Analysis of the anti-fibrotic potential of a JAK inhibitor in a bleomycin-induced pulmonary fibrosis model.

BACKGROUND: Interstitial lung disease (ILD) associated with connective tissue diseases (CTD-ILD) remains to be a major cause of mortality. Different from idiopathic form, CTD-ILD involves more immune dysregulation along with aberrant fibroblast activation. Therefore, therapy targeting both profibrotic and proinflammatory molecules could be ideal for CTD-ILD. Janus kinase (JAK) is a family of intracellular, non-receptor tyrosine kinases that transduce cytokine-mediated signals. The purpose of this study is to reveal the anti-fibrotic potential of JAK inhibitors (JAKis). METHODS: The anti-fibro

Premature Atrial Contractions as a Marker of Atrial Cardiopathy: A Revised Analysis of the ARCADIA Randomized Trial.

INTRODUCTION: Atrial cardiopathy may be associated with an increased risk of stroke independent of atrial fibrillation (AF). In the ARCADIA trial, apixaban was not superior to aspirin in preventing recurrent stroke among patients with a cryptogenic stroke and atrial cardiopathy. We aimed to determine whether the presence of at least one premature atrial complex (PAC), a known harbinger of AF and stroke, would enhance the ability to identify individuals most likely to benefit from apixaban. METHODS: In ARCADIA, atrial cardiopathy was defined by NT-proBNP > 250 pg/mL, a P-wave terminal force gr

Drug retention rate of Janus kinase inhibitors in patients with rheumatoid arthritis: the KEIO-RA cohort study.

This study aimed to investigate the effectiveness and safety of Janus kinase inhibitors (JAKis) in patients with rheumatoid arthritis (RA) in the real world. All consecutive patients with RA from the KEIO-RA cohort who had received a JAKi from July 2013 to June 2023 were included. We assessed drug retention rates, factors associated with retention, disease activity, and adverse events. In total, 294 treatment courses in 201 patients were analyzed. The mean drug continuation duration was 17.2 ± 17.4 months. Full-dose JAKi use and concomitant methotrexate (MTX) use were associated with higher re

Case report: Toxic epidermal necrolysis as a unique presentation of acute graft versus host disease in a pediatric patient.

INTRODUCTION: Acute graft versus host disease (aGVHD) is a common complication of stem cell transplant (SCT), with skin involvement being most common. Severe presentations of skin aGVHD involving rapid progression of rash to bullae formation and mucosal involvement are rare. There are reports of patients with skin aGVHD that present with clinical characteristics mimicking toxic epidermal necrolysis (TEN), suggesting a possible overlap between the two. Management and outcomes of pediatric patients with this overlapping, severe presentation have rarely been described. CASE PRESENTATION: This re

Oral and Topical Janus Kinase Inhibitors in Patients With Cutaneous T-Cell Lymphoma: A Real-World Single-Center Experience.

The JAK/STAT pathway is implicated in the pathogenesis of cutaneous T-cell lymphoma (CTCL), with early studies suggesting that JAK inhibitors (JAKi) may be used for treating CTCL. Patients with CTCL may have other indications for JAKi use, but data on JAKi agents in CTCL is limited due to warnings against their use in malignancy. We retrospectively characterized the experience of our tertiary cancer center with JAKi in patients seen for CTCL between the years 2011 and 2024. We identified 26 CTCL patients who received a JAKi: 7 received a JAKi prior to their CTCL diagnosis (topical (n = 5) or o

Lorlatinib and neurocognitive adverse events: characterization, risk factors, and the potentially mitigative role of social support.

PURPOSE: Lorlatinib is the preferred first-line treatment for patients with ALK+ metastatic non-small cell lung cancer. However, this drug carries high rates of neurocognitive adverse events. This study further characterized these neurocognitive adverse events, examined risk factors, and, for the first time, explored social support as a mitigation strategy. METHODS: The Mayo Clinic medical record was interrogated for patients prescribed lorlatinib between July 2017 and May 2022, and medical records were reviewed in detail. RESULTS: Sixty-three patients were prescribed lorlatinib (median age:

Gastrointestinal Burden in Patients With Pancreatic Insufficient Cystic Fibrosis Before and After Elexacaftor/Tezacaftor/Ivacaftor Use.

BACKGROUND: The advent of highly effective modulator therapies (HEMT), namely elexacaftor, tezacaftor, and ivacaftor (ETI), has resulted in substantial improvements in lung function, growth, and quality of life for people with cystic fibrosis (PwCF). However, the understanding of the impact of ETI on gastrointestinal (GI) disease burden is evolving. This study aims to describe and compare the prevalence of GI manifestations, prescribed GI medications, and GI procedures between 2 time periods, pre-ETI and post-ETI approval, in children with CF (CwCF) and adolescents with CF. METHODS: This is a

Pronounced benefits of JAK inhibition with baricitinib in COVID-19 pneumonia in obese but not lean subjects.

OBJECTIVE: Obesity and age are strongly linked to severe COVID-19 pneumonia where immunomodulatory agents including Janus kinase inhibitors have shown benefits but the efficacy of such therapy in viral pneumonia is not well understood. We evaluated the impact of obesity and age on survival following baricitinib therapy for severe COVID-19. METHODS: A post hoc analysis of the COV-BARRIER multicentre double-blind randomised study of baricitinib versus placebo (PBO) with an assessment of 28-day mortality was performed. All-cause mortality by day 28 was evaluated in a Cox regression analysis (adj

Clinical outcomes of immunomodulation therapy in immunocompromised patients with severe Covid-19 and high oxygen requirement.

Covid-19 disease is implicated in increased mortality among immunocompromised patients. The JAK inhibitor, baricitinib (bar), or the IL-6 inhibitor, tocilizumab (toc), demonstrated a survival benefit in patients with severe disease. However, evidence supporting their use in immunocompromised patients with severe Covid-19 is scarce. We aimed to assess clinical outcomes of bar/toc treatment in immunocompromised patients. A multi-center registry of consecutive immunocompromised patients hospitalized due to severe Covid-19 during the Omicron variant dominance period. After excluding patients who did

Synergistic effect of venetoclax and ibrutinib on ibrutinib-resistant ABC-type DLBCL cells.

Despite the widespread use of R-CHOP therapy in diffuse large B-cell lymphoma (DLBCL), the therapeutic efficacy for this disease remains suboptimal, primarily due to the heterogeneity of refractory and/or relapsed diseases. To address this challenge, optimization of DLBCL treatment regimens has focused on the strategy of combining an additional drug "X" with R-CHOP to enhance efficacy. However, the failure of R-CHOP combined with the BTK inhibitor ibrutinib in treating ABC-type DLBCL patients has raised significant concerns regarding ibrutinib resistance. While some studies suggest that veneto

Outcomes of chronic myeloid leukemia patients after therapeutic failure to conventional tyrosine kinase inhibitors and asciminib.

Patients with chronic myeloid leukemia (CML) who have failed conventional tyrosine kinase inhibitors (cTKIs) and asciminib constitute a complex group of patients with few therapeutic options. A retrospective descriptive multicenter observational study was carried out including patients with CML who had presented a therapeutic failure to ≥ 2 cTKIs and asciminib, and had received or were not candidates to ponatinib. Of the 19 patients enrolled, 8 patients failed asciminib due to intolerance and 11 due to resistance. The most common strategy for intolerant patients was to initiate a previously ad

Rare case of simultaneous occurrence of chronic neutrophil leukemia and T lymphoblastic lymphoma: case report and literature review.

Chronic neutrophil leukemia (CNL) is a rare and life-threatening disease. Cases of CNL combined with lymphoma are rare. Here, we report a case of CNL with T-acute lymphoblastic leukemia/lymphoma (T-ALL/LBL) in a 28-year-old male. After a regimen of ruxolitinib, VICLP (Vincristine, Idarubicin, Cyclophosphamide, Prednisone, Peg-asparaginase) regimen, high-dose cytarabine, and methotrexate regimens, the patient's bone marrow condition partially resolved. However, when the disease relapsed four months later, despite attempts with selinexor, venetoclax, and CAG(aclarubicin hydrochloride, Algocytidi

Prenatal MAM exposure raises kynurenic acid levels in the prefrontal cortex of adult rats.

BACKGROUND: Elevated brain levels of kynurenic acid (KYNA), a metabolite in the kynurenine pathway, are associated with cognitive dysfunctions, which are nowadays often considered as fundamental characteristics of several psychopathologies; however, the role of KYNA in mental illnesses, such as schizophrenia, is not fully elucidated. This study aimed to assess KYNA levels in the prefrontal cortex (PFC) of rats prenatally treated with methylazoxymethanol (MAM) acetate, i.e., a well-validated neurodevelopmental animal model of schizophrenia. The effects of an early pharmacological modulation of

Metronomic dosing of ovarian cancer cells with the ATR inhibitor AZD6738 leads to loss of CDC25A expression and resistance to ATRi treatment.

OBJECTIVE: ATR kinase inhibitors promote cell killing by inducing replication stress and through potentiation of genotoxic agents in gynecologic cancer cells. To explore mechanisms of acquired resistance to ATRi in ovarian cancer, we characterized ATRi-resistant ovarian cancer cells generated by metronomic dosing with the clinical ATR inhibitor AZD6738. METHODS: ATRi-resistant ovarian cancer cells (OVCAR3 and OV90) were generated by dosing with AZD6738 and assessed for sensitivity to Chk1i (LY2603618), PARPi (Olaparib) and combination with cisplatin or a CDK4/6 inhibitor (Palbociclib).
