One hundred and sixty-nine aphakic eyes underwent secondary implantation of anterior chamber lenses. The postoperative visual acuity decreased by one or more lines in five eyes (3%), remained the same in 71 eyes (42%), and improved by one or more lines in 93 eyes (55%). The presence of macular degeneration, background diabetic retinopathy, and open-angle glaucoma with a cup-to-disc ratio of 0.6 or less did not contribute to any visual deterioration.

When we compared the incidence of retinal detachment in 27 eyes after extracapsular cataract surgery and late posterior capsulotomy with the incidence of general aphakic detachment, we found no relationship between the onset of retinal detachment and the numbers, types, or locations of breaks. The interval between cataract extraction and discission had a significant effect on recovery of visual acuity. Sixty-five epikeratophakia procedures have been performed in 63 patients; visual acuity data have been tabulated on 31 patients with 4 to 30 months follow-up. Timolol maleate produced no significant difference in the initial mean intraocular pressure after keratoplasty using disparate-sized grafts (7.5 mm recipient, 8.0 mm donor). However, timolol did significantly reduce the number of eyes with an intraocular pressure elevation greater than 24 mm Hg. A series of patients with equal-sized donor and recipient cuts (8.0 mm) had initial intraocular pressure of 19.6 mm Hg. The normal intraocular pressure in these patients is thought to be due to superficial suturing, thereby preventing angle distortion.

In a clinic where 250 children have been fitted with contact lenses between 1976 and 1982, one hundred and five were bilateral infantile aphakic patients. A review of the data sheets of 43 bilateral infantile aphakic patients in this group implies that the corneal complications of treatment are substantial but surmountable and that the risks and difficulties of contact lens treatment are probably justifiable if the recent reports of improved visual function in these patients are substantiated.

Recent reports suggest that the aphakic eye develops less corneal edema than the phakic eye during hydrogel contact lens wear. Measurements were made of several ocular characteristics that might account for this difference in nine unilateral aphakic subjects. Two patients had corneal erosions and hypopyon iridocyclitis associated with continuous wear of aphakic soft contact lenses. Both patients were treated with cycloplegia and patching without antibiotics or corticosteroids. Both patients fully recovered.

The ultrastructure of Descemet's membrane was studied by transmission electron microscopy in nine corneal buttons from aphakic eyes with bullous keratopathy. This paper reports the study over a 2-year period of forty patients with uniocular aphakia fitted with spherical extended wear hydrophilic lenses: Sauflon 85 per cent and Duragel 72 per cent. The patients' ages ranged from 17 to 87 years (mean 52). They were unable to handle a daily wear soft lens or tolerate a hard lens. The success rate was 85 per cent.

The influence of total vitrectomy on the success rate of filtration surgery was investigated, with 47 operations (6 reoperations) being performed on 41 patients. At six months postoperatively, successful control (IOP less than 21 mm Hg) was achieved in 32 of 41 patients (78%). Twenty-four patients had filtration blebs present. Total vitrectomy favorably influences the result of filtration surgery. However, the morbidity rate (two retinal detachments and two cases of cystoid macular edema) is higher than in standard filtration procedures.

Eighteen rhesus monkeys underwent lens implantation with Choyce Mark VIII, Binkhorst iridocapsular, or Shearing posterior chamber lenses. They were sacrificed 4-28 months postoperative. The eyes were compared clinically and histologically. Controls included unoperated eyes and eyes with lens extraction without implantation. Several histologic findings pertained equally to cataract extraction with or without lens implantation. In a series of 1,000 consecutive intracapsular cataract extractions, the most important peroperative complications were rupture of the capsule in 6.5% of the cases and vitreous loss in 3.0%. Spectacles with moisture chambers for the treatment of dry keratitis. The dry corneal syndrome is very much improved by the creation of a humid "microclimate" maintained by cutaneous transpiration. The placing of spectacles with sealed shells allows these conditions to be achieved. Post operative glasses for aphakic patients. The placing of optical glasses immediately after surgery of the cataract helps the comfort of the aphake very much. I analyzed the aphakic refractive errors of men and women separately in a statistical study of patients with aphakic retinal detachments. I compared the distribution of aphakic refractive errors in a population of 81 adult patients with nontraumatic aphakic retinal detachment with that of a randomly selected control population of 93 adult patients with aphakia. A new procedure for the treatment of uncontrolled aphakic glaucoma has been the subject of a carefully conducted prospective clinical trial in 14 patients. The study spanned more than five years. The transpupillary argon laser photocoagulation of the ciliary process procedure, done on an outpatient basis, shows promise as a convenient, low-risk, and useful alternative procedure in selected aphakic glaucoma cases that are poorly controlled by medical or surgical measures.
