A Contemporary Medicolegal Analysis of Injury Related to Peripheral Nerve Blocks.

BACKGROUND: Closed malpractice claims can provide insight into low-frequency adverse events in many areas of perioperative and chronic pain care. Over the last decade, there have been changes in surgical and regional anesthetic practice, likely impacting adverse event patterns. Given the wide variability and low frequency of complications associated with peripheral nerve blocks, the study of closed malpractice claims offers an opportunity to examine adverse events, and the patient, technical, and provider factors that led to the claim. Knowledge gained from examination of closed claims has alr

Diagnosis of Mild-to-moderate Idiopathic Median Neuropathy at the Carpal Tunnel Based on Signs and Symptoms is Discordant From Diagnosis Based on Electrodiagnostic Studies and Ultrasound.

BACKGROUND: Although idiopathic median neuropathy at the carpal tunnel (IMNCT) is objective and verifiable, distinction of normal and abnormal nerves is imprecise and probabilistic. The associated symptoms and signs (carpal tunnel syndrome [CTS]) vary, particularly for nonsevere (mild and moderate) median neuropathy. Discordance between diagnosis of mild or moderate median neuropathy at the carpal tunnel using symptoms and signs and diagnosis based on objective tests is a measure of the potential for overdiagnosis and overtreatment. QUESTION/PURPOSE: What is the difference in the estimated pr

Maternal hemodynamics, fetal biometry and Doppler indices in pregnancies followed up for suspected fetal growth restriction.

OBJECTIVES: To assess whether, in a cohort of patients with a small-for-gestational-age (SGA) fetus with estimated fetal weight ≤ 10th percentile, maternal hemodynamics, fetal biometry and Doppler indices at presentation can predict the subsequent development of an abnormal fetal Doppler index or delivery of a baby with birth weight < 3rd percentile. METHODS: This was a prospective observational cohort study conducted at a specialist clinic for the management of pregnancies with a SGA fetus at King's College Hospital, London, UK. The study population comprised 86 singleton pregnancies with a

Oral fumaric acid esters for psoriasis.

BACKGROUND: Psoriasis is a chronic inflammatory skin condition that can markedly reduce life quality. Several systemic therapies exist for moderate to severe psoriasis, including oral fumaric acid esters (FAE). These contain dimethyl fumarate (DMF), the main active ingredient, and monoethyl fumarate. FAE are licensed for psoriasis in Germany but used off-licence in many countries. OBJECTIVES: To assess the effects and safety of oral fumaric acid esters for psoriasis. SEARCH METHODS: We searched the following databases up to 7 May 2015: the Cochrane Skin Group Specialised Register, CENTRAL in

Deliberate hypotension with propofol under anaesthesia for functional endoscopic sinus surgery (FESS).

BACKGROUND: Functional endoscopic sinus surgery (FESS) is a minimally invasive technique that is used to treat chronic sinusitis. Small bleeding areas can reduce operative visibility and result in destruction of surrounding structures. Deliberate hypotension (lowering the mean arterial blood pressure to between 50 and 65 mm Hg in normotensive patients) using a range of pharmacological agents during general anaesthesia reduces blood loss in many operations. This review was originally published in 2013 and updated in February 2016. OBJECTIVES: We aimed to compare the use of propofol versus othe

Effectiveness and safety of intermittent preventive treatment with dihydroartemisinin-piperaquine or artesunate-amodiaquine for reducing malaria and related morbidities in schoolchildren in Tanzania: a randomised controlled trial.

BACKGROUND: In high transmission settings, most school-aged children harbour malaria parasites without showing symptoms, often leading to anaemia and possibly impaired psychomotor and cognitive abilities. We aimed to assess the effectiveness and safety of intermittent preventive treatment for malaria in school-aged children (IPTsc) living in highly endemic areas. METHODS: We did an open-label randomised controlled trial in seven primary schools in northeastern Tanzania. Schoolchildren aged 5-15 years were individually randomly assigned (1:1:1) to receive dihydroartemisinin-piperaquine, artesu

Cholinesterase inhibitors for dementia with Lewy bodies.

BACKGROUND: Dementia with Lewy bodies (DLB) was first described in 1983, and clinical diagnostic criteria were published in the early to mid 1990s. It has been suggested DLB may account for up to 15-25% of cases of dementia among people aged over 65, although autopsy suggests much lower rates. Characteristic symptoms are dementia, marked fluctuation of cognitive ability, early and persistent visual hallucinations and spontaneous motor features of Parkinsonism. Falls, syncope, transient disturbances of consciousness, neuroleptic sensitivity, and hallucinations in other modalities are also commo

Extended follow-up and spatial analysis of the American Cancer Society study linking particulate air pollution and mortality.

We conducted an extended follow-up and spatial analysis of the American Cancer Society (ACS) Cancer Prevention Study II (CPS-II) cohort in order to further examine associations between long-term exposure to particulate air pollution and mortality in large U.S. cities. The current study sought to clarify outstanding scientific issues that arose from our earlier HEI-sponsored Reanalysis of the original ACS study data (the Particle Epidemiology Reanalysis Project). Specifically, we examined (1) how ecologic covariates at the community and neighborhood levels might confound and modify the air poll

Racial and Ethnic Disparities in US Colorectal Cancer Mortality by County, Sex, and Age.

IMPORTANCE: In the past 2 decades, substantial declines in national colorectal cancer (CRC) mortality rates have been observed in the US; however, it is unclear whether these improvements have happened ubiquitously across geography, race and ethnicity, sex, and age group. OBJECTIVE: To assess variation in CRC mortality rates and changes over time, across county, race and ethnicity, sex, and age group (<45 years and ≥45 years). DESIGN, SETTING, AND PARTICIPANTS: This US population-based cross-sectional study used validated small-area estimation models to estimate county-level CRC mortality ra

Characterization of Models for Identifying Physical and Cognitive Frailty in Older Adults With Diabetes: Systematic Review and Meta-Analysis.

BACKGROUND: Physical frailty and cognitive frailty are increasingly recognized as critical geriatric syndromes among older adults with diabetes, contributing to adverse outcomes such as disability, hospitalization, and mortality. Early identification of individuals at high risk is therefore essential for timely prevention and intervention. Although a growing number of prediction models have been developed for this population, evidence regarding their methodological rigor, predictive performance, and generalizability remains fragmented. OBJECTIVE: This study aims to evaluate and characterize e

Development and Temporal External Validation of a Parsimonious, Interpretable Machine Learning Model for Predicting 6-Month Mortality in Long-Term Care Facilities: A Retrospective Cohort Study.

BACKGROUND: Early mortality after long-term care facility (LTCF) admission is common; yet, prognostic tools are often derived from Western minimum dataset-based cohorts or require hospital electronic health record linkages that are unavailable at intake in many LTCFs. There is also limited evidence on explainable, admission-feasible machine learning-based prognostication in Asian LTCF settings. OBJECTIVE: The aim of the study is to develop and temporally externally validate an interpretable machine learning model for predicting 6-month all-cause mortality among older adults newly admitted to

Transcutaneous spinal cord stimulation plus locomotor training versus sham-stimulation plus locomotor training in chronic spinal cord injury (eWALK): a multicentre, triple-blind, randomised, sham-controlled trial.

BACKGROUND: Although some studies have investigated the effects of transcutaneous spinal cord stimulation (tSCS), most are small, uncontrolled, and exploratory. We aimed to determine whether 12 weeks of tSCS combined with locomotor training improves walking ability (with stimulation) in people with chronic spinal cord injury (SCI) more than locomotor training alone. METHODS: This international, multicentre, triple-blind, randomised sham-controlled trial (eWALK) was conducted at seven sites across Australia, USA, Scotland, and Spain. We recruited 50 community-dwelling individuals (aged ≥16 yea

Association of baseline immune cell composition with CAR-T cell expansion and survival in Relapsed/Refractory large B-Cell lymphoma.

BACKGROUND: Chimeric antigen receptor T (CAR-T) cell therapy has demonstrated remarkable efficacy in relapsed/refractory large B-cell lymphoma (R/R LBCL), yet nearly 40-60% of patients fail to achieve durable responses. The mechanisms underlying interpatient variability in CAR-T expansion and persistence remain incompletely understood. In this exploratory study, we preliminarily investigated the associations between baseline peripheral blood immune subsets, CAR-T expansion kinetics, and clinical outcomes. METHODS: We retrospectively analyzed 33 patients with R/R LBCL who received CD19/CD22 bi

The clinical and cost analysis of laser therapy for healing hard-to-heal leg ulcers: a self-control service evaluation study.

OBJECTIVE: Hard-to-heal (chronic) leg ulcers (LUs) can impair quality of life and, in severe cases, may require amputation. Although there are a few studies on the use of class III laser therapy with LUs, studies using powerful class IV lasers are scarce. The present study uses a class IV therapeutic laser (K-Cube 4; K-Laser; K-Laser USA, LLC, US) which uses four wavelengths (660nm, 800nm, 905nm and 970nm) of diodes in one device to target haemoglobin, water and cytochrome c oxidase simultaneously, with different frequency modulation and high power levels to enhance clinical efficacy and produ

Effects of transcutaneous auricular vagus nerve stimulation on patients with post-stroke insomnia: a randomized controlled trial protocol.

BACKGROUND: Post-stroke insomnia (PSI) is one of the most common and difficult-to-treat complications following stroke, with a prevalence exceeding 50%, and is an independent risk factor for stroke recurrence and secondary anxiety and depressive disorders. Current first-line strategies, namely sedative-hypnotics and cognitive behavioral therapy for insomnia, are constrained by the risks of tolerance, dependence, and cognitive side effects, as well as by limited medical resources and poor patient adherence. Transcutaneous auricular vagus nerve stimulation (taVNS), a non-invasive neuromodulation

Efficacy, Safety, Quality of Life, and Adherence of Dupilumab in Pediatric Atopic Dermatitis: A Systematic Review.

BACKGROUND: Atopic dermatitis (AD) is a chronic, relapsing inflammatory skin disease that disproportionately affects children, often causing significant physical, psychological, and social burdens. While biologic therapies have transformed AD management in adults, pediatric-specific evidence remains limited. Dupilumab, an interleukin (IL)-4 receptor α antagonist that inhibits IL-4 and IL-13 signaling, is currently the only US Food and Drug Administration (FDA)-approved biologic for pediatric AD. OBJECTIVE: To systematically review the efficacy, safety, quality of life, and adherence of dupilu
