Environmental enrichment decreases asphyxia-induced neurobehavioral developmental delay in neonatal rats.

Perinatal asphyxia during delivery produces long-term disability and represents a major problem in neonatal and pediatric care. Numerous neuroprotective approaches have been described to decrease the effects of perinatal asphyxia. Enriched environment is a popular strategy to counteract nervous system injuries. The aim of the present study was to investigate whether enriched environment is able to decrease the asphyxia-induced neurobehavioral developmental delay in neonatal rats. Asphyxia was induced in ready-to-deliver mothers by removing the pups by caesarian section after 15 min of asphyxia

Evolution of the Thompson score during the first 6 h in infants with perinatal asphyxia.

AIM: This study aimed to determine the evolution of the Thompson score, which provides composite grading of encephalopathy signs, during the first 6 h of birth in neonates with perinatal asphyxia. METHODS: Twenty term infants with perinatal asphyxia were prospectively studied from the University Hospital of Kinshasa during a 12-month period. The Thompson score was performed after 1 h, then hourly until 6 h of birth. RESULTS: Fourteen infants had a Thompson score ≥7 and six had a score <7 after 1 h of birth. The Thompson score remained higher than 7 after 3 h in nine infants (64.3%) and in fo

Changing pattern of perinatal brain injury in term infants in recent years.

Perinatal brain injury in term infants remains a significant clinical problem. Recently a change appears to have occurred in the pattern of such injuries. We sought to characterize the incidence, etiology, clinical manifestations, and outcomes of these injuries. A retrospective chart review identified clinical characteristics of neuroimaging, electroencephalography, and placental pathologic findings. Perinatal depression was defined as hypotonia and the need for respiratory support. From January 2004-December 2009, 29,597 term deliveries occurred. Brain injuries in 33 infants (live term births

Care during labor and birth for the prevention of intrapartum-related neonatal deaths: a systematic review and Delphi estimation of mortality effect.

BACKGROUND: Our objective was to estimate the effect of various childbirth care packages on neonatal mortality due to intrapartum-related events ("birth asphyxia") in term babies for use in the Lives Saved Tool (LiST). METHODS: We conducted a systematic literature review to identify studies or reviews of childbirth care packages as defined by United Nations norms (basic and comprehensive emergency obstetric care, skilled care at birth). We also reviewed Traditional Birth Attendant (TBA) training. Data were abstracted into standard tables and quality assessed by adapted GRADE criteria. For int

Pharmacological neuroprotection after perinatal asphyxia.

Recent progress has provided us with several promising neuroprotective compounds to reduce perinatal hypoxic-ischemic (HI) brain injury. In the early post HI phase, therapies can be concentrated on ion channel blockage (Xenon), anti-oxidation (allopurinol, 2-iminobiotin, and indomethacin), anti-inflammation (erythropoietin [EPO], melatonin), and anti-apoptosis (nuclear factor kappa B [NF-κB]and c-jun N-terminal kinase [JNK] inhibitors); in the later phase, therapies should be targeted to promote neuronal regeneration by stimulation of neurotrophic properties of the neonatal brain (EPO,

Subcutaneous fat necrosis as a complication of whole-body cooling for birth asphyxia.

BACKGROUND: Subcutaneous fat necrosis (SCFN) of the newborn is a form of panniculitis that affects full-term neonates who often have suffered either birth asphyxia or hypothermia. The induction of hypothermia in newborns is becoming frequently used to reduce the neurologic sequelae associated with birth asphyxia. The risk of SCFN in neonates undergoing this therapy is unknown. Observation We describe a neonate who developed an abscess-like presentation of SCFN and subsequent asymptomatic hypercalcemia after undergoing whole-body cooling for hypoxic-ischemic encephalopathy. CONCLUSIONS: Hypoth

Optimizing chest compressions during delivery-room resuscitation.

There is a paucity of data to support the recommendations for cardiac compressions for the newly born. Techniques, compression to ventilation ratios, hand placement, and depth of compression guidelines are generally based on expert consensus, physiologic plausibility, and data from pediatric and adult models.

Oxidative stress in perinatal asphyxia.

Oxygen free radicals may cause tissue injury in perinatal asphyxia. We measured plasma and cerebrospinal fluid levels of malondialdehyde and plasma levels of glutathione peroxidase, catalase, and superoxide dismutase in 50 term newborns with perinatal asphyxia and eight newborns without asphyxia. Neonates with sepsis, major congenital malformations, and hemolytic disease were excluded. The levels of plasma and cerebrospinal fluid malondialdehyde, as well as of plasma glutathione peroxidase, catalase, and superoxide dismutase, were significantly higher in newborns with perinatal asphyxia, and d

Long-term pharmacologic neuroprotection after birth asphyxia: where do we stand?

Perinatal hypoxia-ischemia or birth asphyxia is a serious complication with a high mortality and morbidity. For decades, neuroprotective options have been explored to reduce reperfusion and reoxygenation injury to the brain, which accounts for a substantial part of birth asphyxia-related brain damage. In this review, we focus on neuroprotective strategies with a long-term follow-up, reported in both experimental and clinical studies. Strategies related to modification of excitatory neurotransmitter production and action, reduction in free radical production and inflammation and neoneurogenesis

Therapeutic hypothermia following perinatal asphyxia.

Well constructed and carefully analysed trials of hypothermic neural rescue therapy for infants with neonatal encephalopathy have recently been reported. The data suggest that either selective head cooling or total body cooling reduces the combined chance of death or disability after birth asphyxia. However, as there are still unanswered questions about these treatments, many may still feel that further data are needed before health care policy can be changed to make cooling the standard of care for all babies with suspected birth asphyxia.

Imaging of the brain in full-term neonates: does sonography still play a role?

To date the literature comparing the usefulness of US and MR examinations of the neonatal brain suggests that US is not as effective a modality as MR. However, available studies were done on older equipment and published descriptions of the abnormalities found in the term brain are often incomplete. The purpose of this article is to emphasize technical factors that may be useful to optimize US imaging of the term neonatal brain, to provide a description of the sonographic findings in the brain in full-term neonates with hypoxic-ischaemic injury and to provide some data regarding the accuracy o

Abnormal sagittal sinus blood flow in term infants following a perinatal hypoxic ischaemic insult.

Ultrasound is a useful tool for the study of the superior sagittal sinus in neonates. The normal patterns of blood flow have been established, and the technique has been used to diagnose sagittal sinus thrombosis. This report describes the sonographic diagnosis of abnormally sluggish or absent sagittal sinus flow in two term infants. The first was a 2,320-g female infant with postnatal hypoxia. The second was a 5,000-g male infant who developed hypoxic ischaemic encephalopathy after delivery complicated by shoulder dystocia. In both cases the normal pattern of blood flow in the superior sagitt

Recovery of metabolic acidosis in term infants with postasphyxial hypoxic-ischemic encephalopathy.

AIMS: To describe the base deficit (BD) values and the rate of postnatal recovery of the BD of infants with hypoxic ischemic encephalopathy due to intrapartum asphyxia; and to explore the relationships between the rate of recovery of BD, severe adverse outcome and different time patterns (acute total vs prolonged partial) of asphyxia. METHODS: Clinical and laboratory data were collected from the neonatal period (n = 244) and outcome data to a minimal age of 1 y (n = 218). Rates of recovery of BD were described in four 60 min blocks of time. The values of rate of recovery were compared between

Neonatal magnetoencephalography and spectral analysis.

PURPOSE: We performed a prospective study of neonatal brain function on 44 respective term neonates who were delivered normally, without any clinical signs of brain damage. Thirty were associated with a normal pregnancy and labor with normal values of Apgar scores, umbilical cord pH and birth weight, while 14 neonates were the result of pre-eclamptic pregnancy. METHODS-RESULTS: Biomagnetic measurements were performed by means of a Superconducting Quantum Interference Device (SQUID) in an electrically shielded room of low magnetic noise. Biomagnetic signals (waveforms) recorded from neonatal b

Is temperature important in delivery room resuscitation?

The possibility that temperature may affect the outcome of resuscitation from severe perinatal asphyxia has been a long-standing focus of research. Experimentally it is now well established that even small changes in temperature during severe hypoxia-ischemia critically modulate outcome. Clinical and experimental studies have now shown that hypoxic-ischemic injury continues to evolve after resuscitation. Experimentally, prolonged mild to moderate hypothermia can dramatically reduce this delayed injury, while mild hyperthermia over the same period worsens injury. Indeed there are data indicatin

Perinatal asphyxia at Port Moresby General Hospital: a study of incidence, risk factors and outcome.

We investigated the incidence and outcome of perinatal asphyxia (PA) at Port Moresby General Hospital by a retrospective chart review and prospective collection of data, spanning a total of 2.5 years. 125 babies weighing more than 2000 g at birth with a gestation of 34 weeks or more and with no obvious congenital abnormalities were diagnosed to have PA. During the same time period 22,700 liveborn babies were delivered, a PA incidence of 5.5/1000 livebirths. There was a 31% mortality and considerable morbidity. Hospital records for 114 affected babies and 115 controls (the next baby born by nor

Neonatal morbidity in a hospital at Shimla.

During the study period there were 2063 live births. Of these 573 (27.8%) were low birth weight (LBW), 277 (13.4%) preterm and 148 (7.1%) small for date (SFD) babies. In all, 263 (12.7%) newborns suffered from one or the other morbidity. Birth asphyxia of varying severity developed in 130 (6.3%) babies [88 LBW and 42 normal birth weight (NBW) (p < 0.001)]. Respiratory distress syndrome was diagnosed in 82 (3.9%) babies, most being due to hyaline membrane diseases (31.7%), which affected 26 (9.4%) of preterm babies. Deep infections were seen in 109 (5.3%) newborns [60 LBW and 49 NBW, (p < 0.001
