Subsequent Pain or Injury After Foot and Ankle Surgery in Patients Receiving Workers' Compensation.

BACKGROUND: Workers' compensation (WC) has been associated with poor outcomes following a variety of injuries and surgeries, but rates of subsequent pain or injury (SPI) following surgery have not been studied. The purpose of this study was to investigate the rates, locations, and risk factors of SPI in WC patients and non-WC patients who underwent the same surgeries. METHODS: With institutional review board approval, records from foot or ankle surgery performed by author P.P. from 2009 to 2015 were obtained. A retrospective chart review was performed on all WC and non-WC patients with at lea

Treatment of a Scientifically Neglected Ankle Injury: The Isolated Medial Malleolar Fracture. A Systematic Review.

Isolated medial malleolar fractures are frequently encountered injuries. Literature regarding their treatment, though, is scarce and contradicting. The aim of this systematic review is to compare surgical and conservative treatment of isolated medial malleolar fractures considering complication rates and functional outcomes. PubMed, Embase, Cochrane, and CINAHL were searched for this review. Articles from 1980 or later, written in English, French, German, or Dutch, reporting any outcome of 10 or more isolated medial malleolar fractures in skeletally mature patients were included. Study quality

Diagnostic Accuracy of Radiologic Methods for Ankle Syndesmosis Injury: A Systematic Review and Meta-Analysis.

: Misdiagnosis and inadequate treatment of syndesmosis could result in significant long-term morbidity including pain, instability, and degenerative changes of the ankle joint. The objective of this systematic review and meta-analysis was to determine whether radiologic tests accurately and reliably diagnose ankle syndesmosis injury. Medline, Embase, and Cochrane were searched. The database search resulted in 258 full text articles that we assessed for eligibility, we used eight studies that met all the inclusion criteria. In subgroup meta-analysis, the sensitivity analysis showed significant

Evaluation of Articular Cartilage Injury Using Computed Tomography With Axial Traction in the Ankle Joint.

BACKGROUND: Although chondral or osteochondral injuries are usually assessed by magnetic resonance imaging, its accuracy can be low, presumably related to the relatively thin cartilage layer and the close apposition of the cartilage of the talus and tibial plafond. We hypothesized that axial traction could provide a contrast between the articular cartilage and joint cavity, and it enabled the simultaneous evaluation of cartilage and subchondral bone. The purpose of this study was to assess the feasibility of using computed tomography (CT) imaging with axial traction for the diagnosis of articu

Diagnosis and treatment of chronic lateral ankle instability with ligamentum bifurcatum injury: An observational study.

This study aimed to report our institution's experience in the diagnosis and treatment of chronic lateral ankle instability (CLAI) with ligamentum bifurcatum (LB) injury. This retrospective study included 218 consecutive patients with CLAI who underwent surgery from January 2012 to December 2015. The 218 patients received tendon allograft reconstruction of the lateral ligament. CLAI was combined with LB injury in 51.4% (112/218) of patients. The 112 patients with concurrent LB injury had this treated simultaneously; 36 patients underwent excision of the anterior process of the calcaneus, 68 und

Biopsychosocial Factors Associated with Foot and Ankle Pain and Injury in Irish Dance: A Prospective Study.

BACKGROUND: Foot and ankle pain/injury (FAPI) is the most common musculoskeletal problem suffered in Irish dancing. A prospective examination of risk factors for FAPI in this cohort has never been performed. STUDY DESIGN: Prospective study over 1-year. METHODS: 85 elite adult Irish dancers were screened at baseline for biopsychosocial factors and followed up prospectively each month for 1 year to evaluate FAPI rates and potential risk factors. Subjects who suffered from multiple incidences of FAPI (with no pain/injury reported elsewhere in the body) or at least one moderate episode of FAPI w

Diagnostic Value of Early Magnetic Resonance Imaging After Acute Lateral Ankle Injury.

We report a retrospective study of 171 consecutive patients with a lateral ankle sprain. All the patients with direct or blunt force trauma were excluded. Within 21 days of injury, 115 (67.25%) patients had undergone magnetic resonance imaging to evaluate for more serious or significant injuries. The average patient age was 44.09 years. Of the 115 patients, 75 (65.23%) had findings noted to be "significant." MRI can serve as a valuable and underused tool in the evaluation of acute lateral ankle injuries. The underuse of MRI might explain the high degree of variability in patients recovering fr…

Unlocking the talus by eversion limits medial ankle injury risk during external rotation.

Eversion prior to excessive external foot rotation has been shown to predispose the anterior tibiofibular ligament (ATiFL) to failure, yet protect the anterior deltoid ligament (ADL) from failure despite high levels of foot rotation. The purpose of the current study was to measure the rotations of both the subtalar and talocrural joints during foot external rotation at sub-failure levels in either a neutral or a pre-everted position as a first step towards understanding the mechanisms of injury in previous studies. Fourteen (seven pairs) cadaver lower extremities were externally rotated 20° in

Soccer-specific fatigue decreases reactive postural control with implications for ankle sprain injury.

To quantify the influence of soccer-specific fatigue on reactive dynamic balance, ten male professional soccer players (age 24.7 ± 4.4 yr, body mass 77.1 ± 8.3 kg, VO2max 63.0 ± 4.8 ml·kg·min(-1)) completed an exercise protocol replicating the activity profile of match-play. Pre-exercise, and at 15 min intervals, players completed three balance tasks requiring response to a system perturbed to induce either plantar flexion of the ankle or inversion of the ankle (by rotation or translation). ANOVA revealed a significant main effect for exercise duration in each task, with both reaction time and

A dynamic ultrasound examination for the diagnosis of ankle syndesmotic injury in professional athletes: a preliminary study.

BACKGROUND: Syndesmotic ankle injuries are not easy to recognize when an associated fracture or frank diastasis is not present. There is a need for a simple, fast, inexpensive, and easily reproducible diagnostic tool to assess the integrity of the distal tibiofibular synedesmosis. HYPOTHESIS: Dynamic ultrasound (US) examination can accurately diagnose anteroinferior tibiofibular ligament (AITFL) rupture. STUDY DESIGN: Cohort study (diagnosis); Level of evidence, 2. METHODS: We evaluated 3 groups: 9 consecutive professional athletes with recent AITFL rupture, a control group of 18 subjects w…

Lateral ligament injury of the ankle in children. Follow-up results of primary surgical treatment.

Among 40 acutely injured ankles in children, surgery revealed a cartilaginous and/or bony fragment in 19, and an isolated rupture of the anterior talo-fibular ligament without any lesion of the bone or cartilage in another 17. In four ankles there was no ligament lesion. The lesions were surgically repaired. All ankles healed well and were painless and functionally stable at follow-up. In four ankles radiographs showed a small subfibular fragment, in which bony fusion had failed, but even these ankles were stable. Two other ankles gave a slightly positive sign in the clinical anterior drawer t

Cast versus functional brace in the rehabilitation of patients treated for an ankle fracture: protocol for the UK study of ankle injury rehabilitation (AIR) multicentre randomised trial.

INTRODUCTION: Each year in the UK over 120 000 people fracture their ankle. It is not known what the best rehabilitation strategy is for these people. Traditionally standard care has involved immobilisation in a plaster cast but an alternative is a functional brace, which can be removed to allow early movement. This paper details the protocol for a multicentre randomised trial of plaster cast immobilisation versus functional bracing for patients with an ankle fracture. METHODS AND ANALYSIS: We will recruit adults with a fractured ankle, for which the treating clinician would consider plaster

[Development of prediction model for therapeutic efficacy of Jingu Tongluo Granules on acute lateral ankle ligament injuries based on Delta-radiomics].

The efficacy of Jingu Tongluo Granules in treating acute lateral ankle ligament injuries is remarkable, however, accurately assessing its clinical efficacy remains challenging. Therefore, a machine learning-based prediction model for the therapeutic efficacy of Jingu Tongluo Granules in treating acute lateral ankle ligament injuries was developed using Delta-radiomics. The study involved 107 patients with acute anterior talofibular ligament injuries who received treatment for two weeks with topical Jingu Tongluo Granules based on the protection, optimal load, ice, compression, elevation(POLICE
