AI-Assisted Carotid Ultrasound for Nonexpert Use in Primary Care Cardiovascular Prevention: User-Centered Iterative Development Study.

BACKGROUND: Carotid ultrasound is traditionally confined to specialist settings because examination quality and diagnostic reliability are highly dependent on the operator's technical skill and experience. Recent advances in AI-assisted ultrasound may enable task shifting to primary care staff and support the integration of personalized visual risk communication into routine preventive care. Visualization of subclinical atherosclerosis has been shown to strengthen cardiovascular risk communication and support preventive engagement. However, limited evidence exists on how such technology can be

Optimizing Nurses' Time: Reducing Assessment Frequency in General Care.

BACKGROUND: Nurses represent a limited resource in the hospital environment. For decades, inpatient nurses have conducted head-to-toe assessments in roughly the same manner. At Mayo Clinic in Rochester, Minnesota, the practice has been twice-daily head-to-toe assessments, without reviewing the benefit of this frequency (vis-à-vis once daily) for patients, staff, and the institution. PURPOSE: This quality improvement (QI) project in a general care setting explored whether decreasing comprehensive physical assessments from twice daily to once daily was associated with changes in patient safety

Latent profiles of teaching self-efficacy and clinical teaching behaviors among clinical nurse preceptors and factors associated with profile membership: a cross-sectional study.

BACKGROUND: Clinical nurse preceptors play a central role in supporting nursing students and newly employed nurses during clinical practice. Teaching self-efficacy may be related to preceptors' clinical teaching behaviors, yet little is known about how these psychological beliefs and observable behaviors co-occur within level-based profiles. This study aimed to identify latent profiles of teaching self-efficacy and clinical teaching behaviors among clinical nurse preceptors and examine factors associated with profile membership. METHODS: A cross-sectional survey was conducted from February to

Global perspectives on challenges, coping strategies, and future preparedness of nursing home staff during COVID-19: a systematic review and meta-synthesis.

BACKGROUND: The global public health crisis caused by COVID-19 in late 2019 was unprecedented. Due to their vulnerable population, nursing homes are a key epidemic response area. This study described the challenges and coping strategies of nursing home staff during COVID-19 and proposed recommendations for future public health crises in nursing homes. METHOD: A meta-synthesis was performed to address the research question: What are the experiences of nursing home staff from the perspectives of COVID-19? From the beginning until August 31, 2024, searches were conducted in five international da

Wound care across diverse skin tones in the community nursing context: a critical narrative analysis.

Wound assessment in the UK community setting remains heavily reliant on visual cues, such as erythema, which are unreliable in darker skin tones. This continued reliance on a light-skin norm represents a form of hidden systemic bias that risks delayed diagnosis and avoidable harm for a growing segment of the population. This article draws on empirical and conceptual literature to develop a critical narrative analysis of how skin tone diversity affects wound assessment, diagnosis and management in UK community nursing practice. It synthesises evidence on diagnostic delays, critiques the inadequ

Rethinking Models of Chronic Kidney Disease Care: A Narrative Review.

BACKGROUND: Chronic kidney disease (CKD) affects over 850 million people worldwide and is associated with significant morbidity and mortality. There has been recent expansion in treatment options to reduce CKD progression and cardiovascular risk, and it is essential that this translates into clinical practice. SUMMARY: The primary objectives of this review were to outline current CKD care models and associated care gaps and review the literature for alternative care models, with a focus on the early detection and management of CKD. Several care models have been proposed to improve CKD managem

Peripheral Nerve Blocks Causing Increased Risk for Fall and Difficulty in Ambulation for the Hip and Knee Joint Replacement Patient.

A systematic review of the literature was completed by the Evidence-Based Practice Group for the Patient population, Intervention/Issue, Comparison Intervention, Outcomes, Timing (PICOT) question: "Does the use of a peripheral nerve block increase the risk for falls and difficulty ambulation in patients after lower extremity surgery through postoperative day 2?" A search of multiple databases using specified key terms resulted in 258 articles for total knee arthroplasty or total hip arthroplasty. These were reduced to 13 with exclusion criteria and became primary evidence. Numbers Needed to Ha

The impact of frailty in the elderly on the outcome of surgery in the aged.

As the population continues to age, we will continue to encounter issues involving aging and the elderly. Despite these issues, knowledge is expanding and evolving with new solutions to ongoing problems. Mechanistically, frailty at its root is a symptom of growing old, with cascades and circuitous feedback between organ systems at all levels. Clinically, frailty is as equally dynamic and its multifactorial nature represents a unique challenge to the surgical community and warrants the integration of geriatric assessment into clinical practice. Integration within clinical practice includes usin

Provider confidence in breast examination.

Many physicians have reported interest in further training in clinical breast examination, and it has been speculated that health care providers may lack confidence in their breast examination skills. At a multisite family practice residency, we surveyed a group of physicians and nurse practitioners about confidence in clinical breast examination. Forty-three percent of respondents reported impairment of the clinical breast examination by lack of confidence in skills, and 50% reported feeling only "somewhat" or "moderately" confident in their clinical breast examination skills. Residents often

Blood-pressure screening and supervision in general practice.

Since April 1975 all men aged 35-69 years registered with four general practices in west central Scotland have had their blood pressure checked whenever they visit the surgery. Although the practice locations range from rural to city centre and observers comprise receptionists, nurses, and doctors, a standard procedure has been adopted for the examination, recording, follow-up, and management of high blood pressure. The results confirm that raised blood pressure is common and often goes undetected. Even when hypertension is known, casual blood pressure readings often exceed accepted normal lev

Clinicians' Perceptions About Institutional Factors in Moral Distress Related to Potentially Nonbeneficial Treatments.

IMPORTANCE: Clinicians commonly experience moral distress related to potentially nonbeneficial life-sustaining treatments (LST). Hospitals' institutional culture (eg, shared beliefs, values, and practices), structures (eg, policies, practices, resource allocation), and societal-level factors (eg, national culture, local and national policies, medical hierarchies) may contribute to moral distress related to potentially nonbeneficial LST. OBJECTIVE: To investigate clinicians' perspectives on how hospitals' institutional culture and structures might exacerbate, prevent, or mitigate the influence

[Screening for iron deficiency and pneumococcal vaccination: What role do they play in community cardiology?].

This article presents a single-center, cross-sectional, descriptive epidemiological field study evaluating the screening for iron deficiency and pneumococcal prevention by community cardiologists in patients with heart failure. In total, 90% of the practitioners surveyed reported being familiar with the European Society of Cardiology's recommendations regarding iron deficiency screening, but only 60% perform it at least once a year. In contrast, 90% do not assess the patient's pneumococcal vaccination status. Nearly all respondents stated they are willing to collaborate with an advanced practi

Nursemaid's Elbow Reduction.

Nursemaid's elbow, also known as radial head subluxation, is a common childhood orthopedic injury that can easily be diagnosed and reduced by the advanced practice nurse. It is most common in children 1-4 years of age and typically occurs as the result of a pulling mechanism on an outstretched arm. This leads to subluxation of the radial head at the annular ligament. The child subsequently refuses to use the affected arm, leading the caregiver to present for evaluation. This article explores epidemiology, pathophysiology, clinical presentation, reduction techniques, and parent education.

Providing quality care to the sexual assault survivor: education and training for medical professionals.

Health care providers who perform sexual assault examinations can assist patients who report experiencing a sexual assault by increasing their knowledge and skills in sexual assault care, and serving as Sexual Assault Response Team (SART) coordinators, or as SART team members, in their communities. With little additional material, this training could be provided as a component of basic midwifery and/or advanced practice nursing education programs. This article reviews the essential steps and required training for conducting sexual assault examinations.
