Four of these were derived from a study of 100 schizophrenic patients on maintenance neuroleptic medication, thereby giving a prevalence of 4% in such patients. Three of the six had the OGC develop as a tardive side-effect, and in one patient the episodes persisted for some months after the cessation of the offending neuroleptic drug. The episodes of ocular dystonia were associated with other dystonic movements and a number of psychiatric symptoms, with obsessional thoughts and hallucinations being the outstanding fe

Gait apraxia in normal-pressure hydrocephalus: patterns of movement and muscle activation.

We made gait recordings in 11 patients with normal-pressure hydrocephalus. Sagittal rotations in hip, knee, and ankle joints were determined with intermittent light photography or with polarized light goniometry. In eight patients, gait muscle activation was determined with integrated surface EMG from six leg muscles on one side. We assessed the patterns of movement and muscle activity by averaging data from 20 gait cycles. Reduced speed and range of movements, short steps, small foot-floor clearance, and low swing-to-stance ratio were typical. The activation of the calf muscles was regularly
