Effect of Local Anesthetic Solution at Different Temperatures for Epidural Labor Analgesia on Intrapartum Fever: A Randomized Clinical Trial.

BACKGROUND: Whether heating local anesthetic solutions to core body temperature (37°C) for epidural labor analgesia reduces intrapartum fever incidence remains undefined in the current literature. METHODS: This double-blind randomized controlled trial (RCT) enrolled 220 nulliparous parturients (18-35 years, American Society of Anesthesiologists [ASA] physical status II, term singleton pregnancy). Participants were randomized to receive epidural labor analgesia with 0.075% ropivacaine + 0.5 µg/mL sufentanil at 37°C (warmed group) or 22°C (room-temperature group). Epidurals were placed at L3-L4

Case Report: Dynamic characteristics of myocardial depression after severe bupivacaine-induced local anesthetic systemic toxicity.

BACKGROUND: Bupivacaine-induced local anesthetic systemic toxicity (LAST) can lead to severe myocardial depression and circulatory collapse, and some patients may require venoarterial extracorporeal membrane oxygenation (VA-ECMO) support. However, serial and visually interpretable clinical data regarding the dynamic changes in cardiac function, the timing of the nadir, and the recovery trajectory of severe bupivacaine-induced myocardial depression remain limited. CASE PRESENTATION: A 32-year-old woman at term underwent combined spinal-epidural anesthesia. Shortly after intrathecal administrat

Analgesic and Local Anesthetic Effects of a Clove-Derived Injectable Formulation via Modulation of Oxidative Stress and Nociceptive Signaling.

The major bioactive constituent of Syzygium aromaticum (clove), eugenol, has been reported to possess analgesic and local anesthetic properties. However, direct evidence regarding the local anesthetic and antinociceptive effects of clove extract itself or its injectable formulations remains limited. Therefore, this study evaluated the neuroprotective, antinociceptive, and local anesthetic-like effects of S. aromaticum pharmacopuncture (SAP) using in vitro and in vivo models, with comparative assessment against lidocaine. Primary dorsal root ganglion (DRG) neurons were exposed to H2O2-induced o

Electrophysiological Response of the Non-Anesthetized Primate Brain to Minimally Invasive Local Infrared Neural Stimulation in Chronic Experiments.

Infrared neural stimulation (INS) represents an invasive technique for modulating brain activity in animals, particularly primates, which serve as effective models for human brain research. Noninvasive approaches, such as transcranial laser stimulation, are safer but have lower spatial and temporal resolution, primarily altering metabolic processes rather than directly stimulating specific neurons. Invasive techniques provide better resolution by targeting neurons with focused laser beams but require intricate surgeries that damage the meninges, limiting studies to short-term experiments condu

Structure-Property Relationships in Novel Series of Photoswitchable Local Anesthetic Ethercaine Derivatives: Emphasis on Biological and Photophysical Properties.

The insufficient selectivity of existing local anesthetics can lead to serious adverse effects. Considering the widespread use of this class of drugs, the development of new local anesthetics that do not cause side effects is an important task. One approach to address this issue is the use of photopharmacology, which enables the creation of agents with light-controlled biological activity. Several examples of azobenzene-based photoswitchable blockers of voltage-gated sodium (Nav) channels have been described so far. These compounds can be used as light-controlled local anesthetics, one of whic

A promising novel local anesthetic for effective anesthesia in oral inflammatory conditions through reducing mitochondria-related apoptosis.

Local anesthetics (LAs), such as articaine (AT), exhibit limited efficacy in inflammatory environments, which constitutes a significant limitation in their clinical application within oral medicine. In our prior research, we developed AT-17, which demonstrated effective properties in chronic inflammatory conditions and appears to function as a novel oral LA that could address this challenge. In the present study, we further elucidated the beneficial effects of AT-17 in acute inflammation, particularly in oral acute inflammation, where mitochondrial-related apoptosis played a crucial role. Our

Ropivacaine-epinephrine-clonidine-ketorolac is an effective opioid-sparing local anesthetic for patients undergoing posterior spinal fusion.

BACKGROUND CONTEXT: Ropivacaine-Epinephrine-Clonidine-Ketorolac (RECK) cocktail can improve pain control in patients undergoing lumbar decompression. Given the aging population, rising healthcare costs, the opioid epidemic, and associations of acute pain control with long-term opioid use, effective opioid-sparing analgesia following spinal fusion surgery may impart societal benefits. PURPOSE: We aimed to investigate whether RECK was an effective local anesthetic for patients undergoing posterior spinal fusion. STUDY DESIGN/SETTING: Single-center retrospective cohort study. PATIENT SAMPLE: P

Neuromodulation using a hybrid technique of combined perineural local anesthetic and nerve stimulation in six challenging clinical scenarios.

PURPOSE: Postamputation pain is challenging because of complex mechanisms involving a multitude of pain pathways and psychological factors. This patient population also tends to have extensive comorbidities with or without a background of chronic pain. Electrical neuromodulation such as peripheral nerve stimulation has gained traction in the realm of chronic pain. Recently, the off-label use of hybrid perineural nerve stimulation in combination with locoregional block via the stimulating nerve block catheter has been described in single-center case reports. CLINICAL FEATURES: Herein, we prese

Downregulation of HSPA12A underlies myotoxicity of local anesthetic agent bupivacaine through inhibiting PGC1α-mediated mitochondrial integrity.

Local anesthetics (LAs) are widely used for intraoperative anesthesia and postoperative analgesia. However, LAs (e.g. Bupivacaine) can evoke myotoxicity that closely associated to mitochondrial damage. PGC1a is a mast co-factor for mitochondrial quality control. We have recently demonstrated that PGC1a can be activated by HSPA12A in hepatocytes, suggesting a possibility that HSPA12A protects from LAs myotoxicity through activating PGC1α-mediated mitochondrial integrity. Here, we reported that HSPA12A was downregulated during Bupivacaine-induced myotoxicity in skeletal muscles of mice in vivo a

A Randomized Comparison between 4, 6 and 8 mL of Local Anesthetic for Ultrasound-Guided Stellate Ganglion Block.

BACKGROUND: Because it affords greater accuracy than landmark-based techniques, ultrasound guidance may reduce the volume of local anesthetic required for sympathetic blockade of the upper extremity. We hypothesized that 4 mL would provide a similar clinical effect when compared to larger volumes. METHODS: One hundred and two patients with chronic neuropathic pain of the upper extremity or face were randomly assigned to receive an ultrasound-guided (USG) stellate ganglion block (SGB) with either 4 mL (group A), 6 mL (group B) or 8 mL (group C) mL of 1.0% lidocaine. The role of epidural contrast distribution in predicting the effectiveness of steroid and local anesthetic injection in patients with lumbar spinal stenosis.

STUDY DESIGN: A prospective observational comparative study. BACKGROUND: Because epidural corticosteroids are more effective for pain relief when delivered close to the site of pathology, lumbar spinal stenosis with multilevel stenotic areas can be treated more effectively when medications cover all of the stenotic areas. Distribution of medications to the pathologic sites is considered an important factor in the effectiveness of inter-laminar epidural injection. OBJECTIVE: To determine whether spread of a lidocaine and triamcinolone mixture over the stenotic areas improves clinical symptom

Local Anesthetic Drug Inhibits Growth and Survival in Chronic Myeloid Leukemia Through Suppressing PI3K/Akt/mTOR.

BACKGROUND: Apart from the known anesthetic and antiarrhythmic effects, recent studies also highlight the anticancer activities of local anesthetics. In line with the findings, our work shows that ropivacaine, an amide-linked local anesthetic drug, targets chronic myeloid leukemia (CML) via inhibiting PI3K/Akt/mTOR. MATERIALS AND METHODS: The effects of ropivacaine in CML cell lines and primary stem or progenitor cells were investigated using apoptosis, proliferation and colony formation assays. The effects of ropivacaine on proliferation and survival pathways were analyzed using Western blot
