Evidence for sympathetic neural influence on human corneal epithelial function.

Patients with unilateral Horner's syndrome were studied to determine the influence of sympathetic innervation on the structure and function of the human cornea. There was no difference in total corneal thickness, corneal oxygen uptake rate or corneal touch threshold between the normal and Horner's eyes of these patients. Epithelial thickness, however, was slightly but significantly greater in the Horner's eye than in the normal fellow eye (P less than 0.025). When subjected to a hypoxic "Stress Test", patients with post-ganglionic lesions showed significantly more epithelial greying and microc

Influence of sympathetic nerve activity on renal haemodynamics and release of renin. A preliminary communication.

The role of the sympathetic nerve system is still unknown in different types of hypertension. The procedure used in the evaluation of suspect renovascular hypertension has been revised in such a way that renal blood flow, renal arterial blood pressure and plasma renin activity in renal vein could be determined prior to and after splanchnic blockade. A new divice-videovolumeter-has been used to investigate the blood flow. Some preliminary data are given.

The Stellate Ganglion Block for PTSD: A Retrospective Clinical Case Series.

BACKGROUND/OBJECTIVES: Extant data suggest that the Stellate Ganglion Block (SGB) is effective for posttraumatic stress disorder (PTSD). Clinical data from a large healthcare system are lacking. We report data from a clinical project in the Veterans Health Administration. METHODS: Retrospective data of PTSD and anxiety for 579 patients who received one or more SGBs were analyzed on the full sample and on those who had complete data using general linear models. RESULTS: Receiving the first SGB provided a 36% and 30% reduction in PTSD symptom scores at 1-week and 1-month post-SGB, respectively

Discriminative role of inflammatory indices in clinical response to ganglion impar blockade: A retrospective cohort study.

Coccydynia is a chronic pain syndrome with a complex pathophysiology. While ganglion impar blockade (GIB) is an effective treatment, predictors of response are not well-established. This study aimed to evaluate whether hematologic inflammatory indices or coccygeal morphology could predict the clinical response to GIB in patients with chronic coccydynia. We retrospectively reviewed the records of 119 patients who underwent GIB. Demographics, pain characteristics, coccygeal morphology (Postacchini-Massobrio classification), and preprocedural indices were extracted from the medical records. The p

Effect of Right Stellate Ganglion Block on Preventing Postoperative Nausea and Vomiting in Gynecological Laparoscopic Patients: A Randomized Controlled Trial.

INTRODUCTION: Postoperative nausea and vomiting (PONV) is one of the most common postoperative complications, with particularly high rates in patients undergoing high-risk surgeries such as gynecologic laparoscopy. Although there are many pharmacological and non-pharmacological methods that can prevent PONV, the incidence remains high. This study assessed the effectiveness of a right stellate ganglion block (SGB) in preventing PONV in gynecological laparoscopy patients, while also exploring the potential mechanisms involved. METHODS: Two hundred patients were randomly assigned to either a rig

The 90% minimum effective volume and concentration of lidocaine for ultrasound-guided stellate ganglion blocks in adults: a biased-coin design, up-and-down sequential allocation trial.

BACKGROUND: As ultrasound imaging technology matured, stellate ganglion blocks (SGBs) have become increasingly precise and safe, and their use in clinical practice has become widespread. However, the 90% minimum effective volume (MEV90) and concentration (MEC90) of lidocaine for ultrasound-guided SGB remain unclear. We aimed to determine the MEV90 and MEC90 of lidocaine used in ultrasound-guided SGBs. METHODS: Patients with indications for an SGB were recruited, without restrictions on sex or disease type. In this two-part study, we first determined the MEV90, and then used these results to d

Lumbar Sympathetic Nerve Modulation Using Absolute Ethanol for the Treatment of Primary Lower-Extremity Hyperhidrosis: A Dose-Effect Pilot Study.

BACKGROUND Primary lower-extremity hyperhidrosis (PLEH) can be treated by CT-guided lumbar sympathetic nerve modulation using absolute ethanol. However, doses of ethanol that are too high can cause nerve injury, and doses that are too low have suboptimal results. The present study aimed to investigate the dose-effect relationship of CT-guided lumbar sympathetic nerve modulation with absolute ethanol for PLEH. MATERIAL AND METHODS The study was conducted at the First Affiliated Hospital of Jiaxing University between 07/2014 and 02/2017. Twenty participants were enrolled in each group. The doses

Ultrasound-guided percutaneous peripheral nerve stimulation for analgesia following total knee arthroplasty: a prospective feasibility study.

BACKGROUND: Peripheral nerve stimulation has been used for decades to treat chronic pain but has not been used for postoperative analgesia due to multiple limitations, beginning with invasive electrode placement. With the development of small-diameter/gauge leads enabling percutaneous insertion, ultrasound guidance for accurate introduction, and stimulators small enough to be adhered to the skin, neurostimulation may now be provided in a similar manner to continuous peripheral nerve blocks. Here, we report on the use of ultrasound-guided percutaneous peripheral nerve stimulation to treat posto

Ultrasound-guided stellate ganglion block: safety and efficacy.

Cervical sympathetic and stellate ganglion blocks (SGB) provide a valuable diagnostic and therapeutic benefit to sympathetically maintained pain syndromes in the head, neck, and upper extremity. With the ongoing efforts to improve the safety of the procedure, the techniques for SGB have evolved over time, from the use of the standard blind technique, to fluoroscopy, and recently to the ultrasound (US)-guided approach. Over the past few years, there has been a growing interest in the ultrasound-guided technique and the many advantages that it might offer. Fluoroscopy is a reliable method for id

An unusual cause of S1 radicular pain presenting as early phantom pain in a transfemoral amputee: a case report.

Recent epidemiologic studies have shown back pain to be a significant cause of pain in lower-limb amputees, but only a handful of cases have reported sciatica in amputees. The symptoms are usually described as a phantom pain or neuropathic pain in the residual limb that is often refractory to conventional treatments. These symptoms typically occur with back pain and are distinct from the patient's usual symptoms. Interestingly, back pain is not a universal finding. We present a patient with presumed phantom limb pain subsequently discovered to be caused by an S1 radiculopathy. This patient's s

[Meaning of stylopharyngeal fascia in intraoral block techniques].

BACKGROUND: The almost unknown stylopharyngeal fascia may be one of the reasons for unsuccessful therapy of the trigeminal neuralgia by a transoral block of the superior ganglion of the sympathetic trunk. We investigated the anatomy of the fascia to show the efficiency of the block for this therapy. MATERIALS AND METHODS: 103 halves of human heads were investigated. The stylopharyngeal fascia was dissected by a lateral approach. We classified three groups. Group A was formed by fascias without perforation, group B by perforated fascias and group C by all halves without a developed fascia. RE

[Influence of cervical sympathetic nerve block on blood flow volume and barrier function of intestinal mucosa after combined radiation and burn injury in rat].

OBJECTIVE: To investigate the influence of cervical sympathetic nerve block (SB) on blood flow volume and barrier function of intestinal mucosa after combined radiation and burn injury in rat. METHODS: SD rats were divided into three groups: control (n = 18), combined injury group (n = 100, rats with Co gamma ray body irradiation with a dose of 5 Gy plus 15% TBSA full-thickness burn injury), and combined injury with SB treatment (n = 100, with the same dose of gamma-ray irradiation and burn injury, treated with SB). Twenty rats were sacrificed on 0, 1, 5, 7 days after combined injuries for va

[Peripheral nerve block as a component of anesthesia during operations on lower extremity vessels].

Lumbar plexus block via inguinal paravascular access that was included as part of an anesthetic support at venectomy in the great saphenous vein, was studied for its impact on the development of postoperative pain syndrome and stress responses. The authors revealed a significant postoperative pain relief at rest and in motion, and reductions in the need for analgesics and in the magnitude of a stress response to a surgical injury.

Autonomic responses to single- and dual-chamber pacing.

We investigated the autonomic effects of short-term, single- and dual-chamber pacing by evaluating frequency-domain indexes of heart rate variability (HRV). The study group comprised 25 patients (mean age 62 +/- 7 years) without organic heart disease and with normal sinus node function who were implanted with a permanent dual-chamber DDD (n = 16) or VDD (n = 9) pacing system for transient high-degree atrioventricular block. Continuous overdrive pacing for 15 minutes slightly above the intrinsic rhythm was programmed to ensure complete capture in AAI, DDD, and VVI modes, and the atrioventricula

Complete but transient visual loss after yttrium aluminum garnet laser transscleral cyclocoagulation.

We report the case of a patient who had no light perception after neodymium:yttrium aluminum garnet (Nd:YAG) laser transscleral cyclocoagulation. To the best of our knowledge, no similar cases have been reported previously. A 56-year-old man with intractable open-angle glaucoma had intraocular pressures of 42 mmHg OD and 35mmHg OS. The patient rejected a recommended filtering operation, although argon laser trabeculoplasty and maximal medication had been ineffective OU. We used the free-running mode of an Nd:YAG laser to treat this patient. After the tenth application, the patient suddenly not

Psychophysical observations on patients with neuropathic pain relieved by a sympathetic block.

Patients with sympathetically maintained pain (SMP) were tested with noxious heat pulses, innocuous mechanical stimuli, and transcutaneous electrical nerve stimulation before and during local anesthetic sympathetic blocks that relieved their pain. The perceived intensity of the pain evoked by these stimuli was measured by the patients' responses on a visual analog scale and compared to the responses obtained when the same stimuli were applied to contralateral normal skin. In 5 of 7 patients tested, graded noxious heat stimuli (43-51 degrees C) applied to painful skin resulted in heat-pain inte

[Ophthalmic herpes: treatment of herpetic neuralgia by repeated stellate block].

Eight cases of herpes zoster with severe neuralgic pain were treated by repeated stellate blocks, 5 during the acute stage and the 3 others later on. Of the 5 patients treated during the acute stage 3 were cured, the pain disappearing completely. Four other patients were substantially improved. Only one case was a failure. It is important to begin the injections soon after the beginning of the illness, preferably in the first couple of weeks after the rash appears.
