Variation in antibiotic consumption in very preterm infants-a 10 year population-based study.

OBJECTIVES: Wide variations in antibiotic use in very preterm infants have been reported across centres despite similar rates of infection. We describe 10 year trends in use of antibiotics and regional variations among very preterm infants in Norway. PATIENTS AND METHODS: All live-born very preterm infants (<32 weeks gestation) admitted to any neonatal unit in Norway during 2009-18 were included. Main outcomes were antibiotic consumption expressed as days of antibiotic therapy (DOT) per 1000 patient days (PD), regional variations in use across four health regions, rates of sepsis and sepsis-a

The use of routinely collected electronic prescribing data to benchmark intravenous antibiotic use between two tertiary paediatric haematology-oncology inpatient units: a retrospective study.

BACKGROUND: High-quality systematic data on antimicrobial use in UK inpatient paediatric haematology-oncology services are lacking, despite this population being at high risk from antimicrobial exposure and resistance. OBJECTIVES: We conducted a retrospective study to demonstrate how routinely collected electronic prescribing data can address this issue. PATIENTS AND METHODS: This retrospective study describes and compares IV antibiotic consumption between two UK paediatric haematology-oncology inpatient units, between 2018 and 2022. Both sites provide similar services and receive proactive

Antibiotic Overuse in Premature Low Birth Weight Infants in a Developing Country.

BACKGROUND: Neonatal sepsis is a leading cause of child morbidity and mortality, especially in premature and low birth weight infants. Prompt antibiotic therapy is warranted, but its inappropriate use leads to bacterial resistance and adverse outcomes. Our objective is to describe the antibiotic use for late-onset sepsis in Peruvian premature infants. METHODS: This study is a prospective study as a secondary analysis of a clinical trial in 3 neonatal care units in Peru. We included infants in the first 72 hours of life, with birth weight (BW) <2000 g. We described the antibiotic use as length

Four-year analysis of microbial aetiology and antimicrobial sensitivity patterns of peritoneal-dialysis related peritonitis in a tertiary care facility.

Peritonitis related to peritoneal dialysis increases morbidity and mortality and is the main reason for switching to haemodialysis. In this study, we analysed the dialysate of 164 peritoneal dialysis patients that was sent to our laboratory between January 2005 and August 2009. There were 196 peritonitis episodes identified in 78 patients. For all episodes, microbial aetiologies and in-vitro antimicrobial sensitivities were determined in addition to parameters such as the leukocyte count (WBC) and the result of the Gram stain. Results in children were analysed separately because of previously

The Enterococcus faecalis superoxide dismutase is essential for its tolerance to vancomycin and penicillin.

OBJECTIVES: Enterococcus faecalis is a human commensal that has the ability to become a pathogen. Because of its ruggedness, it can persist in the hospital setting and cause serious nosocomial infections. E. faecalis can acquire multiple drug resistance determinants but is also intrinsically tolerant to a number of antibiotics, such as penicillin or vancomycin, meaning that these usually bactericidal drugs only exhibit a bacteriostatic effect. Recently, evidence has been presented that exposure to bactericidal antibiotics induced the production of reactive oxygen species in bacteria. Here, we

Chemoproteomic profiling of adaptive ATP-binding protein activity in Enterococcus faecium in response to host-derived cues.

Enterococcus faecium has become a significant nosocomial and opportunistic pathogen, increasingly implicated in severe infections such as urinary tract infections, bacteraemia, and endocarditis. Due to acquired resistance to vancomycin and ampicillin and high intrinsic resistance to other antibiotics, E. faecium infections are often challenging to treat. However, relatively little is known about the molecular factors used by E. faecium during infection. Here, we employed desthiobiotin-ATP as an activity-based probe to investigate the activity profile of ATP-binding proteins in E. faecium upon

Microbial community structure and antibiotic resistance genes in Dinaric karst groundwater under contrasting hydrological conditions.

Karst aquifers are vulnerable drinking water resources, yet microbiome-resistome studies remain scarce. We investigated microbial community composition and antibiotic resistance gene (ARG) profiles in five karst groundwater sources along an anthropogenic gradient in coastal Croatia under contrasting hydrological conditions. 16S rRNA amplicon sequencing showed that hydrological conditions were associated with community composition. Distance-based redundancy analysis identified total organic carbon as the variable associated with community structure, while ammonium and Escherichia coli were asso

Emerging Threat of Acinetobacter radioresistens Infection in Immunocompromised Patients.

Acinetobacter radioresistens is an uncommon human pathogen rarely reported to cause bacteremia. Its accurate identification is crucial yet challenging, with implications for antimicrobial stewardship due to its potential to harbor carbapenem resistance genes. We present the case of a 77-year-old male with metastatic lung adenocarcinoma, COPD, and dementia who presented with acute hypoxic respiratory failure and septic shock. Initial empiric antibiotics were vancomycin and cefepime. Gram stain of positive blood cultures revealed Gram-negative coccobacilli. The VERIGENE BC-GN microarray system i

Development and application of a rapid dual RPA-LFS assay for simultaneous detection of Enterococcus faecalis and Enterococcus faecium.

Enterococcus faecium and Enterococcus faecalis, ranked among the top three global nosocomial pathogens (WHO data), pose escalating threats due to multidrug resistance (particularly vancomycin-resistant strains, VRE) and foodborne transmission. Conventional detection relies on time-consuming culture-based typing (ISO 7899-2; 48-72 h) with > 12% false-negative rates, while molecular methods like qPCR require sophisticated instrumentation, limiting field deployment. This study developed a rapid dual recombinase polymerase amplification-lateral flow strip (RPA-LFS) assay for simultaneous detection

Pathogens and their resistance behavior in necrotizing fasciitis.

BACKGROUND: Necrotizing fasciitis (NF) is a rare but life-threatening condition in which extensive soft tissue destruction can occur very quickly if left untreated. Therefore, timely broad-spectrum antibiotic administration is of prognostic importance in addition to radical surgical debridement. AIM: This study evaluates the cases of NF in our hospital during the last ten years retrospectively with respect to the pathogens involved and their antimicrobial resistance. This approach aims to provide guidance regarding the most targeted initial antibiotic therapy. METHODS: We performed a retrosp

Prevalence, Antimicrobial Resistance and Toxin-Encoding Genes of Clostridioides difficile from Environmental Sources Contaminated by Feces.

Clostridioides difficile (C. difficile) is the most common pathogen causing antibiotic-associated intestinal diseases in humans and some animal species, but it can also be present in various environments outside hospitals. Thus, the objective of this study was to investigate the presence and the characteristics of toxin-encoding genes and antimicrobial resistance of C. difficile isolates from different environmental sources. C. difficile was found in 32 out of 81 samples (39.50%) after selective enrichment of spore-forming bacteria and in 45 samples (55.56%) using a TaqMan-based qPCR assay. A
