Trajectories of sleep disturbances in the subacute phase after stroke: a longitudinal actigraphy study.

BACKGROUND: Sleep disturbances are common after stroke and may negatively impact recovery and quality of life. However, monitoring sleep parameters over time with objective measures remains underreported. OBJECTIVE: To investigate longitudinal changes in actigraphy-derived sleep parameters from the subacute to later stages after stroke. METHODS: This longitudinal observational study included 18 individuals with stroke who completed seven-day wrist-actigraphy monitoring at baseline and at the most distal valid follow-up assessment obtained more than three months after stroke. The median inter

Association of tirzepatide with changes in OSA-related measures based on baseline characteristics - post hoc analyses of SURMOUNT-OSA.

PURPOSE: Obstructive sleep apnea (OSA) is a common disorder characterized by repetitive collapse of the upper airway during sleep. Given that excess adiposity is a known risk factor for OSA, we aimed to descriptively assess the association of tirzepatide, a GIP/GLP-1 receptor agonist, with changes in AHI, hypoxic burden, body weight, and blood pressure in different patient populations based on baseline characteristics such as age, sex, BMI, AHI, and neck circumference. METHODS: These post hoc analyses examined data from two Phase 3 randomized, double-blind studies evaluating maximum tolerated

Widespread decoupling of spindles and slow waves in temporal lobe epilepsy.

OBJECTIVE: Memory impairment is common in people with temporal lobe epilepsy (TLE). Recent studies in healthy subjects showed a positive correlation between sleep spindles coupled to slow waves (SWs) and memory performance. We aimed to determine differences in spindle-SW coupling in TLE patients compared to healthy controls using combined high-density electroencephalography and polysomnography. METHODS: The study population consisted of 20 patients (12 female, 36.5 ± 9.9 years old) with unilateral drug-resistant TLE (10 left temporal) and 20 age- and sex-matched controls (12 female, 31.2 ± 6.…

Diagnostic Performance of a Type III Portable Monitoring Device for Obstructive Sleep Apnea in Pregnant Women: A Prospective Validation Study.

Rationale: Obstructive sleep apnea (OSA) during pregnancy is linked to negative maternal and neonatal outcomes. Diagnosing OSA in this population is particularly challenging. Portable monitoring devices (PMD) present a potential alternative to polysomnography (PSG), but their effectiveness in pregnant women is uncertain. Objective: To evaluate diagnostic accuracy of a PMD to detect OSA in pregnant women. Methods: A prospective study of 136 middle-aged pregnant women (34 ± 4 years) in their third trimester of pregnancy (34 ± 3 weeks) who underwent laboratory type III PMD and PSG simultaneously.…

An EEG Screening Method for Severe Obstructive Sleep Apnea Based on Limited Penetrable Difference Visibility Graph and Graph Convolutional Network.

Obstructive Sleep Apnea (OSA) is a common respiratory disease characterized by recurrent airway block during sleep, which does great harm to the human body. Utilizing the electroencephalogram (EEG) has been proven instrumental in OSA detection, as sleep apnea occurrences induce discernible alterations in EEG patterns. In this study, we propose a Limited Penetrable Difference Visibility Graph (LPDVG) method to screen severe OSA. This method exhibits strong anti-noise performance, effective information extraction, and a certain degree of generalization ability. First of all, this study construct

A Comparison of Snoring Changes With a Supine-Avoidance Alarm Device Compared to Constant Positive Airway Pressure Treatment in Patients With Supine-Predominant OSA.

This study aimed to quantify how much snoring occurs in patients with supine-predominant OSA and the comparative effectiveness of supine-avoidance therapy versus CPAP to reduce objective measures of snoring. Participants had a 1-week in-home sleep posture assessment and a in-home PSG study before being randomised to either CPAP or supine-avoidance therapy for 6-8 weeks, then switched treatments for another 6-8 weeks. Snoring and treatment outcomes were examined in a subgroup of patients with supine dependent snoring. Baseline measurements showed snoring frequency was 48.9 [95% CI 16.7 to 188.7

Positional therapy for the treatment of positional obstructive sleep apnea in children: A randomized controlled crossover trial.

BACKGROUND: Obstructive sleep apnea (OSA) is prevalent in children, but many children remain untreated. Up to one half of children with OSA have positional OSA, a phenotype characterized by the predominance of airway obstruction while supine. Positional devices that prevent sleeping supine may be beneficial for positional OSA. We evaluated the efficacy of positional devices for treating positional OSA in children. STUDY DESIGN AND METHODS: This was a randomized crossover trial of children aged 4-18 years old with positional OSA. There were two treatment periods separated by 1-4 weeks. Each pe

Endogenous Circadian System Attenuates Nighttime Vascular Endothelial Function in People With Untreated Obstructive Sleep Apnea.

BACKGROUND: In people with obstructive sleep apnea (OSA), adverse cardiovascular events, such as myocardial infarction, occur more commonly in the middle of the night than at other times of the day. These events could occur as a result of the nighttime physiological changes in OSA, but the endogenous circadian system has been implicated in increased cardiovascular risk in OSA. Vascular endothelial function is a marker of prospective cardiovascular risk and is impaired in OSA. We hypothesized that the nighttime risk in OSA could be due to a circadian system-driven nighttime impairment in vascul

Is a change in mouth opening associated with improvements in quality of life in children with type 1 obstructive sleep apnoea after adenotonsillectomy? Protocol for the JawChild prospective cohort study.

INTRODUCTION: Adenotonsillectomy is the primary treatment for type 1 obstructive sleep apnoea (OSA1). Although polysomnography (PSG) remains the gold standard for measuring Apnoea-Hypopnoea Index, it is a labour-intensive procedure and does not correlate with improvements in quality of life postadenotonsillectomy. Mouth breathing is associated with poorer quality of life in children. Mandibular movement (MM), which measures mouth opening, is a validated measure of respiratory effort that can be easily and safely assessed in children using the JAWAC technology. This study aims to evaluate the r…
