10-year experience in prosthetics of extremity stumps in conditions of the Central Military Clinical Hospital N 6 was analyzed. In the hospital during the period from 1989 to 1999 prosthetic appliance was made in 203 patients with amputating defects of extremities, 37 of them with pair stumps and 1 with lack of segments of all extremities. Most patients (80.5%) had stumps at the level of femur and crus. Basing on the analysis of modern scientific literature and experience gained in hospital the authors have formulated organizational and methodical states. Thanks to observance of the stated pri

A series of microsurgical free flap reconstructions to amputation stumps of the upper as well as the lower extremities was reviewed in 7 male and 2 female patients. Indications included preservation of length after trauma in 6 patients and cure of local infection in 2 patients. In 1 patient an extensive defect after resection of a recurrent shoulder sarcoma required use of a complete arm fillet free flap for tumor reconstruction. Microvascular free flaps used included four scapular flaps, two fillet flaps from the amputated extremity, one anterolateral thigh flap, and one lateral arm flap. Sev…

Traumatic injuries to the thumb are frequently associated with mutilation of one or more of the remaining digits. The most common digital injuries associated with thumb loss are partial or complete amputation of the index finger or index and long fingers. Occasionally, satisfactory index finger function will be preserved with damage or amputation of the middle or ring fingers, making them the best candidates for transposition. Although pollicization of the index finger has received considerable attention in the literature, pollicization of injured middle and ring finger remnants has not. Trans

Persistent postsurgical pain after amputation is common and has a number of proposed risk factors. We describe the management of a patient with persistent pain after a below-the-knee amputation. We used a combination of general, peripheral, and neuraxial anesthesia techniques to control postoperative phantom limb and stump pain in a patient at high risk for developing chronic pain. With preemptive management and mechanism-directed therapies, adequate preoperative and postoperative pain control were achieved in attempts to reduce the risk of chronic phantom limb pain.

A 40-year-old woman had her right extremity avulsed at the proximal upper arm level and the wrist and hand of her left extremity irretrievably injured in a traffic accident. The right distal forearm was surgically amputated and replanted onto the stump of the left distal forearm. New strategy for nerve repair was applied and the function recovery of the cross-replanted hand was favorable. We thought that cross-extremity replantation was indicated when the patient suffered from bilateral total or subtotal amputation at different levels and orthotopic replantation was impossible.

The authors analyze the treatment of 54 patients with 65 limb stumps resultant from injuries to the limbs in 41 patients shortly exposed to traumatic agents and in 13 patients after a prolonged crushing of the limb during the earthquake in Armenia. Amputations were carried out as primary surgical wound treatment followed by open healing of the wound. To prevent infections, 12 patients were treated with the use of a local gnotobiologic isolation, the rest by traditional methods with due consideration for the wound process. Local treatment of stump wounds in the patients with the crush syndrome

BACKGROUND: The alignment of a prosthesis affects the way load is transferred to the stump through the socket; this is critically important for its comfort and function. The aim of the study was to assess the value of radiopaque materials in evaluation of stump-socket fit in people with below-knee amputation.
MATERIAL AND METHODS: Thirty-two amputees (skin breakdown group) had cutaneous lesions in the stump, and another 26 amputees (control group) had no lesion in the stump after training with their prosthesis. Metallic markers were attached to weight-bearing points in the internal socket. X-

BACKGROUND: : Fitting a femoral prosthesis in a transfemoral amputee with a very short amputation stump is challenging. This case report aimed to introduce an effective and simple method that can preserve the residual limb length by the implantation of antibiotic-loaded bone cement for the treatment of a patient with femoral periprosthetic infection.
CASE: : A 30-year-old man who had osteosarcoma at the age of 13 years underwent transfemoral amputation 17 years after the initial surgery because of periprosthetic infection. Antibiotic-loaded bone cement was inserted into the infected bone marr

BACKGROUND: The newly launched "German law of patients' rights" strenghthens the rights of patients and places special emphasis on the right to full information. This includes information on errors, mistakes and complications due to surgery.
ASSESSMENT: Surgery complications and mistakes are of main importance especially in the area of amputation because rehabilitation chances are dependent on good surgical quality. Therefore, it is necessary that surgeons know about these problems. Complications may be hematomas, bleeding, dysvascular problems, skin- or muscle-necrosis, and infections and wo

OBJECTIVE: The primary objective of this study is to compare the prevalence of phantom limb pain in New Zealand and Cambodian amputees and to assess the demographics of a sample of amputees from these two countries.
DESIGN: All participants were interviewed using a 12-question survey that covered demographic data and reason for amputation and assessed the presence of phantom limb sensation, phantom limb pain, and stump pain. Amputees attending an artificial limb center in Cambodia were approached and interviewed in person. New Zealand amputees attending the Waikato artificial limb center were

Pediatric traumatic limb amputations are rare and their acute and long term management can be challenging in this subgroup of patients. The lengthy and costly hospital stays, and resulting physical and psychological implications leads to significant morbidity. We present a summary of treatment principles and the evidence base supporting the management options for this entity. The initial management focuses on resuscitating and stabilization of the patients, administration of appropriate and adequate analgesics, and broad spectrum antibiotics. The patient should ideally be managed by an orthope

OBJECTIVE: The present study aims to evaluate early and late results after standardized below-knee amputation according to Brückner in vascular patients.
PATIENTS AND METHODS: During the period 1982 to 1989 72 patients with occlusive arterial disease stage IV underwent below-knee amputation at an average age of 69.7 years. 38 of the amputees could be re-investigated at the end of 1991 after an average follow-up of 5.3 years. Evaluation criteria consisted of wound healing, knee joint preservation, prosthetic fitting, stump appearance, gait capacity or ambulation, the need for assistance by ano

We report a new technique to create an effective lower extremity weight-bearing stump for two patients who had extensive segmental loss of femoral bone proximal to the distal femoral condyles. One patient had previously had complete resection of the proximal part of the femur because of an infection following the insertion of a custom femoral replacement and hip arthroplasty prosthesis. The other patient had had débridement of the femur from the subcapital line to the femoral condyles because of post-traumatic osteomyelitis after failure of a reconstruction with a massive allograft. Modern sockets for lower limb amputees utilize total contact and distribute some weight on the stump end. Its tolerance to bear weight varies but is better after joint disarticulation, however, systematic measures have been missing. Different levels, indications, shapes etc. were analysed with 102 measurements in 69 patients. The maximal-end-weight-bearing of the stump measured on a scale was much lower after transmedullar amputations than after disarticulations. Men had a mean tolerance more than 15 kg but women less than 10 kg. There was a positive correlation to body weight. Diabetics toler

Emergency appendectomy for acute appendicitis is the most common emergency surgical procedure performed all over the world. While amputating the appendix from the cecum, a small (usually less than 5 mm) stump is left behind. Below this, the suture or stapler is applied to secure the base of the appendix, which is now known as a stump. Stump appendicitis, the inflammation of appendiceal remnant after an appendectomy, is a rare phenomenon. Additionally, the incidence of adenocarcinoma in the stump of the appendix is also extremely rare and can present with the symptoms of appendicitis. Adenocarc

OBJECTIVE: To describe the factors associated with early (≤30 days) and late (31-365 days) hospital readmissions after lower extremity amputations in Canada.
METHODS: A retrospective cohort study was carried out for all Canadian adults who underwent elective lower extremity amputations in the years 2006 to 2008 for nontraumatic indications. Patients were identified from the Canadian Institute for Health Information's Discharge Abstract Database, which includes all hospital admissions across Canada, with the exception of the Province of Quebec.
RESULTS: During the study period, 3823 patients

INTRODUCTION: Previously, traumatic amputations particularly those caused by blunt trauma were treated with proper subsequent amputations. However, this led to postoperative pain, disturbances in multiple functions of the extremities including sensory capability and fine motor dexterity.
METHODS: We presented five cases of traumatic fingertip amputation treated with spare part composite graft. In spare part composite grafting, both the amputated finger and the distal end (stump) of injured finger was fixated directly after skeletonization of the amputated finger. The blood supply initially de

The incidence of the diabetic foot is increasing worldwide. Because evidence has shown that transmetatarsal amputation is associated with fewer failures in amputations of the diabetic foot with or without peripheral arterial disease, improving its management and surgical technique is a mission for the surgeon. Conventional transmetatarsal amputation has held firm, however, for more than 150 years. With a new concept for the transmetatarsal amputation method aimed at a better outcome, we propose a modified procedure for preserving the soft tissue between the metatarsal bones (the vasculature co
