The familiarity with the ancient disease anthrax from the second millennium B.C. through the second millennium A.D. is reviewed, providing the backdrop to the modern understanding of this disease as covered in the remainder of the volume. By means of an overview of the aetiology, ecology, epidemiology, clinical manifestations, pathology and bacteriology of the naturally acquired disease, this opening chapter also lays down the groundwork for the subsequent state-of-the-art chapters.

Anthrax, a disease of great historical interest, is once again making headlines as an agent of biological warfare. Bacillus anthracis, a rod-shaped, spore-forming bacterium, primarily infects herbivores. Humans can acquire anthrax by agricultural or industrial exposure to infected animals or animal products. More recently, the potential for intentional release of anthrax spores in the environment has caused much concern. The common clinical manifestations of anthrax are cutaneous disease, pulmonary disease from inhalation of anthrax spores, and GI disease. The course of inhalational anthrax is…

In December 1988 an indigenous case of cutaneous anthrax was identified in Texas. The patient, a 63-year-old male Hispanic from southwest Texas, was a sheep shearer and had a recent history of dissecting sheep that had died suddenly. He experienced an illness characterized by left arm pain and edema. A necrotic lesion developed on his left forearm, with cellulitis and lymphadenopathy. After treatment with oral and intravenous penicillins, the patient fully recovered. Western blot testing revealed a fourfold or greater rise in antibody titer to Bacillus anthracis protective antigen and lethal f…

Most livestock vaccines in use throughout the world today for immunization against anthrax are derivatives of the live spore vaccine formulated by Sterne in 1937 and still use descendants of his strain 34F2. Credit belongs to this formulation for effective control in many countries with considerable reduction, sometimes complete elimination, of the disease in animals and, since man generally acquires it from livestock, in man also. However, there are some contraindications of its use and situations in which it cannot be easily administered, and room for development of a successor is discussed.…

Two llama calves died 3 days after inoculation with anthrax vaccine. Concurrent administration of ivermectin and other biologics may have enhanced the infectivity of the Sterne strain vaccine of Bacillus anthracis. This experience suggests that the Sterne strain of anthrax vaccine can induce fatal disease when given to young llamas and should be used only with extreme care and in face of strong "at risk" situations.
