BACKGROUND: Today, treatment of the nondystrophic myotonias consists of mexiletine, although care has to be taken because of the proarrhythmogenic potential of this drug. In this article, we report years of experience with the carbonic anhydrase inhibitor acetazolamide.
PATIENTS: We present three children with nondystrophic myotonias.
RESULTS: During acetazolamide treatment, symptoms and signs of myotonia decreased in our children.

Twenty patients with primary open-angle glaucoma uncontrolled on single topical therapy completed a double dummy crossover study to compare acetazolamide tablets with a sustained release formulation (Sustet). The two preparations were equally effective, but no direct relationship was found between the intraocular pressure and the plasma concentration of acetazolamide. No difference between the formulations was found in the frequency or severity of side effects.

The inhibition by cyanate and acetazolamide of pig muscle carbonic anhydrase III (CA III) CO2 hydratase activity was studied in order to explore mechanistic features possibly unique to the muscle isoenzyme. Inhibitors of carbonic anhydrase were tested for their effects on Photosystem II (PS II) activity in chloroplasts. We find that formate inhibition of PS II turnover rates increases as the pH of the reaction medium is lowered. Bicarbonate ions can inhibit PS II turnover rates. The relative potency of the anionic inhibitors N-3, I-, OA-c, and Cl- is the same for both carbonic anhydrase and PS II. The inhibitory effect of acetazolamide on PS II increases as light intensity decreases, indicating a lowering of quantum yields in the presence of the inhibitor.
