Functional Regurgitation of Atrioventricular Valves and Atrial Fibrillation: An Elusive Pathophysiological Link Deserving Further Attention.

In patients with structurally normal atrioventricular valvular apparatus, functional regurgitation of the mitral or tricuspid valves has been attributed mainly to ventricular dilation and/or dysfunction, through a combination of annulus dilation and tethering of the valve leaflets. The occurrence of functional regurgitation of atrioventricular valves in patients with long-standing persistent atrial fibrillation and atrial dilation but normal ventricular size and function has received much less attention, and its peculiar mechanisms still remain to be understood. This distinct form of functiona

Endoscopic management of bile duct stones in patients with surgically altered anatomy.

Bile duct stones in patients with surgically altered anatomy still pose a challenge to endoscopists. For successful endoscopic management of bile duct stones, there are multiple hurdles: Intubation to the afferent limb, biliary cannulation, ampullary intervention and stone extraction. The major advancement in this area is the development of dedicated device-assisted endoscopes for endoscopic retrograde cholangiopancreatography (ERCP). In patients with Billroth II reconstruction, a high technical success rate is reported using a duodenoscope but can be complicated by a potentially high perforat

Morphological description of male genital organs of Marca's marmoset (Mico marcai).

Morphological characterisation of the genital organs of primates may bring significant contributions to the understanding of different reproductive behaviours and support new conservation strategies. However, relevant or detailed descriptions of genital morphology of several primate species are still lacking. This study describes the gross and microscopic anatomy of the internal and external genitalia of Marca's marmoset (Mico marcai). The same organs described in other primate species were identified here, but some anatomical particularities were detected, such as absence of a dartos tunic, p

Descent of the human larynx: An unrecognized factor in airway distress in babies with cleft palate?

The intranarial epiglottis, a feature of all newborn mammals, allows suckling and breathing to continue almost simultaneously by separating an oral food stream from a nasal airstream. In contrast to other mammals, the human larynx descends in the neck between birth and six months, extending the distance between the caudal aspect of the soft palate and the cephalic tip of the epiglottis. The mechanism of airway protection changes from a pattern in which an upright epiglottis is grasped by an intact palatopharyngeal sphincter to one in which the epiglottis folds down over the laryngeal aditus an

The marmoset monkey as a model for visual neuroscience.

The common marmoset (Callithrix jacchus) has been valuable as a primate model in biomedical research. Interest in this species has grown recently, in part due to the successful demonstration of transgenic marmosets. Here we examine the prospects of the marmoset model for visual neuroscience research, adopting a comparative framework to place the marmoset within a broader evolutionary context. The marmoset's small brain bears most of the organizational features of other primates, and its smooth surface offers practical advantages over the macaque for areal mapping, laminar electrode penetration

Long-term results of the transattical approach: an alternative technique for cochlear implantation.

Cochlear implantation through the standard posterior tympanotomy approach may sometimes be challenging. In cases with complicated anatomy or malformations, an alternative surgical technique may be chosen. The transattical approach is a simple technique based on a mastoidectomy and a transmeatal cochleostomy. The electrode is driven to the middle ear through the attic. This way it overcomes some of the standard technique's drawbacks. Our aim is to assess the value of this alternative approach in cochlear implant surgery. We conducted a comparative retrospective study; we included 74 patients op

Spatial packing, cranial base angulation, and craniofacial shape variation in the mammalian skull: testing a new model using mice.

The hypothesis that variation in craniofacial shape within and among species is influenced by spatial packing has a long history in comparative anatomy, particularly in terms of primates. This study develops and tests three alternative models of spatial packing to address how and to what extent the cranial base angle is influenced by variation in brain and facial size. The models are tested using mouse strains with different mutations affecting craniofacial growth. Although mice have distinctive crania with small brains, long faces, and retroflexed cranial bases, the results of the study indic

Role of echocardiography in the diagnosis and evaluation of severity of mitral and tricuspid stenosis.

The presence, severity, and hemodynamic consequences of mitral and tricuspid stenosis can be determined by echocardiographic techniques. In mitral stenosis two-dimensional echocardiographic imaging allows definition of leaflet anatomy and dynamics, subvalvular disease, ventricular function, and involvement of other valves. Spectral and color Doppler echocardiographic techniques permit accurate measurement of transvalvular gradient, determination of functional orifice area, evaluation of associated valvular regurgitation, and assessment of pulmonary artery pressures. These approaches are of rec

Postoperative bile duct strictures.

Bile duct strictures are an uncommon but serious complication of primary operations on the gallbladder or biliary tree. Most strictures occur as a result of injury to the bile duct during cholecystectomy. In addition, strictures can occur at the site of previous biliary anastomoses for reconstruction of the biliary tree. Most patients with benign bile duct strictures present soon after their initial operation; however, in some cases, presentation is delayed for years. Cholangiography is essential for defining the anatomy of the biliary tree prior to management. In many cases, nonoperative bili

The present status of spinal fusion in the treatment of lumbar intervertebral joint disorders.

Every type of fusion has its enthusiastic supporters. Unfortunately, the literature includes comparative analyses of various fusion procedures. Much of the effort spent in the clinical assessment of spinal fusion must be reinvested; neither the indications for diskectomy nor spinal fusion have been sufficiently clear-cut and accurately recorded. Moreover, spinal fusions often have not been applied to treat the disorder or to correct the deformity. It is essential at the outset to establish the mechanical fault; this is achieved only by a thorough understanding of the underlying pathologic lesi

The rationales of interlocking nailing of the femur, tibia, and humerus.

Interlocking nailing has been increasingly used to treat acute and chronic afflictions of long bones. Although the interlocking construct is similar, regardless of the anatomic area of application, the results and complications of this type of fixation in the femur, tibia, and humerus differ. This review compares the anatomy, biologic response to injury, and functional expectations of these 3 distinct long bones with respect to interlocking nailing. Final results seem related more closely to the bony anatomy and soft tissue injury than to any inherent success of interlocking techniques.

Evaluation of radiological data for three-dimensonal physical models to assist personalized anesthesia in neuroaxial interventions with difficult anatomy.

The objective of this study is to compare conventional anesthesia practices with the use of a preoperative, personalized, life-sized three-dimensional (3D) physical spine model in individuals where neuroaxial anesthesia is anticipated to be challenging due to difficult anatomy as difficult anatomy as the presence of spinal segment fusion, reduced spinal mobility and flexibility, joint stiffness, and movement restriction in the lumbar spine. Twenty individuals aged between 30 and 90, who were expected to undergo neuroaxial anesthesia with a difficult anatomical profile, and who had completed pr

Development and Testing of a Novel Stabilizer Clamp to Reduce Motion During Instrumented Cervical Fusions: The Caption System.

BACKGROUND: Navigation of spinal instrumentation is heavily dependent on maintaining an accurate registration of the local anatomy to the navigation software. Any motion of the anatomy reduces accuracy. Historically, navigation in the cervical spine has been limited by the mobile nature of the anatomy. Here, we describe the development and testing of a novel stabilizer clamp designed to reduce motion and improve stability for navigated-instrumentation accuracy. METHODS: Testing was completed both on a flat-top table and separately on a Jackson table. On both tables, the cadaveric specimen was

The Morphological Considerations of the Male Reproductive Tract of the Adult White Spotted Rose Beetle Oxythyrea funesta (Coleoptera: Scarabaeidae)-Light and Scanning Electron Microscope Observations.

Scarabaeidae includes many phytophagous beetles, many of which are significant agricultural pests. Despite their importance, research on the reproductive tract of this family remains limited, and existing studies are mostly restricted to drawings and light microscopy. In fact, there is no study on this subject belonging to this genus. Therefore, in this study, we examined the histology and anatomy of the male reproductive tract in Oxythyrea funesta (Poda, 1761) using light and scanning electron microscopy. Our findings revealed that the reproductive system of O. funesta comprises two testes, v

Histological Evidence for the Presence of a Glandular Stomach in the Mudskipper Scartelaos histophorus From the Mekong Delta, Vietnam.

This study describes the gross anatomy and histological organization of the digestive tract of the mudskipper Scartelaos histophorus from the Mekong Delta, Vietnam. Thirty adult specimens were collected from intertidal mudflats, and the digestive tract was examined using gross anatomical observation and routine paraffin histology with haematoxylin and eosin staining. Gross anatomical observation differentiated the digestive tract into the oesophagus, stomach, intestine and rectum, whereas histological examination focused on the stomach, intestine and rectum. The stomach was characterized by pr

The Sublingua of Lemur catta and Varecia variegata: Only a Cleaning Function?

The sublingua is an anatomical structure located under the tongue. This rare organ can be present in some animals as a rudimentary structure, but among prosimian primates, such as lemurs and lorises, it is fully developed. In addition to the sublingua, prosimians have modified lower incisors and canines called "dental comb". The anatomy of sublingua has been studied macro and microanatomically since the early 19th century. Most authors argue that the sublingua is an oral morphological adaptation to develop a toothbrush's role in cleaning the dental comb. However, others assert that the functio
