Celiac Disease: A Review from Genetic to Treatment.

Celiac disease (CD) is a complex disorder influenced by genetic and environmental factors. When people with a genetic predisposition to CD consume gluten, an inflammatory response is triggered in the small intestine, and this reaction can be alleviated by the elimination of gluten from the diet. The clinical manifestations of CD vary greatly from person to person and begin at a young age or in adulthood. Influence of genetic factors on CD development is evident in carriers of the DQ2 and/or DQ8 allele. HLA genotypes are associated with gut colonization by bacteria, particularly in individuals

Sexual precocity in the setting of parental use of a compounded testosterone cream: case report and review of the literature.

OBJECTIVES: Person-to-person transmission of transdermal testosterone - termed secondary exposure - is a rare but important cause of precocious puberty. CASE PRESENTATION: A 3.5-year-old male was evaluated for precocious puberty based on a 6 month history of penile growth, development of secondary sexual characteristics, and growth spurt. Total testosterone level was significantly elevated at 1,460 ng/dL with normal prepubertal gonadotropin levels. Further history revealed that the father had been using transdermal testosterone for about 1 year. Within 2 months of discontinuation, total testo

Mental health, availability to participate in social change, and social movement accessibility.

People with illnesses and disabilities routinely face obstacles to political participation, including participation in social movements. Conventional social movement studies primarily theorize impediments to social movement participation in terms of personal constraints, as implied by the term "biographical (un)availability." However, studies in disability, health, and illness resist locating disability-related constraints solely within the individual, pushing fields to ask how environments can be disabling in and of themselves. Thus, by extending social movement theory through this Disabled/C

Progressive encephalomyelitis with rigidity and myoclonus (PERM).

Progressive encephalomyelitis with rigidity and myoclonus (PERM) is a subtype of stiff-person syndrome (formerly stiff-man syndrome). It is rare and disabling, and characterised by brainstem symptoms, muscle stiffness, breathing issues and autonomic dysfunction. We describe a 65-year-old man who presented with odynophagia together with tongue and neck swelling, followed by multiple cranial nerve palsies culminating in bilateral vocal cord paralysis with acute stridor. He subsequently developed progressive generalised hypertonia and painful limb spasms. Serum antiglycine receptor antibody was s

Solutions for Shared Resource Lab Remote Quality Control and Instrument Troubleshooting during a Pandemic.

The COVID-19 pandemic has dramatically affected shared resource lab (SRL) staff in-person availability at institutions globally. This article discusses the challenges of ensuring reliable instrument performance and quality data output while facility staff and external service provider on-site presence is severely limited. Solutions revolve around the adoption of remote monitoring and troubleshooting platforms, provision of self-service troubleshooting resources specific to facility instruments and workflows, development of an assistance contact policy, and ensuring efficiency of limited in-per

Looking back, looking forward: Methodological challenges and future directions in research on persons with profound intellectual and multiple disabilities.

BACKGROUND: Within the context of the Special Interest Research Group (SIRG) on Persons with Profound Intellectual and Multiple Disabilities (PIMD), researchers often discuss the methodological problems and challenges they are confronted with. The aim of the current article was to give an overview of these challenges. METHODS: The challenges are centred on six topics. These reflect the main components of a study's design: (a) participant demarcation, (b) participant recruitment, (c) data collection and instruments, (d) data analysis, (e) ethics/including the "voice" of persons with PIMD and (

PDE based geometry model for BCG immunotherapy of bladder cancer.

BCG immunotherapy has shown significant success for bladder cancer treatment, but due to the complexity of the interaction between immunity and cancer, clinical outcomes vary significantly between patients. A possible approach to overcome this difficulty may be to develop new methodologies for personally predicting the results of therapy by integrating patient data with dynamic mathematical model. We present a model describing a BCG immunotherapy dynamic taking into consideration an approximation of the bladder's geometry using PDE. We show that the proposed model takes into account the initia

Cases in Precision Medicine: The Role of Polygenic Risk Scores in Breast Cancer Risk Assessment.

Polygenic risk scores (PRSs) have been consistently associated with elevated breast cancer risk in cohort studies and are associated with risk in both women with and those without a family history of breast cancer. However, before clinical implementation, several issues must be addressed, including understanding the potential clinical utility and optimal method to communicate personalized screening recommendations that incorporate the PRS. Several trials are under way to answer some of these questions and facilitate clinical implementation. Because these PRSs have been developed in women of Eu

Personalized Cardiovascular Regenerative Medicine: Targeting the Extreme Stages of Life.

Cardiovascular regenerative medicine is an exciting new approach that promises to change the current care of million people world-wide. Major emphasis was given to the quality and quantities of regenerative products, but recent evidence points to the importance of a better specification of the target population that may take advantage of these advanced medical treatments. Patient stratification is an important step in drug development. Tailoring treatment to the patient's specificity allowed significant improvement in cancer therapy, but personalized regenerative medicine is still at the initi

Social engagement during game play in persons with Alzheimer's: Innovative practice.

This study examined social engagement during game play among persons with Alzheimer's. In addition to being engaged with the game, engagement with players was noticed. Players would congratulate each other when they won, and they encouraged each other to do well. Natural conversation occurred as a result of playing the game. The researchers observed that as placement accuracy increased, the players developed a friendly competition. They played to win and experienced a sense of accomplishment when they correctly matched tile pieces. Interestingly, the game provided the players an opportunity to

Ratings of Broader Autism Phenotype and Personality Traits in Optimal Outcomes from Autism Spectrum Disorder.

The study examines whether "optimal outcome" (OO) children, despite no longer meeting diagnostic criteria for Autism Spectrum Disorder (ASD), exhibit personality traits often found in those with ASD. Nine zero acquaintance raters evaluated Broader Autism Phenotype (BAP) and Big Five personality traits of 22 OO individuals, 27 high functioning individuals with ASD (HFA), and 23 typically developing (TD) peers. HFA children displayed higher ratings than their peers on all BAP traits. OO were indistinguishable from TD, with the exception of greater extraversion (e.g., increased talkativeness), a

Aerosol transmission of infectious disease.

OBJECTIVE: The concept of aerosol transmission is developed to resolve limitations in conventional definitions of airborne and droplet transmission. METHODS: The method was literature review. RESULTS: An infectious aerosol is a collection of pathogen-laden particles in air. Aerosol particles may deposit onto or be inhaled by a susceptible person. Aerosol transmission is biologically plausible when infectious aerosols are generated by or from an infectious person, the pathogen remains viable in the environment for some period of time, and the target tissues in which the pathogen initiates inf

Progress in the development of poliovirus antiviral agents and their essential role in reducing risks that threaten eradication.

Chronic prolonged excretion of vaccine-derived polioviruses by immunodeficient persons (iVDPV) presents a personal risk of poliomyelitis to the patient as well as a programmatic risk of delayed global eradication. Poliovirus antiviral drugs offer the only mitigation of these risks. Antiviral agents may also have a potential role in the management of accidental exposures and in certain outbreak scenarios. Efforts to discover and develop poliovirus antiviral agents have been ongoing in earnest since the formation in 2007 of the Poliovirus Antivirals Initiative. The most advanced antiviral, pocap

Practical aspects of genetic counseling in breast cancer: lights and shadows.

In unselected populations, less than 10% of breast cancers are associated with germline mutations in predisposing genes. Breast cancer type 1 and 2 (BRCA1 and BRCA2) susceptibility genes are the most common involved genes and confer a 10-30 times higher risk of developing the disease compared to the general population. A personal or family history suggestive of inherited breast cancer syndrome may be further evaluated to assess the risk of genetic predisposition and the presence of a genetic mutation. Breast cancer genetic counseling should include a careful risk assessment with associated psy

[New insights into the pathogenesis of sensitive skin].

Many persons in the developed countries report sensitive skin. Persons with sensitive skin have a predisposition to several skin diseases, most importantly dry skin, but also atopy, seborrheic dermatitis, acne vulgaris, rosacea, and perioral dermatitis. Their complaints may be triggered by environmental factors such as low temperature, wind, high temperature, sun exposure and stress. Various skin care products and cosmetics are not tolerated. A disturbed skin barrier function and reduced stratum corneum water content are most important in the pathophysiology of sensitive skin. Environmental fa

A study to define: profound and multiple learning disabilities (PMLD).

This study aimed to define the term 'profound and multiple learning disabilities' (PMLD). A shared understanding of terminology or diagnostic terms describing groups of individuals is important for the purposes of strategic development, service planning, and the provision and equity of service delivery. A literature review provided different definitions and meanings associated with the term. The meaning attributed to the definitions was explored in focus groups and individual interviews (face to face and telephone) with service managers, commissioners, practitioners, frontline healthcare staff

What did I say? Versus what did I think? Attributing false beliefs to self amongst children with and without autism.

The task used most widely to assess recognition of false belief in self and others is the 'Smarties' unexpected contents task. Amongst individuals with and without autism, the Self and Other-person test questions of this task are of an equivalent level of difficulty. However, a potential confound with this task may allow the Self test question to be passed without false belief competence. Three groups of participants (with autism, developmental disability and typical development) undertook a new unexpected contents task which did not suffer from this confound. The main finding was that partici

[Combating the new influenza A (H1N1) virus. I. Overview of the relevant virological aspects].

In April 2009 a new influenza virus was discovered, which spread from Mexico to the rest of the world. The new influenza A (H1N1) virus is genetically related to swine flu viruses, and differs substantially from circulating human influenza viruses. It is able to spread from person to person. Because it is a completely new virus, there is probably little immunity in the population. The course of the infection is relatively mild, but the virus will mutate and it is not yet certain whether this will affect severity of the influenza. General practitioners have an important role in surveillance and

Invasive salmonellosis in Malawi.

The incidence of invasive salmonellosis has increased among children and HIV-infected adults in Malawi. This has been associated with the emergence of drug resistance in the non-typhoidal Salmonella serovars Enteritidis and Typhimurium. In contrast, S. Typhi isolates have remained fully sensitive to commonly used antibiotics and the estimated incidence of typhoid fever, although still present, has fallen slightly among both adults and children. Infection with S. Typhi is not closely associated with underlying immunosuppression but it is possible that the non-typhoidal Salmonellae have adapted

Measuring decision-making capacity in cognitively impaired individuals.

Cognitive and functional losses are only part of the spectrum of disability experienced by persons with Alzheimer's disease and related dementias. They also experience losses in the ability to make decisions, known as decision-making capacity. Researchers have made substantial progress in developing a model of capacity assessment that rests upon the concept of the 4 decision-making abilities: understanding, appreciation, choice and reasoning. Empirical research has increased our understanding of the effects of late-life cognitive impairment on a person's ability to make decisions. This review
