Peritoneal Inclusion Cysts Presenting as a Femoral Hernia: A Case Report.

Peritoneal inclusion cysts (PICs) are rare benign mesothelial cystic proliferations, typically occurring in women with prior abdominopelvic pathology. We report a case of a woman in her 70s presenting with an irreducible groin mass, found to have a concurrent fatty indirect inguinal hernia and femoral hernia containing multiloculated PICs in the absence of known risk factors, including prior abdominopelvic surgery or inflammation. Preoperative ultrasound suggested a hydrocoele of the canal of Nuck.  Intraoperative exploration revealed a multiloculated cystic lesion within the femoral canal con

Peritoneal Tuberculosis Presenting as Massive Ascites in Pregnancy: A Case Report.

Abdominal tuberculosis, especially peritoneal tuberculosis, is uncommon in pregnancy. The diagnosis during pregnancy is a challenge due to many signs and symptoms being nonspecific and mimicking the symptoms of pregnancy. The presence of ascites, with anorexia and weight loss in pregnancy from endemic regions, should be addressed with suspicion of abdominal tuberculosis.

Peritoneal strumosis and the role of imaging in the diagnostic workup: A case report.

Peritoneal strumosis, also termed highly differentiated follicular carcinoma of ovarian origin (HDFCO), is a rare entity arising from struma ovarii, with extra-ovarian dissemination of thyroid tissue. Radiologic descriptions remain limited. We report a case of a 38-year-old woman with a known diagnosis of struma ovarii, a monodermal teratoma mainly of benign thyroid tissue, who presented with multiple peritoneal lesions detected on imaging. Magnetic resonance imaging (MRI) revealed diffusion-restricting enhancing soft tissue implants throughout the abdomen and pelvis, while nuclear medicine im

Peritoneal mesothelioma as an incidental finding during inguinal hernia surgery.

Malignant peritoneal mesothelioma is a rare tumor disease, usually associated with asbestos exposure, which is often only diagnosed at an advanced stage. We report the case of a 52-year-old patient who was found to have marked ascites during surgery for a recurrent inguinal hernia. Subsequent diagnostics, including diagnostic laparoscopy, established the diagnosis of malignant epithelioid peritoneal mesothelioma. After tumor reduction by omentum resection and hyperthermic intraperitoneal chemotherapy, a complete remission was achieved. The case underlines the importance of differentiated clari

Malakoplakia mimicking pelvic malignancy with liver metastasis and peritoneal carcinomatosis.

Malakoplakia is a rare chronic inflammatory condition characterised by defective macrophage function, most commonly affecting the genitourinary tract. Although usually localised, it may occasionally present as a mass-forming lesion mimicking malignancy. We report the case of a 46-year-old woman presenting with a pelvic mass, acute kidney injury, and hydroureteronephrosis. Imaging demonstrated a necrotic 10.3cm pelvic mass with lymphadenopathy and hepatic lesions, raising suspicion of metastatic gynaecological malignancy. Surgical exploration identified a fibrotic, thickened pelvic peritoneum w

What is worth knowing about peritoneal metastases in colorectal cancer?

Peritoneal metastases (PM) from colorectal cancer (CRC) represent a unique clinical challenge with distinct biological behavior and therapeutic implications. Although PM has traditionally been associated with poor prognosis, recent advances in diagnostics, surgical techniques, and intraperitoneal therapies have offered selected patients opportunities for prolonged survival and, in some cases, long-term disease-free status. This review discusses the pathophysiology, risk factors, diagnostic strategies, and treatment options for CRC patients with PM. The peritoneum is the second most common site

Long-term success of percutaneously placed peritoneal dialysis catheters.

Percutaneous peritoneal dialysis (PD) catheters demonstrate short-term outcomes comparable to surgically placed catheters but remain underutilized because of concerns regarding long-term durability and limited follow-up data. We retrospectively reviewed 117 PD catheters in 90 patients who initiated PD between 2014 and 2020 at a freestanding home dialysis unit, with follow-up through December 2022. Eighty-three catheters were placed percutaneously by interventionalists and 34 laparoscopically for suspected hernias, prior abdominal surgery, or refractory peritonitis. The primary outcome was cath…

Rapidly accumulating ascites as an early clue to encapsulating peritoneal sclerosis.

Early identification of encapsulating peritoneal sclerosis (EPS) remains challenging, particularly in the absence of definitive radiological signs. We report a case of a 69-year-old male with end-stage kidney disease on peritoneal dialysis (PD) for 9 years, who developed poor appetite and weight loss. After complete drainage of ascites, PD was ceased for 1 day, during which 500 mL of newly formed ascites accumulated. Ascitic interleukin-6 and CA-125 levels were markedly elevated, raising concern for evolving EPS. Although computed tomography showed bowel dilatation and focal peritoneal thicken

Textbook outcome in ovarian cancer with peritoneal carcinomatosis: a systematic review.

OBJECTIVE: Textbook Outcome (TO) has emerged as a composite quality metric in oncologic surgery, but its role in ovarian cancer-related peritoneal carcinomatosis remains unclear. This systematic review aimed to evaluate the definitions, achievement rates, and clinical relevance of TO in patients undergoing cytoreductive surgery for ovarian cancer with peritoneal carcinomatosis. METHODS: A systematic literature search was conducted in PubMed, Embase, Scopus, and Web of Science following PRISMA 2020 guidelines. Studies evaluating composite outcomes or TO in ovarian cancer patients with peritone

Association of nurse-patient ratios with outcomes in patients on peritoneal dialysis.

Centre-related factors, in addition to patient-related factors, are known to influence the outcomes of patients on peritoneal dialysis (PD). It is unclear if the nursing staff-to-patient ratios at different PD centres affect peritonitis outcomes. Our study aimed to examine whether full-time equivalent (FTE) PD nurse-patient ratios in individual PD centres were associated with centre-level outcomes of patients on PD. We used a cross-sectional survey of nursing leads of PD units across Australia and multi-centre registry data linkage. Of the 38 PD units in Australia, 28 (73.7%) nursing leads res…

Antisenescent and antiinflammatory effect of N-acetylcysteine in peritoneal mesothelial cells.

BackgroundPeritoneal dialysis induces an intraperitoneal inflammatory reaction, which causes damage to the peritoneum. Inflammation also accelerates cellular senescence. We studied in vitro effect of the dialysates from peritoneal dialysis patients on the senescence of the peritoneal mesothelial cells (MCs). The effect of N-acetylcysteine (NAC) on that process was studied.MethodsReplicative senescence was induced in MC cells exposed to culture medium, medium mixed with the dialysate ± NAC 0.025 mmol/L. After 10 passages, markers of the cellular senescence and secretory activity of the cells we

Peritoneal Dialysis Outcomes in Autosomal Dominant Polycystic Kidney Disease.

KEY POINTS: Peritoneal dialysis can be used successfully in patients with autosomal dominant polycystic kidney disease. Compared with days receiving hemodialysis, peritoneal dialysis days were associated with better patient survival, which has increased over time. Compared with hemodialysis days, peritoneal dialysis days were associated with a higher rate of kidney transplant, which has increased over time. BACKGROUND: Peritoneal dialysis (PD) is a form of home dialysis that is underutilized in the United States. Patients with autosomal dominant polycystic kidney disease (ADPKD) are frequentl

Human factors testing in a novel automated peritoneal dialysis system.

BackgroundBecause automated peritoneal dialysis (APD) systems are used in the home and operated by patients or their care partners, the human-device interface must be thoughtfully designed to facilitate intuitive operation to reduce the risk of errors that could occur during their use. User-friendly design should also help reduce anxiety to peritoneal dialysis (PD) adoption by patients incident to end-stage kidney disease (ESKD). Usability issues remain a significant barrier to PD adoption and are an important contributor to premature death, serious injuries, and PD technique failure.MethodsA

Presumptive congenital peritoneal encapsulation in a young mixed-breed dog.

A 9-month-old female mixed-breed dog weighing 3.25 kg was referred for elective ovariohysterectomy. A previous attempt had been aborted when the abdominal organs appeared to be enclosed within an abnormal membranous structure. At surgery, the abdominal viscera visible within the operative field were covered by a thin, translucent membrane that could be bluntly dissected from the organ surfaces without hemorrhage. The excised membrane was submitted for histopathological analysis. Postoperative non-contrast computed tomography did not identify abnormal soft-tissue densities or displacement of ab

Comparison of seprafilm and pitavastatin treatments in an experimental model of peritoneal adhesion.

Peritoneal adhesions are common following abdominal surgery and lead to significant morbidity. Both pharmacological agents and barrier methods have been investigated for prevention, but with limited success. In this study, the efficacy of intraperitoneal pitavastatin was evaluated and compared with Seprafilm. Thirty-two female Wistar albino rats were randomly divided into four groups (n = 8): control, saline, pitavastatin (30 mg/kg intraperitoneal), and Seprafilm (30 × 20 mm). Macroscopic adhesions were graded using the Majuzi classification, and microscopic adhesions were assessed using the Z

Pathophysiology, prevention and treatment of postoperative peritoneal adhesions: a WSES narrative.

BACKGROUND: Adhesions are a common complication following surgery, and they can cause significant morbidity. However, adhesions are not easily diagnosed with imaging, and often only become apparent when they cause intestinal obstruction symptoms. The pathophysiology of adhesion formation is complex, thus despite advances in surgical techniques and postoperative care, adhesions remain a persistent problem in clinical practice. Despite evidence for efficacy of some strategies in reducing adhesion formation question remain regarding the indications and impact on clinically relevant outcomes. The

Localized Epithelioid Peritoneal Mesothelioma: A Rare Mimicker of Gynecological Malignancy.

Localized epithelioid peritoneal mesothelioma (LEPM) is a rare variant of peritoneal mesothelioma that presents as a solitary, circumscribed mass rather than with the diffuse serosal spread that characterizes the far more common diffuse form. Because it may arise in the pelvis and be accompanied by ascites and a mildly elevated carbohydrate antigen (CA)-125 level, it is readily mistaken for a gynecological or primary peritoneal malignancy. Although it is generally regarded as less aggressive than the diffuse form, its clinical behavior can be unpredictable. We report the case of a 76-year-old
