The number of patients hospitalized for acute infection in the frontal sinuses at the Department of Oto-Rhino-Laryngology of Turku University Hospital has increased markedly during the last decade. Causes for this increase were evaluated by comparing the backgrounds and medical findings of the 134 patients treated in the years 1977-81 and those of the 421 patients treated in the years 1982-86. During the period between June 1987 and March 1988 the authors performed 10 dacryocystorhinostomies with temporary intubation of the new efferent pathways, using a Hatt set. We have followed 12 patients with acute symptoms on exposure to psyllium over a three-year period. All were hospital personnel with evidence of atopy. Exposure was by inhalation on dispensing psyllium powder. Symptoms which occurred within minutes of exposure included rhinitis (12/12), conjunctivitis (10/12), shortness of breath and wheezing (9/12). 121 habitual nose-bleeders were investigated. In all but one, local changes were found in the nasal mucosa. Abnormal vessels (varicose vessels, telangioma, or a network of small vessels) were found in 102 (84%), and 18 (15%) had mucosal atrophy (rhinitis anterior sicca or septal perforation) without abnormal vessels. Two patients who suffered from allergic rhinitis, conjunctivitis and contact urticaria caused by the two-spotted spider mite (Tetranychus urticae, Koch) are described. The response of the nasal mucous membrane to anti-human IgE serum was examined. The application of anti-IgE sera to the nasal mucous membrane induced a nasal allergy-like symptom as well as nasal eosinophilia.
