The treatment of complex proximal humerus fractures that involve displacement of the greater tuberosity remains challenging. Good functional outcomes and decreased pain are possible with open reduction and internal fixation. Keys to a successful outcome include preservation of blood supply through decreased soft-tissue stripping, restoration of normal anatomic relationships (particularly the greater tuberosity), and stable internal fixation that allows early range of motion. To attain these goals, we advocate fixation with a fixed-angle locking proximal humerus plate using a 2-incision approac

Eight children with type II and III fractures of the intercondylar eminence of the tibia were treated. The osteochondral fragments were reduced arthroscopically and held with Kirschner wires with the knee in extension. The wires were then incorporated into a long leg plaster. There was no associated significant collateral ligament or meniscal damage in any of the knees requiring operative intervention. At follow-up all the fragments had healed. None had symptoms of ligamentous laxity. One knee showed increased antero-posterior laxity compared with his normal knee associated with a 20 degree bl

Conservative treatment of acetabular fractures mean a long period with immobilisation followed by quite a few of complications that are life-threatening to old patients. Besides that these fractures often cause painful posttraumatic osteoarthritis of the hip joint. Open reduction and osteosynthesis is favored more and more, but the complications described cannot be avoided absolutely by surgical treatment. The sequelae are risky reoperations and the necessity for implantation of total joint prosthesis. As shown in the enclosed case reports, total hip replacement can be emphasized as first step

Fifteen patients having undergone open reduction and internal fixation for scaphoid fractures were later examined by computer tomography. Herbert-screws were used in nine cases and Ender-plates in four. In two cases, Ender-plates replaced a small fragment AO-screw as well as one Herbert-screw. Computer tomography of the wrist is not indicated on a routine basis, but whenever standard X-rays are unable to determine whether surgically treated fractures have healed or nonunion has been established. Patient cooperation is of utmost importance, if artifacts due to patient movement are to be avoided

Nine male patients were treated for condylar fractures of the mandible by a method involving detachment of the condylar head. Pre-operatively, all patients had severe malocclusion. Radiologically, most condyles were anteromedially displaced. Open reduction was indicated but repositioning of the condyle was impossible without ramus osteotomy followed by temporary condylectomy. On follow-up roughly 32 months after surgery no permanent complications were seen. Joint function was almost completely restored. Radiographic evaluation revealed some shortening and deformation of the condyles which had

PURPOSE OF THE STUDY Fractures of the scapula are less frequent, with the incidence reported in literature of approximately 1% of all fractures and 3-5% fractures of shoulder. These fractures are predominantly treated non-operatively. Osteosynthesis is indicated in displaced intra-articular fractures and severely displaced extra-articular fractures of the scapular body, its lateral border in particular. Apart from open reposition and osteosynthesis, also minimally invasive osteosynthesis under fluoroscopic and arthroscopic control has recently been used to treat intra-articular fractures of th

BACKGROUND: Among diabetics, patients with peripheral neuropathy are at increased risk of developing complications following an ankle fracture. While the outcomes in these patients treated nonoperatively have been poor, the outcomes in those undergoing open reduction and internal fixation are at the best modest. We hypothesize that closed reduction and internal fixation with tibiotalocalcaneal nail is an effective primary procedure in this complication prone patient group.
METHODS: A retrospective review of diabetic patients with peripheral neuropathy who underwent acute treatment of an ankle

Complex polytrauma resulting from high-energy trauma may necessitate interdisciplinary care. A rare pelvic injury called locked (overlapping) pubic symphysis is commonly linked to unstable pelvic ring disruption and frequently necessitates surgery. A 25-year-old man sustained a high-energy crush injury after a large rock fell onto him. The patient presented to King Hussein Medical City Emergency Department on February 5, 2026. After initial assessment, imaging demonstrated a locked pubic symphysis with unstable pelvic ring disruption, a right sacral fracture extending into the neural foramina,

INTRODUCTION: Pelvis and acetabulum fractures are serious injuries, especially in polytrauma patients. Traditionally, open reduction with internal fixation is the way to go; fortunately, percutaneous fixation has evolved with the advancement of understanding pelvis anatomy and fixation pathways. Nonetheless, the debate regarding indications, safety, radiation exposure, and other complications emerged. Our paper presented a reverse guide wire technique for percutaneous anterior column/superior pubic rami intramedullary screw fixation.

INTRODUCTION: Carpometacarpal (CMC) joint dislocations and fracture-dislocations are infrequent and often challenging to diagnose. Among these, dorsal fracture-dislocations involving the capitate and third metacarpal, as well as the hamate with the fourth and fifth metacarpals, represent an exceptionally rare subset. These injuries are typically the result of high-energy mechanisms, such as axial loading transmitted through a clenched fist striking a solid surface. Their subtle clinical signs and often inconspicuous radiographic findings, particularly in the context of polytrauma, contribute t

BACKGROUND: The treatment of Vancouver B periprosthetic proximal femur fractures (PPFFs) is complex due to the overlap between arthroplasty and orthopedic trauma techniques. Our purpose was to assess the effects of fracture type, treatment difference, and surgeon training on the risk of reoperation in Vancouver B PPFF.
METHODS: A collaborative research consortium of 11 centers retrospectively reviewed PPFFs from 2014 to 2019 to determine the effects of variations in surgeon expertise, fracture type, and treatment on surgical reoperation. Surgeons were classified as per fellowship training, fr

BACKGROUND: Unlike articular shear fractures of the distal radius, radiocarpal fracture-dislocations defined as complete dislocation of the lunate from its articular facet of the radius are relatively uncommon. The management principles of these fractures have not been defined and there is no consensus on approach to management of these injuries. The aim of this study is to review our series of radiocarpal fracture-dislocations and propose a radiographic classification to guide surgical management.
METHODS: This study is reported based on STROBE guidelines. A total of 12 patients underwent op

The feasibility of a user-specific finite element model for predicting the in situ strength of the radius after implantation of bone plates for open fracture reduction was established. The effect of metal artifact in CT imaging was characterized. The results were verified against biomechanical test data. Fourteen cadaveric radii were divided into two groups: (1) intact radii for evaluating the accuracy of radial diaphysis strength predictions with finite element analysis and (2) radii with a locking plate affixed for evaluating metal artifact. All bones were imaged with CT. In the plated group…

OBJECTIVES: Little data exist regarding the outcomes of total hip arthroplasty (THA) after acetabular fracture treatment failure. We hypothesize that these patients achieve a lower level of function than those who undergo primary THA for osteoarthritis (atraumatic).
DESIGN: Retrospective review. Control group consisted of sequential patients who underwent a primary THA for osteoarthritis and were 60 years or older at the time of surgery.
SETTING: Level I Academic Trauma Center.
PATIENTS: One hundred seventy-one patients older than 60 years when they sustained an acetabular fracture were inc

The paper describes a rare fracture of proximal femur, classified by Delbet (Am J Surg 6:793-797, 1929) as type I (transepiphyseal type) combined with a fracture of the midshaft of ipsilateral femur in a 2-year-old child. Immediate operation with open reduction and internal fixation was successful. During the postoperative course, avascular necrosis (AVN) of capital femoral epiphysis was seen by bone scan. Applying an abduction orthosis used for the treatment of Legg-Calvè-Perthes disease, collapse of capital epiphysis was prevented. Although, a minimal area of AVN and coxa vara remained, no c

UNLABELLED: Fracture-dislocations of the Lisfranc joint associated with plantar soft tissue defects are difficult to reconstruct because both the Lisfranc joint and plantar soft tissue defects must be repaired. Inadequate reduction and fixation of the Lisfranc joint causes persistent weight-bearing foot pain, and an inappropriate choice of soft tissue reconstruction for plantar weight-bearing areas causes ulcers.
CASE 1: A 48-year-old man sustained a fracture-dislocation of the Lisfranc joint. Soft tissue defects occurred in the dorsal and plantar parts beyond the forefoot, exposing the metat

The acute traumatic rupture of the tibialis posterior tendon in association with closed ankle fractures is rare and often under-recognised. If recognised early, outcomes can be excellent. There are 28 known cases in the literature, and we report two further cases associated with bimalleolar ankle fracture dislocation. A 49-year-old presented with valgus deformity at the ankle joint and global tenderness following a work injury as a mechanic. A plain radiograph showed a displaced oblique comminuted fracture of the lateral malleolus with valgus angulation at a syndesmosis, with significant talar
