Equity-focused patient safety: reframing cultural competence, cultural humility and structural competence as determinants of patient safety.

Cultural competence and cultural humility have previously been framed as concepts reflecting the knowledge, skills and commitment to self-reflection concerning person-centred healthcare. Emerging evidence has linked cultural misunderstanding, language barriers and implicit bias with diagnostic error, medication mismanagement and inequitable health outcomes for patients. Beyond this, the concept of structural competence has acknowledged how organisational, institutional, social and political structures can shape care and impact patient safety. There has yet to be appreciation of these terms tog

[Traumatic Experiences and Posttraumatic Stress Disorder of Adolescents in Boarding Schools].

In the context of abuse cases in boarding schools, stress among children and adolescents in boarding schools in Germany was discussed for the first time. So far, however, there is a lack of studies dealing with post-traumatic stress disorders of adolescents in boarding schools. The aim was to analyze the extent of trauma experiences and symptoms of post-traumatic stress disorders as well as psychopathological abnormalities in adolescents in boarding schools on the basis of self-reports using a secondary analysis of theMAYSI-2 and UCLA PTSD Reaction Index for DSM-IV screening questionnaires and

Implant or Tooth?-A Prospective Clinical Study on Oral Health-Related Quality of Life for Patients with "Unrestorable" Teeth.

In cases of severely compromised teeth, dental practitioners are confronted with the therapeutic decision of whether to restore a tooth or replace it with an implant. Comparative scientific evidence on patient perception of both treatment approaches is scarce. The subject of this prospective clinical study was to compare oral health-related quality of life (OHRQoL) between two treatment groups: restoration of severely destroyed teeth after orthodontic extrusion (FOE) versus tooth extraction and implant-supported single crown restoration (ISC). A self-selected trial was performed with 21 patien

Transferring more than one embryo simultaneously is justifiable in most patients.

Elective single embryo transfer (eSET) was first introduced to IVF in 1999, and its subsequent integration into mainstream reproductive endocrinology and infertility has been hugely consequential. It can be viewed as the first (among many since) 'add-ons' to IVF that has significantly and adversely affected how IVF is practised, resulting in astonishing declines in live birth rates after fresh non-donor IVF cycles worldwide. We propose that, like most 'add-ons' to IVF over recent years, the almost universal use of eSET worldwide lacks proper validation of its underlying hypothesis and is based

Strategies to Improve Bone Healing: Innovative Surgical Implants Meet Nano-/Micro-Topography of Bone Scaffolds.

Successful fracture healing is dependent on an optimal mechanical and biological environment at the fracture site. Disturbances in fracture healing (non-union) or even critical size bone defects, where void volume is larger than the self-healing capacity of bone tissue, are great challenges for orthopedic surgeons. To address these challenges, new surgical implant concepts have been recently developed to optimize mechanical conditions. First, this review article discusses the mechanical environment on bone and fracture healing. In this context, a new implant concept, variable fixation technolo

Survey to explore understanding of the principles of aseptic technique: Qualitative content analysis with descriptive analysis of confidence and training.

BACKGROUND: In many countries, aseptic procedures are undertaken by nurses in the general ward setting, but variation in practice has been reported, and evidence indicates that the principles underpinning aseptic technique are not well understood. METHODS: A survey was conducted, employing a brief, purpose-designed, self-reported questionnaire. RESULTS: The response rate was 72%. Of those responding, 65% of nurses described aseptic technique in terms of the procedure used to undertake it, and 46% understood the principles of asepsis. The related concepts of cleanliness and sterilization were

Participant experiences and perceptions of physical activity-enhancing interventions for people with physical impairments and mobility limitations: a meta-synthesis of qualitative research evidence.

Disabled people face multiple personal, environmental and social barriers that interfere with leading a physically active lifestyle. Thus, there is an urgent need for behaviour change interventions to increase physical activity (PA) by specifically addressing the situations of disabled people, and barriers to participation. This original meta-synthesis of qualitative research was undertaken to explore participants' experiences and perceptions of PA-enhancing interventions for adults with physical impairments resulting in mobility limitations. Published articles were identified through a rigoro

A study of environmental influences on the well-being of individuals with psychiatric disabilities in Philadelphia, PA.

The influence of community-level indicators in understanding mental health has been of interest since the seminal work of Faris and Dunham (1939). The emergence of Geographic Information Systems (GIS), and concepts such as social capital and recovery add new tools and concepts for further explorations in this area. This correlational study uses an innovative GIS approach to examine the relative contribution of community indicators, such as crime, unemployment, social capital, and individual-level sociodemographic characteristics, such as gender, age, and experience of homelessness, to the well

[Asomatognosia and oral drive: a psychoanalytical perspective].

OBJECTIVE: The psychoanalytic concept of specular image refers to the complex construction that associates the body image with the language coordinates of the individual, thus making him/her a human subject. The acquisition of this specular image implies the loss of corporeal exchanges between mother and child, i.e., the "neutralization" of those body parts or extensions where these exchanges take place. These conceptions of body image and subjectivity lead to the hypothesis that neurological disturbances of body schema may alter the patients' subjectivity and their relation to the lost "objec

Surrender, disidentification, and reappraisal: a qualitative comparison of psilocybin responders and non-responders in treatment-resistant OCD.

INTRODUCTION: Psilocybin treatment has shown promise for treatment-resistant obsessive-compulsive disorder (OCD), yet clinical response is variable. Little is known about the subjective experiences associated with divergent outcomes or the psychological processes differentiating responders from non-responders. MATERIALS AND METHODS: A qualitative case comparison was conducted using semi-structured interviews from six participants enrolled in a randomized controlled trial of single-dose psilocybin with psychological support for treatment-resistant OCD. Participants were purposively selected to

Awareness of antimicrobial resistance and antibiotic use among poultry farmers in Accra, Ghana: A cross-sectional survey.

BACKGROUND: Antimicrobial resistance (AMR) in livestock production is an escalating global public health threat, fuelled in part by the misuse of antibiotics in animal farming. This study aimed to assess the knowledge, attitudes, and practices regarding antimicrobial use and AMR among poultry farmers in Accra, Ghana. METHODS: A cross-sectional survey of 400 poultry farmers in five communities across the Greater Accra Region was conducted using a structured questionnaire. The survey collected data on demographics, AMR awareness, antibiotic usage practices (treatment, prophylaxis, or growth pro

A simulation-based framework for tracking decision-making development in youth basketball: operationalising Vygotsky's zone of proximal development through Bayesian multilevel modelling and verbal protocol analysis.

INTRODUCTION: Sports coaching research remains polarised between direct instruction approaches emphasising feedback and repetition vs. constraints-led approaches favouring athlete discovery through task design. This debate persists partly because existing methods cannot distinguish whether athletes are genuinely internalising tactical understanding or merely following external instructions. We present a methodological simulation study proposing a framework that operationalises Cultural-Historical Activity Theory-particularly Vygotsky's Zone of Proximal Development (ZPD)-through Bayesian multil

An individually adjusted approach for communicating epidemiological results on health and lifestyle to patients.

If scientific research on modifiable risk factors was more accessible to the general population there is a potential to prevent disease and promote health. Mobile applications can automatically combine individual characteristics and statistical models of health to present scientific information as individually tailored visuals, and thus there is untapped potential in incorporating scientific research into apps aimed at promoting healthier lifestyles. As a proof-of-concept, we develop a statistical model of the relationship between Self-rated-health (SRH) and lifestyle-related factors, and a si

Neuropsychological Profiles of Athletes and Views of Parents Choosing Flag Versus Tackle Football Participation.

BACKGROUND: Previous studies have found that injury rates are slightly higher in children who play flag football versus tackle football. It is unclear if this difference is due to the way each type is played or taught or whether there are intrinsic differences in attitudes or neuropsychological characteristics in children and their parents. PURPOSE: To determine whether children who play flag football score differently from those who play tackle football on validated neuropsychological tests. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: Each participating athlete (age

[Asymmetries in dynamic plantar pressure distribution measurement in able-bodied gait: application to the study of the gait asymmetries in children with hemiplegic cerebral palsy].

OBJECTIVES: The aim of this study was to analyse, firstly, the plantar pressure distribution in healthy subjects in order to validate or invalidate the previous studies results on the asymmetrical profile of the stance phase. The studies of asymmetries was based on the identification of a propulsive foot and a loading foot from a concept introduced by Viel. Secondly, the approach was applied to the study of gait asymmetries in two children with hemiplegic cerebral plasy. MATERIAL AND METHOD: Thirty healthy control subjects and two hemiplegic children (H1 and H2) performed a walking test at se

Embodied Neuropsychodynamics of the Relational Self Across Space and Time: An Integrative Narrative Review.

Extensive explorations in neuroscience, psychology, and psychotherapy increasingly recognized the embodied and relational foundations of selfhood, underscoring the need for an integrated framework spanning development, psychopathology, and therapeutic change. This narrative review synthesizes empirical and theoretical literature across neuroscience, embodiment research, predictive processing, developmental science, phenomenology, and psychodynamic theory, proposing a multidimensional neuropsychodynamic framework of embodied selfhood and its clinical implications. A central contribution is the

Assessing the relationship between community resilience and health outcomes: an observational local-authority level study in England.

BACKGROUND: Community resilience is a relevant concept in public health, but its empirical relationship with health outcomes remains underexplored. This study examines whether a Community Resilience Index (CRI) is associated with population health outcomes in England, and whether it offers explanatory added value beyond the Index of Multiple Deprivation (IMD). METHODS: The CRI comprises 44 indicators reflecting community-level resilience to chronic stressors. Associations between CRI scores and five health outcomes, deaths of despair, cardiovascular disease (CVD) mortality, COVID-19 mortality

Fab-Fc and Fab-Fab interactions of variable strength and valency contribute to the high concentration viscosity of IgG1 antibodies.

The variable domains in Fab regions are well-established contributors to high concentration viscosity of IgG, primarily through charge and hydrophobic interactions. In contrast, the roles of the Fc and the number of self-interacting sites (valency) are less well understood. Here, we investigate the relative contributions of Fab-Fab and Fab-Fc interactions to high concentration viscosity for a diverse panel of 20 IgG1 antibodies, by rheometry, coarse-grained simulations, and molecular surface property analysis. Strikingly, fragmentation of IgG1 into F(ab')2 plus Fc reduced viscosity (-11 to -93

Self-Supervised Joint Reconstruction and Denoising of T2-Weighted PROPELLER MRI of the Lung at 0.55T.

PURPOSE: To improve 0.55T T2-weighted PROPELLER lung MRI by developing a self-supervised framework for joint reconstruction and denoising. METHODS: T2-weighted 0.55T lung MRI datasets from 44 patients with prior COVID-19 infection were used. Each PROPELLER blade was split along the readout direction into two disjoint subsets: one subset for training an unrolled network, and the other for loss calculation. Following the Noise2Noise paradigm, this framework split k-space into two subsets with independent, matched noise but identical underlying signal, enabling joint reconstruction and denoising
