Mediastinal lymphoma with complete atrioventricular block in a horse.

A 15-year-old, neutered-male pony presented with a history of weight loss during 4 months. Clinical evaluation revealed severe bradycardia and complete atrioventricular block. At necropsy, a lobulated mass in the anterior mediastinum and moderate enlargement of the superficial cervical lymph nodes were observed. The vagus nerve and the brachiocephalic trunk were embedded in this anterior mediastinal tumor. Histologically, the mass was composed of sheets of neoplastic lymphoid cells expressing CD3, with a low mitotic rate. To the best of our knowledge, this is the first reported case of mediast

Dual Atrioventricular Nodal Non-Re-Entrant Tachycardia.

A patient presented with symptoms of palpitations. Her standard 12-lead electrocardiogram captured 3 potential causes of her symptoms (premature atrial contractions, junctional rhythm, and narrow complex tachycardia). Further workup uncovered dual atrioventricular node physiology with 1:2 sinus conduction and resultant alternating QRS from a slow and fast conduction pathway. (Level of Difficulty: Intermediate.).

Bradyarrhythmias and conduction blocks.

Bradyarrhythmias are a common clinical finding and comprise a number of rhythm disorders including sinus node dysfunction and atrioventricular conduction disturbances. Clinical presentation varies from asymptomatic electrocardiogram findings (eg, during a routine examination) to a wide range of symptoms such as heart failure symptoms, near syncope or syncope, central nervous symptoms, or nonspecific and chronic symptoms such as dizziness or fatigue. Conditions resulting in bradyarrhythmic disorders are divided into intrinsic and extrinsic conditions causing damage to the conduction system. Fur

[The nonpharmacological treatment of tachyarrhythmias. The surgery of supraventricular arrhythmias not due to pre-excitation].

Surgery can provide treatment for supraventricular tachyarrhythmias without operative risk and with a definite improvement of patients' quality of life. Ectopic atrial tachycardia is a rare but invalidating arrhythmia that may lead to cardiomyopathy: intraoperative mapping is necessary for the location of the location of the ectopic focus; in our experience, surgical, cryothermal or isolation ablation of the area were effective in 9 patients out of 9, with regression of the cardiomyopathy. Atrioventricular node reentry tachycardia is a reentry tachycardia which is often associated with Wolff-P…

Predominant myocardial sarcoidosis.

Unexpected autopsy findings of a predominantly myocardial sarcoidosis are reported. For a period of two years prior to death, the 42-year-old man suffered from atypical angina pectoris and ventricular arrythmias with episodic ventricular fibrillation. Echocardiography revealed a non-obstructive hypertrophic cardiomyopathia. He succumbed to sudden cardiac arrest. Sarcoidosis granulomas were widespread in the ventricular and atrial myocardium with intensive involvement of the sinus node, atrioventricular node, and the main bundle of His. According to a review of the previous literature on this s…

Recovery of the conduction system from ischemic arrest and cardioplegia in perfused rat heart.

Normothermic ischemic arrest of the perfused rat heart for 1 h was followed by severe reperfusion arrhythmias, depressed excitability, atrioventricular (AV) node and sinoatrial (SA) node conduction blocks and by marked impairment of contractility. Excitability and conduction recovered within 10 to 15 mins whereas contractility remained greatly depressed. This finding is in agreement with a theoretically based assumption of lesser sensitivity of conduction system to ischemia compared to the working myocardium. When the hearts were exposed to ischemia at 20 degrees C, the sinus rhythm recovered

[Clinical safety and efficacy of single-chamber atrial pacing in sick sinus syndrome: long-term follow-up].

INTRODUCTION AND OBJECTIVES: Although atrial pacing is a more physiological mode of stimulation in sinus node dysfunction, the pacing modes most often are used DDD and VVI. The aim of our study was to demonstrate that AAI/AAIR pacing is effective and safe by analyzing the complications and mortality of this pacing mode in a long-term follow-up study. PATIENTS AND METHOD: Between 1982 and 2000 definitive AAI-mode pacemakers were implanted for sinus node dysfunction in mode AAI in 160 patients. We analyzed the clinical characteristics, evolution, and complications of the AAI pacing mode during

Fasciculoventricular pathways-A rare and innocent variant: A Retrospective study focusing on clinical and electrophysiologic characteristics.

BACKGROUND: Fasciculoventricular pathways (FVPs) are variants of pre-excitation syndrome which were investigated insufficiently because of its rarity. OBJECTIVE: This report aimed to represent one of the largest series of FVP, focusing on its clinical and electrophysiological properties. METHODS: We analyzed retrospectively 26 consecutive patients who underwent electrophysiological study (EPS) for FVP between January 1998 and June 2020. RESULTS: Among 1437 patients with accessory pathways, 26 had FVP (1.80%). All the 26 patients (100%) were males, with a mean age of 22.15 ± 3.50 years (rang

Multiple Shifts of the Earliest Retrograde Atrial Activation Site Along the Tricuspid Annulus During the Fast-Slow Form of Atrioventricular Nodal Reentrant Tachycardia by Radiofrequency Modification.

A 70-year-old woman was admitted for treatment of supraventricular tachycardia. Ventriculoatrial conduction was revealed through programmed ventricular stimulation; the coronary sinus ostium (CSos) was the earliest atrial activation site. The fast-slow forms of atrioventricular nodal reentrant tachycardia (AVNRT) were induced by ventricular extra-stimuli. During tachycardia, the earliest atrial activation site was located at the bottom of CSos. Radiofrequency (RF) energy application to this site resulted in the delay of local electrical potential, prolongation of tachycardia cycle length, and

[Broad QRS complex tachykardia - what is the mechanizm?].

We present the ECG with broad QRS complex tachycardia and left bundle branch block (LBBB) morphology. There are negative P waves in inferior leads (II, III, aVF) and positive in V1 with the RP interval of 124-164 ms. The slow-slow atrioventricular nodal reentrant tachycardia (AVNRT) conducted with LBBB aberrancy was diagnosed. The differential diagnosis of slow-slow AVNRT from atrioventricular reentrant tachycardia using concealed posteroseptal accessory pathway is discussed.

Intermittent rate-dependent retrograde conduction over a concealed atrioventricular accessory pathway: what is the mechanism?

Intermittent retrograde conduction over an accessory pathway (AP) is quite an uncommon phenomenon. We describe the case of a 60-year-old woman with recurrent episodes of palpitations. Atrioventricular re-entry tachycardia was diagnosed at electrophysiological study. Interestingly, retrograde conduction over the AP was detectable only when ventricular pacing was performed at fast rate. Supernormal retrograde conduction over the AP was considered the most likely explanation of this unusual finding.

[Slow and fast AV nodal pathways in tachycardia complicating Wolff-Parkinson-White syndrome: report of a case].

Electrophysiologic studies in a patient with intermittent ventricular pre-excitation revealed several types of paroxysmal narrow-QRS tachycardia (PSVT). One type of PSVT was characterized by normal retrograde atrial sequences with P waves occurring simultaneously with QRS. This type of PSVT reflected AV nodal reentry with anterograde slow pathway and retrograde fast pathway conduction. A second PSVT reflected alternation of anterograde fast and slow AV nodal pathway conduction and retrograde anomalous pathway conduction. A third PSVT reflected anterograde slow AV nodal pathway and retrograde a

De subitaneis mortibus. XI. Young girl with palpitations.

A seventeen-year-old girl in apparent good health was found dead in bed one morning. Two clinical features of note were multiple premature beats present in her electrocardiograms for several years, and a tall thin habitus without stigmata of Marfan's syndrome. At necropsy examination the heart was grossly normal. At its margin abutting the central fibrous body the atrioventricular (A-V) node exhibited numerous fronds and outcroppings, some forming loop connections from one part of A-V node to another. Groups of A-V nodal cells detached from the A-V node were also found connected to the crest o

Surgery for cardiac arrhythmias.

From March 1989 to March 1991, 17 patients underwent curative surgical ablation of cardiac arrhythmias at the Singapore General Hospital. In the supraventricular tachycardia group, 13 patients with the Wolff-Parkinson White syndrome (WPW) underwent surgical ablation of the accessory conduction pathway by the classical endocardial approach. Two patients who had atrioventricular nodal tachycardia (AVNRT) underwent surgical dissection around the atrioventricular node to divide one of the dual conduction pathway responsible for the tachycardia. In the ventricular tachycardia group, two patients un…

Is Left Bundle Branch Pacing Feasible in Patients With Ventricular Septal Defect?

His bundle typically passes through the central fibrous body of the atrioventricular node and then enters the membranous portion of the interventricular septum, where it branches into the left and right bundle branches. The feasibility and safety of left bundle branch area pacing (LBBAP) in patients with perimembranous ventricular septal defect (pmVSD) have not been reported.

Coexistence of sinus bradycardia and junctional tachycardia in a patient with fibrotic atrial cardiomyopathy.

A 65-year-old woman was referred for catheter ablation in the treatment of persistent tachycardia after surgery for atrial fibrillation and mitral regurgitation. Bipolar voltage mapping of both atria revealed that severe and extensive atrial fibrosis isolated the sinoatrial node from the atrioventricular junction and led to the coexistence of sinus bradycardia and persistent junctional tachycardia.

Environmental Fe, Ti, Al, Cu, Hg, Bi, and Si Nanoparticles in the Atrioventricular Conduction Axis and the Associated Ultrastructural Damage in Young Urbanites: Cardiac Arrhythmias Caused by Anthropogenic, Industrial, E-Waste, and Indoor Nanoparticles.

Air pollution exposure is a risk factor for arrhythmia. The atrioventricular (AV) conduction axis is key for the passage of electrical signals to ventricles. We investigated whether environmental nanoparticles (NPs) reach the AV axis and whether they are associated with ultrastructural cell damage. Here, we demonstrate the detection of the shape, size, and composition of NPs by transmission electron microscopy (TEM) and energy-dispersive X-ray spectrometry (EDX) in 10 subjects from Metropolitan Mexico City (MMC) with a mean age of 25.3 ± 5.9 and a 71-year-old subject without cardiac pathology.
