Association between hemoglobin scavenger receptor and heme oxygenase-1-related anti-inflammatory mediators in human coronary stable and unstable plaques.

Heme oxygenase-1 (HO-1) is a cytoprotective enzyme that is induced by intraplaque hemorrhage and degrades free heme and releases ferrous iron, which is rapidly sequestered by ferritin. In vitro studies have shown that binding of hemoglobin to hemoglobin scavenger receptor (CD163) induces HO-1 and the anti-inflammatory mediator interleukin (IL)-10. We immunohistochemically examined the relationship between CD163 expression in macrophages and intraplaque hemorrhage, HO-1, IL-10, and ferritin using coronary atherectomy specimens from patients with stable (SAP) or unstable angina pectoris (UAP). A…

Immunolocalization of platelet glycoprotein IIb/IIIa and P-selectin, and neutrophil-platelet interaction in human coronary unstable plaques.

Platelet aggregation at the site of plaque rupture or erosion is a dominant feature in the pathophysiology of plaque destabilization. To elucidate the role of glycoprotein (GP) IIb/IIIa in coronary plaque destabilization, we immunohistochemically studied the presence of GP IIb/IIIa in coronary atherectomy specimens obtained from patients with stable angina (SAP) and unstable angina pectoris (UAP). Moreover, we immunohistochemically investigated the presence of P-selectin, which is known to be a marker of platelet activation, in these specimens. All these patients underwent atherectomy at prima…

Implications of small reference vessel diameter in patients undergoing percutaneous coronary revascularization.

OBJECTIVES: The purpose of this study was to determine whether small reference diameter of the culprit coronary artery influences the outcom … f interventional devices differed markedly. In contrast to stents (18.5% vs. 41.9%) and directional atherectomy (3.7% vs. 13.5%), conventional balloon angioplasty (73% vs. 50%) and rotational atherectomy (16.1% vs. 8.3%) were used more often in smaller vessels (p < or = 0.0001, each). Success rate was lower in the small vessel group (92% vs. 95%; p = 0.006). …

Optimizing Stent Expansion in Calcified Coronary Lesions: A Case Series on the Efficacy of Scoring Balloons for Acute Underexpansion.

Stent underexpansion remains a critical challenge in the treatment of calcified coronary lesions through percutaneous coronary intervention (PCI). It is a significant risk factor for in-stent restenosis, stent thrombosis, and adverse clinical outcomes. Although methods such as atherectomy, intravascular lithotripsy, and scoring balloons are used in the preparation period before stent placement, there is a paucity studies on their use in acute stent underexpansion. In this context, the use of scoring balloons for post-dilatation of acutely underexpanded stents offers a promising, cost-effective…

Transient rise in His-lead threshold due to acute myocardial infarction.

An 85-year-old male was admitted to our center with a non-ST elevation myocardial infarction. The patient had a dual-chamber pacemaker in situ with an atrial and His lead. A transient increase in His threshold and loss of nonselective capture occurred at the presentation of right coronary artery infarction, peaking during rotational atherectomy therapy causing loss of capture and complete atrioventricular block. A follow-up interrogation, 2 weeks postrevascularization, showed a return to a normal nonselective capture morphology and threshold measurements. Physicians should be aware of this com…

A State-of-the-Art Review: The Percutaneous Treatment of Highly Calcified Lesions.

Coronary artery calcification is prevalent in coronary heart disease with its progression being predictive of future adverse cardiac events. Its presence is considered to be a marker of interventional procedural complexity. Several adjunctive percutaneous coronary intervention tools, such as modifying balloons, atherectomy devices and intravascular lithotripsy, now exist to successfully treat calcified lesions. In this state-of-the-art review, a step-wise progression of strategies is described to modify coronary plaque, from well-recognised techniques to techniques that should only be consider…

Impella-Assisted Intracoronary Lithotripsy of Severely Calcified Left Main Coronary Artery Bifurcation for NSTEMI With Cardiogenic Shock.

High calcification of coronary artery plaque is a frequent cause of suboptimal stent expansion, which can result in stent thrombosis and restenosis. Shockwave intravascular lithotripsy (S-IVL) represents a new frontier in the treatment of highly calcified coronary lesions. It can be an excellent alternative to intracoronary atherectomy in extremely high-risk lesions. We present a case of a 57-year-old man with known severe coronary artery disease (CAD) who presented with non-ST segment elevation myocardial infarction (NSTEMI), cardiogenic shock and was successfully treated with impella-assiste…

Are Coronary Calcium-Modifying Techniques Levelling the Playfield?

Patients with heavily calcified coronary arteries represent a challenge in percutaneous coronary intervention (PCI), as severe calcification … junctive devices such as cutting and scoring balloons improve plaque modification in focal disease. Atherectomy techniques, including rotational and orbital systems, are effective for more complex lesions but require technical expertise and carry procedural risks. Intravascular lithotripsy has emerged as a promising, less aggressive modality capable of fractu …

Rotastenting in an anomalously arising right coronary artery after an ugly dissection.

Rotational atherectomy is contraindicated in dissected coronary arteries since it can lead to progression of the dissection or perforation. In our case, the right coronary artery (RCA) arose anomalously from the left coronary sinus. The lesion in the RCA was an undilatable calcified one. There was a dissection in the RCA due to high pressure balloon dilatation. Since the patient was hemodynamically unstable and there were no options besides rotablation, we proceeded with caution. Rotablation and stenting were successfully done. Our case report highlights the importance of the need for good gui…

The parallel wire technique to deal with wire-induced dissection of a complex calcified lesion.

Coronary dissection is not an uncommon phenomenon during coronary angioplasty. Coronary dissection can occur due to a variety of interventions, including balloon inflation, atherectomy, stent edge dissections, guide catheter trauma and wire-induced dissections, to name a few. Once a severe dissection has occurred, it is often challenging to find the true lumen. The parallel wire technique has been described in the literature for the recanalization of chronic total occlusions and also to recanalize guide catheter-induced spiral dissections of the right coronary artery. We report a novel case of…

Percutaneous transluminal angioplasty of coronary arteries with anomalous origin.

Coronary angioplasty to diseased anomalous coronary arteries is being more readily applied in revascularization of these vessels. Several cases of angioplasty have been described in various coronary anomalies. Probably more attempts of PTCA to anomalous coronary arteries are performed but not reported. For this reason, we cannot assume that the success rate is the same as in "normal" origin arteries or which complications occurred, including the need for coronary bypass. Although not yet reported, in some instances other techniques, such as laser atherectomy and coronary stents, may be applied…

Management of Coronary Artery Perforation.

Coronary artery perforation (CAP) is a rare but potentially life-threatening complication of percutaneous coronary intervention (PCI), however if recognized and managed promptly, its adverse consequences can be minimized. Risk factors for CAP include the use of advanced PCI technique (such as atherectomy and chronic total occlusion interventions) and treatment of severely calcified lesions. There are 3 major types of CAP depending on location: (a) large vessel perforation, (b) distal vessel perforation, and (c) collateral perforation. Large vessel perforation is usually treated with implantati…

Combination therapy for complex peripheral artery disease: the evidence for combining mechanical revascularization with anti-restenotic drug delivery systems.

The management of complex, lower-extremity peripheral artery disease remains a difficult challenge, with ample room for improvement in the results seen with current generation devices. Therapies which use a combination of multiple devices for their mechanical and anti-restenotic properties may be advantageous, and offer an opportunity to improve the durability of revascularization through optimizing and individualizing care. Combination therapy typically utilizes drug delivery devices in addition to stents and atherectomy to achieve improved results. However, there is a paucity of high-level e…

Histological evaluation of coronary plaque in patients with variant angina: relationship between vasospasm and neointimal hyperplasia in primary coronary lesions.

OBJECTIVES: This study was designed to determine whether coronary vasospasm in patients with variant angina pectoris (VAP) may produce focal organic lesions at the site of vasospasm that would contribute to disease progression. BACKGROUND: Recent clinical angiographic and experimental studies have demonstrated the potential role of vasospasm in the worsening of organic coronary stenosis. METHODS: We studied histologically the coronary plaques obtained at atherectomy in 202 patients with moderate to severe coronary stenosis. This population included 22 patients with VAP, 100 patients with chr…
