Optic neuropathy associated with sleep apnea syndrome.

OBJECTIVE: The study aimed to determine ocular abnormalities in sleep apnea syndrome (SAS), an entity characterized by repetitive upper airway obstructions during sleep, inducing hypoxia and sleep disruption with the risk of cardiovascular and neurologic sequelae. DESIGN: The study design was a case series. PARTICIPANTS: Nine patients referred for evaluation of suspected SAS participated. INTERVENTION: Complete ophthalmologic examination, including computerized perimetry, was performed. MAIN OUTCOME MEASURES: Correlations between the respiratory disturbance index (RDI) during night sleep, …

Sleep apnea in patients receiving growth hormone.

Among 145 patients treated with recombinant human growth hormone (GH), four developed sleep apnea (two obstructive, two mixed) associated with tonsillar and adenoidal hypertrophy in three. These four patients had no local risk factors predisposing to upper airway obstruction (i.e., frequent pharyngitis or sinusitis). Clinical and/or polysomnographic features of sleep apnea improved following cessation of GH therapy in one patient, and following tonsillectomy and adenoidectomy in all patients. The present observations indicate that, albeit rarely, obstructive and/or central sleep apnea may occu…

Laser-assisted uvulopalatoplasty for obstructive sleep apnea.

Laser-assisted uvulopalatoplasty (LAUP) can reduce snoring by sequentially removing excessive vibratory tissue of the velopharynx. The procedure can be performed under local anesthesia in an office setting. Since the appearance of the soft palate is similar after both LAUP and uvulopalatopharyngoplasty, LAUP may also be efficacious in the management of obstructive sleep apnea syndrome (OSAS). LAUP was performed in 34 consecutive prospectively evaluated patients with OSAS. Of the 34 patients, 28 (82%) were male; the mean age was 53 years, and the mean body mass index was 31.1. In the study popu…

[Obstructive sleep apnea syndrome and cardiovascular diseases].

Obstructive sleep apnoea syndrome is due to pharyngeal obstruction of inspiratory airflow with preservation of thoraco-abdominal respiratory movements. This disease has been described for about thirty years, but is now the subject of growing interest. According to the increasingly abundant literature on this subject, OSAS is associated with essentially cardiovascular morbidity and mortality (systemic hypertension, pulmonary hypertension, heart failure, coronary heart disease, arrhythmias, cerebral vascular accidents and sudden death). The pathophysiology of its underlying mechanisms and its co

Familial aggregates in obstructive sleep apnea syndrome.

Obstructive sleep apnea syndrome (OSAS) was diagnosed in157 subjects based on clinical symptoms, physical evaluation, cephalometric x-ray films, and polysomnography. These index cases identified 844 living first-degree relatives. Mailings were sent to 792 (94%). The mailing consisted of two identical questionnaires, one for the family member of the index case and one to be given to a friend (not a relative) of approximately the same age. In response, we received 531 (63%) questionnaires from relatives and 198 (25%) questionnaires from age-matched nonrelated friends, which were used as a contro…

Pharyngeal narrowing/occlusion during central sleep apnea.

We hypothesized that subatmospheric intraluminal pressure is not required for pharyngeal occlusion during sleep. Six normal subjects and six subjects with sleep apnea or hypopnea (SAH) were studied during non-rapid-eye-movement sleep. Pharyngeal patency was determined by using fiber-optic nasopharyngoscopy during spontaneous central sleep apnea (n = 4) and induced hypocapnic central apnea via nasal mechanical ventilation (n = 10). Complete pharyngeal occlusion occurred in 146 of 160 spontaneously occurring central apneas in patients with central sleep apnea syndrome. During induced hypocapnic …

Sleep apnea syndromes: overview and diagnostic approach.

The two broad categories of sleep apnea syndrome are associated with obstructive or mixed events on the one hand, and central events on the other. The pathogenesis of both seems to involve periodic reduction in respiratory drive, although obstructive apneas may also involve an anatomic abnormality of the upper airway. Patients with obstructive sleep apnea syndrome most commonly exhibit resuscitative snoring and daytime sleepiness. Snoring is generally less prominent in the central sleep apnea syndromes; those with daytime hypercapnia generally complain of daytime sleepiness, whereas those with…

Obstructive sleep apnea attack in complex craniosynostosis.

For treatment of patients with complex craniosynostosis, increased intracranial pressure, severe exophthalmos, and apnea attacks are important indications for operative intervention. Little has been reported on obstructive sleep apnea in complex craniosynostosis. Obstructive sleep apnea attacks in complex craniosynostosis are evaluated in relation to the upper and middle pharyngeal space and to the dimensions of the maxilla and mandible measured on cephalograms. The value of the pharyngeal air space in craniosynostosis had no relationship with either the frequency of sleep apnea attack or with…

Transpalatal advancement pharyngoplasty for obstructive sleep apnea.

Uvulopalatopharyngoplasty (UPPP) is a commonly performed procedure for obstructive sleep apnea (OSA). However, results are inconsistent. Patients in whom the UPPP procedure has failed have a smaller change in airway size as compared to responders, and also many demonstrate continued obstruction at the palate. We present a modification, transpalatal advancement pharyngoplasty, that increases upper oropharyngeal and retropalatal airway size by advancing the soft palate. Eleven patients with severe OSA and multiple sites of airway narrowing were corrected by this method. Three patients had prior …

[CT studies on obstructive sleep apnea syndrome].

To ascertain whether there are structural and functional changes in the upper airway of obstructive sleep apnea syndrome (OSAS) pts. We studied 33 pts of OSAS with positive polysomnographic tests, 14 pts of non-apnea snore with negative polysomnographic tests, and 18 normal subject by using CT scanner (Somatom Dr 3). The results are as followings 1. There were narrowing region in the upper airway in OSAS pts and the non-apnea snore pts. Most of the narrowings were located at the level of oropharynx, which was correspondent to the level of the soft palate and the uvula. The length of the soft p…

Laryngeal obstruction and obstructive sleep apnea syndrome.

Obstructive sleep apnea syndrome has been studied intensively since it was introduced in the 1970's; these studies have shown that the site of upper airway obstruction appears to vary among patients. Snoring is typically defined as the sound created by rhythmic oscillations of the soft palate in the inspiratory air stream during sleep. Snoring occurs in 100% of patients with obstructive sleep apnea and, for selected patients, is treatable by surgery. Snoring can, however, mean different things to different patients and physicians. Five atypical cases of obstructive sleep apnea syndrome are pre…

[Polysomnographic diagnosis of obstructive sleep apnea syndrome].

All night polysomnographic studies performed in 149 patients with OSA syndrome revealed marked disturbances of the sleep structure and the disorders of breathing during sleep: obstructive apneas in all patients, hypopneas in 25% and central apneas in 12%. The mean apnea index was 68 +/- 29, the mean percent of sleep time spent in apnea 50 +/- 21%. The typical obstructive apnea duration was 25 +/- 7s in NON REM sleep and 40 +/- 34s in REM sleep. The longest obstructive apnea duration was 48 +/- 24s in NON REM sleep and 71 +/- 42s in REM sleep. During obstructive apneas the typical arterial oxyg…

[Sleep apnea in well-controlled myasthenia gravis].

UNLABELLED: Polygraphic monitoring of the respiratory status during sleep provides a definitive test for the presence of the sleep apnea syndrome. This syndrome has been recognized in various kinds of neurological disorders. However, as far as we know, there have been no reports of describing the sleep apnea syndrome in myasthenia gravis (MG). We conducted overnight polygraphic sleep studies of 10 patients of clinically well-controlled MG. PATIENTS: We examined 10 patients of MG (4 men and 6 women). The subtypes of MG were IIA(1), IIB(8) and V(1) according to Osserman's criteria. RESULTS: Si…
