Paediatric obstructive sleep apnoea: is a polysomnogram always necessary?

Obstructive sleep apnoea (OSA) is a common entity in children, most present with sleep disturbances such as snoring, choking during sleep, enuresis, restless sleep, or apnoeic spells. Other symptoms include poor school performance, hyperactivity, failure to thrive, heart failure and cor pulmonale. Most authors would concur that the polysomnogram (PSG) is the gold standard for the diagnosis of OSA, and that adenotonsillectomy is the surgical procedure of choice, with high curative rates and relatively low morbidity. Close post-operative monitoring of all children with OSA cannot be over-emphasi…

Pharmacotherapies for obstructive sleep apnoea: where are we now?

Obstructive sleep apnoea (OSA) is common, causes considerable morbidity and probably contributes to mortality particularly through associated cardiovascular disease. The physical therapy of continuous positive airway pressure (CPAP) is extremely effective in the majority of patients but most patients would prefer an alternative. Intuitively, OSA should be amenable to pharmacotherapy. The upper airway of affected individuals can be narrowed but is patent during wakefulness. Collapse of the airway during sleep occurs when negative intra-luminal pressure generated by inspiratory effort exceeds th…

Obstructive sleep apnea associated with cerebral hypoxemia and death.

An increase in the arousal threshold may predispose critically ill patients with obstructive sleep apnea (OSA) to prolonged apneas and death during sleep. We report two cases in whom polysomnographically documented OSA resulted in EEG changes compatible with cerebral hypoxemia with subsequent respective transient encephalopathy in one instance and death in the other.

Sleep disturbances in elderly: a subjective evaluation over 65.

Sleep in elderly people shows progressive changes caused by general aging processes. Several alterations are described in medical literature: changes of sleep/wake rhythm and modifications both in sleep duration and in sleep architecture. The aim of our study was to evaluate sleep disturbances in elderly people, with and without cognitive impairment, through a sleep questionnaire. Our population included 1000 subjects, over 65 years of age, stratified by sex and age. The first 600 interviews were included in this report. All patients underwent a mini mental state examination (MMSE) and a questi…

Sleep and gastroesophageal reflux disease: a wake-up call.

The medical community has mostly ignored the relationship between sleep and gastroesophageal reflux (GER), although recent studies have begun to explore the connection and suggest possible treatments. These studies have found that the relationship is reciprocal; reflux influences sleep and sleep influences reflux. It has been found that nighttime GER can result in a number of complications, and more generally, lower the quality of life for its sufferers, establishing sleep-related GER as a more intolerable form of GER. Though some lifestyle modifications may help reduce the risk of nighttime r…

[Sleep apnea syndrome in patients with chronic heart failure].

Sleep apnea syndrome (SAS) in patients with chronic heart failure (CHF) increases the risk of death. SAS was divided into 4 types: obstructive sleep apnea-hypopnea syndrome (OSAHS), upper airways resistance syndrome (UARS), central sleep apnea syndrome (CSAS), and sleep hypoventilation syndrome (SHVS). CSAS is caused by temporary cessation of central drive to respiratory muscles, OSAHS results from partial or complete collapse of the pharynx, UARS have typical symptoms of OSAHS and no changes on polysomnography, whereas SHVS results from pathological PCO2 increase with subsequent hypoxemia. In…

[Cardiogenic oscillation--utility of classifying types of sleep apnea].

Cardiogenic pulse waves that travel along the airway wall are captured as airflow variations synchronized with the electrocardiogram at the airway exit. We have used airflow variations caused by cardiogenic pulse waves (hereinafter referred to as cardiogenic oscillation) to classify types of sleep apnea. Pulse waves do not travel downstream if there is an airway obstruction or choke point (wave speed theory). Thus, cardiogenic oscillation was detectable in central apnea or hypopnea, but not in obstructive apnea. In mixed-type sleep apnea, cardiogenic oscillation disappeared, and thus airway ob…

Seizure associated with sleep deprivation and sustained-release bupropion.

This case report describes a generalized seizure associated with sustained-release bupropion use and sleep deprivation. The subject, a 31-year-old female smoker, was participating in a clinical trial evaluating an investigational medication for smoking cessation that used sustained-release bupropion as an active control. After 5 weeks of bupropion use, the subject experienced a generalized tonic clonic seizure after staying up nearly all night packing and moving to a new residence. The patient had no other risk factors for seizures. We suggest that sleep deprivation may add to the risk of bupr…

Sleep, sleepiness and school start times: a preliminary study.

BACKGROUND: High school students are reported to be excessively sleepy, resulting in decreased academic performance, increased psycho-social problems and increased risk of morbidity and mortality from accidents. Early school start times have been noted to contribute to this problem. This report attempts to confirm the relationship of early school start times with decreased sleep and increased sleepiness. METHODS: We examined sophomore and junior students in 2 local high schools with different start times and measured the amount of time slept and sleepiness. RESULTS: We found that students at…

Sleep-disordered breathing associated with long-term opioid therapy.

Three patients are described who illustrate distinctive patterns of sleep-disordered breathing that we have observed in patients who are receiving long-term, sustained-release opioid medications. Polysomnography shows respiratory disturbances occur predominantly during non-rapid eye movement (NREM) sleep and are characterized by ataxic breathing, central apneas, sustained hypoxemia, and unusually prolonged obstructive "hypopneas" secondary to delayed arousal responses. In contrast to what is usually observed in subjects with obstructive sleep apnea (OSA), oxygen desaturation is more severe and…

Clinical, anatomical, and physiologic relationship between sleep and headache.

The intimate relationship between sleep and headache has been recognized for centuries, yet the relationship remains clinically and nosologically complex. Headaches associated with nocturnal sleep have often been perceived as either the cause or result of disrupted sleep. An understanding of the anatomy and physiology of both conditions allows for a clearer understanding of this complex relationship and a more rational clinical and therapeutic approach. Recent biochemical and functional imaging studies in patients with primary headache disorders has lead to the identification of potential cent…

Sleep-disordered breathing in heart failure: characteristics and implications.

Sleep-disordered breathing, namely obstructive sleep apnea (OSA) and central sleep apnea (CSA), are both often encountered in the setting of heart failure (HF), and have distinct differences in terms of prevalence, pathophysiology and consequences. OSA is independently associated with an increased risk for cardiovascular disease and for congestive HF in the general population. It is conceivable that this breathing disorder may have particularly deleterious effects in patients with coexisting heart disease, especially in those with a failing heart. There are considerable data addressing the int…

Sleep disordered breathing and nocturnal polyuria: nocturia and enuresis.

Although nocturnal voiding is frequently attributed to urologic disorders, nocturia and enuresis are also important symptoms of sleep-disordered breathing. However, polyuria can be elicited by obstructive sleep apnea as well as bedrest, microgravity and other experimental conditions where the blood volume is shifted centrally to the upper body. The nocturnal polyuria of sleep apnea is an evoked response to conditions of negative intrathoracic pressure due to inspiratory effort posed against a closed airway. The mechanism for this natriuretic response is the release of atrial natriuretic peptid…

Treating pediatric patients with obstructive sleep disorders: an update.

Obstructive sleep apnea syndrome in children continues to be an important subject for otolaryngologists because of the high prevalence of the disease. The evaluation of a child with OSAS remains controversial, although there is little controversy that T&A is the optimal treatment for these children. The search for the optimal T&A technique is ongoing, although now either "cold" tonsillectomy or "hot" tonsillectomy is standard. Quality-of-life studies confirm the significant benefit gained after a child undergoes a T&A.

[Therapeutic results of respiratory disturbance during sleep in children].

We retrospectively analyzed 72 children suffering from respiratory disturbance during sleep at Mie University Hospital for sleep study or surgical treatment from 1992 to 2001. Their clinical symptoms included snoring (100% of 72 inpatients), sleep apnea (60%), mouth breathing (44%), and nasal obstruction (30%). Forty-seven reported sleep apnea and 28 (60% of total) corresponded to criteria for the sleep apnea syndrome (apnea index: AI > or = 5.0). These symptoms were caused by adenoid vegetation and/or hypertrophy of the palatine tonsils. No significant correlation was seen among size of the a…

[Cephalometric assessment in obstructive sleep apnea and hypopnea syndrome].

Several cephalometric studies have been published to study sleep apnea and hypopnea. The purpose of this review is to provide a clear analysis of the different and apparently contradictory cephalometric results and to describe certain pitfalls of the method. In general, an excessive development of the tongue and soft palate are observed in these subjects, limiting the pharyngeal airway. Bony anomalies include mandibular retrognathism and an excessive anterior ventral development of the skull associated with insufficient anterior growth of the base of the skull. The head is held in extension. T…

[Surgery and obstructive sleep apnea syndrome: indications and precautions].

Surgery has long been a therapeutic option for obstructive sleep apnea syndrome (OSAS). The first procedures targeted the soft palate but results of isolated uvulo-palatopharyngoplasty (UPPP) were inconstant. A better understanding of the mechanisms involved in pharyngeal collapse incited interest in the role of the tongue and the retropharyngeal space. Surgical procedures were then developed for this level of the upper airways. There are a large number of possibilities. Excepting tracheostomy which court-circuits the collapsed zone, all the other procedures are designed to widen more or less …
