Incidents of catastrophic overhydration (maternal and neonatal) identified through medico-legal reviews.

The authors describe two incidents where a mother postpartum and a neonate in the nursery were grossly overhydrated in tertiary care medical centers. In the first case, while receiving intensive care for HELLP (hemolysis, elevated liver enzymes and low platelets) syndrome, involving the participation of eight specialists and subspecialists, the weight of the mother increased by 54% between the 2nd and 8th postpartum days. In the second case, between the time of his birth and his demise 13 days later, the weight of the newborn child increased by 67%. In neither of these cases, was the significa

Pediatric fatality secondary to EDTA chelation.

BACKGROUND: Chelation therapy has emerged as a popular treatment modality to remove heavy metals that are thought to cause autism. We report a fatality that occurred as a consequence of chelation therapy for autism when the incorrect form of EDTA was administered. CASE REPORT: A five-year-old autistic male was being chelated in a physician's office. While receiving his third treatment he went into cardiac arrest. It was not determined until after the child's death that he had been given edetate disodium rather than edetate calcium disodium, causing profound hypocalcemia and triggering the car

Rituximab administration in third trimester of pregnancy suppresses neonatal B-cell development.

We describe the effect on the neonate of administration of rituximab to a woman with idiopathic thrombocytopenic purpura (ITP). Rituximab, an anti-CD20 antibody, was given weekly for 4 weeks to a woman with ITP in her third trimester of pregnancy. One month after the last rituximab administration a healthy girl was born. She had normal growth and development during the first six months. At birth, B-lymphocytes were not detectable. Rituximab levels in mother and neonate were 24000 and 6700 ng/mL, respectively. Only 7 cases of rituximab administration during pregnancy were described. No adverse

Mirror gametes donation.

Candidate recipients of donor gametes have a moral obligation to make a contribution to the system from which they benefit by reciprocating. They can do this as a couple when the partner of the person who has no usable gametes donates and receives the mirror gametes in return. In an indirect exchange system, the couple receives bonus points for their donation so that their waiting time for the gametes is reduced or even fully canceled. A system of strict reciprocity is rejected as unfair because it excludes categories of people from access to donor gametes. Also candidate recipients who for me

WAGR syndrome: is the 'R' always justified?

Although mild-to-moderate intellectual disability is usually considered part of WAGR syndrome (Wilms' tumour (WT), Aniridia, Genital abnormalities, and metal Retardation, due to 11p13 deletion) the neuropsychological profile of the syndrome is little reported in the literature. We report about a 12-year-old boy presenting with WAGR syndrome (WT, right complete aniridia, bilateral cryptorchidism, interstitial deletion involving band 11p13) but with no mental retardation. An in-depth clinical evaluation revealed no behavioural or social problems and the child's neuropsychological profile was fou

Oblique anastomosis in Soave endoanal pullthrough for Hirschsprung's disease--a way of reducing strictures?

The Soave endorectal pullthrough is a commonly performed procedure for the definitive management of children with Hirschsprung's disease (HD). Anastomotic stricture is a recognised complication of this procedure. There are multiple causes for these strictures, circular anastomosis being one of them. There are techniques described which alter the shape of the anastomosis of the pulled through bowel to decrease the incidence of strictures. These are oblique and heart-shaped anastomoses. We describe a new technique of oblique anastomosis where the pulled through bowel is anastomosed posteriorly 0

Genetics and genetic counseling: practices and opinions of primary care physicians in Turkey.

PURPOSE: The purpose of this study was to assess the educational needs of physicians relating to genetics and genetic counseling in the Denizli region of Turkey. METHODS: Data were collected by questionnaire about physicians' approaches to genetics and genetic counseling. RESULTS: A total of 60 (60.0%) of 100 physicians working in Denizli province returned a questionnaire. Physicians described "their most knowledgeable subjects" in basic genetic information as chromosome abnormalities (41.8%), in genetic disorders as xeroderma pigmentosum (80.0%), and in genetic counseling as directing the p

Pain- and distress-reducing interventions for venepuncture in children.

OBJECTIVE: To compare the effect of eutectic mixture of local anaesthetics (EMLA) and a placebo cream on reported pain and observed distress associated with venepuncture, and to investigate effects of procedural information before and distraction during venepuncture. METHODS: Children 3-12 years of age undergoing venepuncture under five experimental and a control condition reported their pain at venepuncture on visual scales. Distress was observed when the child entered the waiting room, just before, and during venepuncture. RESULTS: Distress increased over the measurement occasions, but a d

Towards a better understanding of the aetiology of Legg-Calvé-Perthes' disease: acetabular retroversion may cause abnormal loading of dorsal femoral head-neck junction with restricted blood supply to the femoral epiphysis.

The most accepted part of the aetiology of Perthes' disease is an intermittent arterial occlusion. Many different reports now also suggest that some mechanical factor plays a role. In search of mechanical differences between normal hips and Perthes hips, many secondary morphologic differences of the proximal femur and the acetabulum are known. Recent research by the author has shown morphological changes of the acetabulum, independent of Perthes' disease in a large part of patients. In particular, retroversion of the acetabulum was found in a large part of adults who suffered from Perthes' dis

Infantile erythema elevatum diutinum: report of a vesiculo-bullous case.

Erythema elevatum diutinum (EED) is a localized, low-grade form of leukocytoclastic vasculitis mainly affecting adults, with only a few pediatric cases reported in the literature. The pathogenesis of this dermatosis is unknown, although an Arthus-type reaction to bacterial and viral antigens seems involved. We describe a case of EED in a 9 year-old child suffering from recurrent pharyngotonsillitis induced by Streptococcus pyogenes. Clinical presentation of the dermatosis was atypical, with vesicles and bullae intermingled with red-purple papules and plaques distributed mainly over the extenso

Nonoperative treatment of traumatic pancreatic duct disruption in children with an endoscopically placed stent.

The presence of ductal injury is the main determinant of consequence and a cause of significant mortality and morbidity in children with blunt pancreatic trauma. Proper treatment must be initiated on the basis of accurate anatomic diagnosis of the type and location of the injury. Computed tomography is an insufficient method for the diagnosis of the type and location of pancreatic ductal injury. Endoscopic retrograde pancreatography (ERP) is a reliable technique for determining the status of the pancreatic duct in children and may allow for definitive treatment of ductal injury by stenting in

Non-anaplastic pleomorphic xanthoastrocytoma with neuroradiological evidences of leptomeningeal dissemination.

CASE REPORT: A case of a non-anaplastic pleomorphic xanthoastrocytoma (PXA) presenting with leptomeningeal dissemination (LMD) affecting a 9-year-old girl is presented. DISCUSSION: The neoplasia in this young girl had the otherwise classical clinical features of PXA: the relatively advanced paediatric age of the patient, the seizures as presenting sign; the primary site in the temporal lobe; and the MRI findings of the partially solid and cystic superficial lesion. Only the tumour involvement of the chiasma and the infundibulus was a relatively unusual finding. In a 5-year period, the tumour

[DiGeorge syndrome, a review of 52 patients].

UNLABELLED: The deletion of chromosome 22q11.2 is involved in the majority of DiGeorge or velo-cardiofacial syndrome. The phenotypic variability was noted in the "CATCH 22" acronym. This acronym doesn't recapitulate the full spectrum of the symptoms. The diagnosis of this syndrome can be done with the prenatal diagnosis, with fetal pathology or with a child alive. METHODS: Review of 52 cases with the microdeletion 22q11. Six cases were diagnosed during the prenatal period, 12 cases at fetal pathology examination, and 34 cases during infancy. RESULTS: Cardiac malformations were the major indi

Porcelain gallbladder with extrahepatic bile duct obstruction in a child.

An extrahepatic bile duct obstruction was diagnosed in a 13-year-old boy presenting with pruritus, abdominal pain and jaundice. Several weeks after sphincterotomy and biliary stenting via endoscopic retrograde cholangiopancreaticography which relieved the obstruction, the patient was operated on. Severe fibrosis encased the extrahepatic biliary tract, so only cholecystectomy was performed because planned hepaticojejunoanastomosis could jeopardize the vascular supply to the liver. Histopathology showed calcification of the gallbladder wall and chronic fibroproliferative changes in the surroundi

Comparison of percutaneous and surface functional electrical stimulation during gait in a child with hemiplegic cerebral palsy.

The purpose of this brief report was to compare the immediate effects of surface functional electrical stimulation (S-FES) and percutaneous functional electrical stimulation (P-FES) of the tibialis anterior applied during gait in a child with hemiplegic cerebral palsy. A three-dimensional gait analysis was conducted while an 11-yr-old girl with right hemiplegia walked with S-FES, P-FES, and no stimulation. The results indicated that both P-FES and S-FES increased dorsiflexion at initial contact, peak dorsiflexion in swing, and mean dorsiflexion in swing compared with walking without stimulatio

Maternal brain death, pregnancy and the foetus: the medico-legal implications for Ireland.

This paper examines some of the medico-legal issues that arose as a result of a situation which occurred in May 2001 in Ireland when a woman who was a British citizen and who was fourteen weeks pregnant collapsed and suffered a brain haemorrhage. She was taken to hospital where she was placed on life support but declared brain-dead. As a result of the uncertainty regarding the hospital's obligation to the foetus, life-support was maintained until further opinion was sought. After two weeks the foetus died and life support was only then discontinued. In Ireland there currently exists neither me

Dissecting giant vertebro-basilar aneurysms.

OBJECT: Vertebro-basilar dissecting aneurysms (VDAs) are very rare in children. Strokes or subarachnoid hemorrhage are characteristic clinical manifestations, but often only non-specific symptoms are found. Radiological diagnosis may be difficult to obtain and therapy hard to perform. We report on a child who presented with an intracranial mass effect due to a giant VDA. METHODS: The patient was a 8-year-old girl with right hemiparesis, ataxia, dysphagia and dysphonia and worsening of her neurological deficits. Neuroimaging discovered a giant dissecting aneurysm arising from the left vertebra

Lumbar disc herniation in early childhood: case report and literature review.

INTRODUCTION: Lumbar disc herniation is a common condition in adults, uncommon in adolescents and exceptional below the age of 10 years. CASE REPORT: We report on a 6-year-old boy who presented with low-back pain and sciatica after having sustained an injury in a rear-end automobile accident. Due to our limited experience with protruded discs in this age group, we decided to be conservative initially. However, the persistence of incapacitating pain prompted surgical treatment. Operative findings consisted of a torn annulus, attached at one end to the cartilaginous plate, which formed a major

Aggressive CD34-positive fibrous scalp lesion of childhood: extrapulmonary solitary fibrous tumor.

Although solitary fibrous tumor (SFT) was originally described as a pleural tumor, an increasing number of extrapleural sites of SFTs have been documented. This has been attributed not only to the heightened awareness of the spectrum of histopathological features that characterizes SFTs but also to the recognition of the role of CD34 immunostaining in soft tissue tumors in general, and in SFTs in particular. Despite the large number of documented extrapleural SFTs in adults, cranial SFTs are rare, having been documented in the meninges, scalp, and infratemporal fossa. Extrapleural SFTs are, to
