BACKGROUND: Torsion of an otherwise normal appendix vermiformis is exceedingly rare and usually presents with symptoms consistent with acute … x in the right lower quadrant. Upon laparoscopy, a torsed, necrotic appendix vermiformis was found. Appendectomy was performed, and the patient recovered uneventfully.
CONCLUSION: In an infant girl with lower abdominal pain, the differential diagnosis should include torsed appendix besides more common causes, such as torsed ovary, intussusception, or small bo …

BACKGROUND: Laparoscopic appendectomy (LA) has been an established treatment method for uncomplicated acute appendicitis. Controversy still exits regarding the superiority of either laparoscopic or open technique for the treatment of complicated appendicitis.
OBJECTIVE: To examine for benefits in postoperative morbidity comparing laparoscopic versus open appendectomy for complicated appendicitis.
METHODS: A retrospective analysis was performed using the American College of Surgeon's National Surgical Quality Improvement Project (ACS-NSQIP) dataset between 2005 and 2007. Inclusion criteria we…

Appendicitis is the most common surgical emergency of acute abdomen in children, principally at school age and teenagers. Patients are most often treated surgically to remove the appendix, either through a standard small incision in the right lower part of the abdomen or laparoscopically. The current is a retrospective study about the experience of a group of surgeons from the Fundación Hospitalaria--Buenos Aires Private Children Hospital who performed 40 laparoscopic appendectomies in patients under 18 years of age. The data registered and analysed belonged to patients with acute appendicitis

Insertion of abdominal drainage after appendectomy is controversial. We report on a case of duodenal perforation due to a silastic open drainage tube placed in the right para-colic gutter after appendectomy for perforated appendicitis. This case offers a clue against too liberal usage of abdominal drainage after appendectomy. Conservative management of bowel perforations secondary to abdominal drainages should be attempted in the absence of generalised peritonitis.

BACKGROUND: Inversion of the appendix is an alternative to incidental appendectomy to prevent future appendicitis. This study investigated outcome and complications in a group of patients who underwent simple inversion of the appendix.
PATIENTS AND METHODS: Inversion of the appendix was performed in 41 patients, including 21 women (51%) and 20 men (49%) (mean age, 48.7 years; range, 12-85 years). A simple inversion technique was used in 65% of patients. Twenty-nine patients had colonoscopy between 3 to 44 months after surgery (mean, 8 months), none for the sake of the study.
RESULTS: During …

Appendiceal intussusception (the inside-out appendix) was often practiced in pediatric surgery when the appendectomy was performed during laparotomy. Complications are exceptional. We report the first case of appendiceal intussusception in an adult appendectomized by this procedure during childhood, reviewing the radiological aspect, pathogenesis, and treatment. This delayed complication of appendiceal intussusception should be known to the surgeon. Treatment during colonoscopy is possible. This inflammation of the vaginated appendix can be prevented by coagulating the appendicular vessels to …

OBJECTIVE: Two common factors, cigarette smoking and appendectomy, have been found to play a role in ulcerative colitis (UC). Data on their role in the development of extraintestinal manifestations (EIM) are scarce.
METHODS: The relationship between cigarette smoking, appendectomy, and EIM was examined in a prospective study involving 535 (M/F = 319/216) consecutive UC patients followed up for 18 yr. We considered the major EIM: seronegative spondyloarthropathy, pyoderma gangrenosum/erythema nodosum, acute anterior uveitis, and primary sclerosing cholangitis. We excluded patients with a histo…

BACKGROUND/PURPOSE: The routine use of interval appendectomy for the treatment of perforated appendicitis, with or without abscess, remains controversial. The purpose of this study is to confirm the efficacy of this approach and to identify factors associated with failures and complications.
METHODS: All patients (n = 101) with their clinical diagnosis of perforated appendicitis confirmed with imaging were treated prospectively with fluids and intravenous antibiotics (clindamycin, ceftazidime) and were discharged home on oral fluids and analgesics regardless of fever. Intravenous antibiotics …

PURPOSE: To compare negative appendectomy and perforation rates in children who underwent ultrasonography (US), computed tomography (CT), or no imaging before urgent appendectomy.
MATERIALS AND METHODS: All children who underwent urgent appendectomy during a 4(1/2)-year period were identified in a surgical billing database. Pathology reports were coded as negative or as showing acute inflammation or perforation. Imaging up to 14 days before appendectomy or abscess drainage was noted, and imaging-based diagnoses were compared with pathologic findings. Patient age and sex were recorded.

OBJECTIVE: A review of the literature reveals contradictive conclusions regarding appendicitis during pregnancy due to the few patients included in previous reports.
METHODS: From 1980 to 1985, the Danish National Patient Registry identified the patients having the WHO codes Y60 (pregnancy), 54000 (acute appendicitis) and the operation code 43 000 (appendectomy). Each patient's file was reviewed by the authors.
RESULTS: The ratio of appendicitis (n = 58) to appendectomy (n = 117) was 50%. The appendicitis group was significantly older (26.2 years) than the normal appendix group (23.8 years).…

In this article, we present 80 noncomplicated acute appendicitis that we operated. The patients was separated into two groups. The first group, appendiceal stump was tied and dropped in abdominal cavity in 40 cases and second group, it was tied and inverted into the cecum in the other 40 cases. The cases were observed in the postoperative period concerning intraabdominal abscess, brid ileus, incisional wound infection and hospitalization duration. No significant difference has been found between these two groups.

Incidental appendectomy is a more and more frequent part of gynaecological laparotomies. Doubts about the increased post-operative morbidity and mortality have been shown to be unfounded in numerous series. Opinions on contra-indications to incidental appendectomy are divided. German language authors do not do incidental appendectomies with Caesarean Sections, operations for Ectopic pregnancies, Tubo-Ovarian Abscess and Inoperable Carcinoma. In the English speaking literature an increasing number of authors show that even under these circumstances the appendectomy is without danger. Incidental…

INTRODUCTION: Appendicitis and inguinal hernias are among the most common surgical pathologies in the pediatric population. Laparoscopic surgery allows for visualization of the myopectineal orifices leading to incidental diagnosis of patent processus vaginalis or inguinal hernia in up to 5.7% of appendectomies. Despite the prevalence of these incidental diagnoses, there is no consensus on their management during appendectomy.
METHODS: We performed a retrospective cohort study using the Nationwide Readmissions Database to identify all pediatric (≤18 y old) patients who underwent appendectomy f…

BACKGROUND: Endoscopic transcecal appendectomy (ETA) has gained increasing clinical adoption as a minimally invasive surgical approach. However, conventional appendiceal stump closure techniques present notable limitations due to their operational complexity. This technological constraint underscores the clinical need to develop optimized closure methodologies. In this study, we proposed an innovative "internal traction-assisted suspended closure (ITASC)" technique and conducted a multicenter retrospective comparative study to evaluate its clinical efficacy.
METHODS: Data from patients who un…

PURPOSE: Appendicitis is one of the most common emergency diseases. Currently, it is unclear whether there are differences in performing appendectomy during in-hour (IH) versus out-of-hour (OH). METHODS: A comprehensive search for studies that compared IH and OH was conducted in Web of Science, PubMed and EMBASE. Data of including studies were subjected to meta-analysis. The study was registered on PROSPERO (CRD42023467592). RESULT: The result indicated that the OH group had more complicated appendicitis (OH 25.1% vs. IH 24.5%, RD = 0.02; 95% CI, 0.00 to 0.05; p = 0.04, I2 = 72%), higher level…

Post-appendectomy intra-abdominal abscess (PAA) is a common and problematic complication in children with perforated appendicitis, contributing to prolonged hospitalization, readmissions, and increased healthcare costs. Despite advances in surgical and antimicrobial management, substantial heterogeneity persists in definitions, risk stratification, and treatment strategies. This narrative review aims to synthesize current evidence regarding the pathophysiology, risk factors, diagnostic pathways, clinical impact, and therapeutic approaches to PAA in the pediatric population. PAA occurs predomin…

Appendiceal mucinous neoplasms are rare entities that account for a small proportion of all gastrointestinal malignancies and are most often discovered incidentally on histopathological examination of appendectomy specimens. We present the case of a 36-year-old woman who presented to the emergency department with acute right lower quadrant abdominal pain and was found to have acute appendicitis on computed tomography imaging. She underwent an uncomplicated laparoscopic appendectomy, and final pathology revealed a low-grade appendiceal mucinous neoplasm (LAMN) confined to the appendix, staged a…

Post-appendectomy fecal fistula is a rare complication, but it is associated with significant morbidity. Etiological factors of post-appendectomy fecal fistula formation include severe peri-appendicitis involving the base of the appendix as well as adjoining caecal wall; neoplasia of the appendix and caecum; infective bowel conditions especially intestinal tuberculosis, actinomycosis and Crohn's disease; distal obstruction and gossypiboma. Diagnosis of post-appendectomy fecal fistula is usually made on CT-scan but Fistulography is also useful in the late stages of the disease. Treatment of pos…
