BACKGROUND: Traumatic injuries to the extrahepatic biliary tree are extremely rare, accounting for less than 0.5% of abdominal traumas. They are often associated with severe concomitant vascular and hepatic lesions, and their diagnosis may be delayed due to overshadowing hemorrhagic injuries. Management strategies depend on the patient's hemodynamic status, ranging from primary repair in stable patients to staged damage-control procedures in unstable situations.
CASE PRESENTATION: We report the case of a 29-year-old male with no medical history who presented with a thoraco-abdominal stab woun

AIM: To evaluate the technical feasibility of a modified tapered metal tip and low profile introducer for one-step endoscopic ultrasound (EUS)-guided biliary drainage (EUS-BD) in a new experimental biliary dilatation porcine model.
METHODS: A novel dedicated device for one-step EUS-guided biliary drainage system (DEUS) introducer has size 3F tapered catheter with size 4F metal tip for simple puncture of the intestinal wall and liver parenchyma without graded dilation. A self-expandable metal stent, consisting of both uncovered and nitinol-covered portions, was preloaded into DEUS introducer.

BACKGROUND: Bouveret syndrome, a rare complication of cholelithiasis, is nonspecific symptoms and high mortality rates, with no established standard treatment. Here, we detail a case of effective endoscopic intervention in an elderly patient.
CASE PRESENTATION: A 91-year-old female patient who presented with a one-month history of nausea, vomiting, and significant abdominal distension. An abdominal CT scan revealed a biliary-enteric fistula, pneumobilia, gallstones, a calcified mass in the horizontal portion of the duodenum, and gastric dilatation. Endoscopic examination confirmed the presenc

The clinical, diagnostic, and therapeutic aspects of 27 patients with pancreatopleural fistula (PPF) reported in the literature, and two additional patients managed by the authors, form the basis of this report. The diagnosis of PPF is based on the triad of massive pleural effusions, elevated pleural fluid amylase, and protein levels. PPF is a complication of chronic fibrocalcific pancreatitis in most cases. It may develop as a consequence of disruption of a dilated obstructed pancreatic duct. Pseudocysts are involved in the process in at least half of reported cases. A substantial number of P

BACKGROUND: Xanthogranulomatous cholecystitis, a rare variant of cholecystitis, may infrequently be complicated by spontaneous cholecystocutaneous fistula.
CASE PRESENTATION: We report the case of a 75-year-old Saudi Arabian man who presented with "a painful area of redness" (cellulitis) over his right upper abdomen. Abdominal computed tomography revealed multiple collections, which were drained surgically. A discharging sinus was identified, and a fistulogram revealed cholecystocutaneous fistula during his follow-up visit. The patient underwent laparoscopic management and recovered uneventfu

Bronchobiliary fistulas (BBFs), defined as abnormal connections between a bronchus and the bile duct, are rare. Bronchial occlusion with silicon spigots under radiographic guidance is a good alternative means of treating a BBF when surgical intervention is considered too risky. A 60-year-old man had a diagnosis of intrahepatic cholangiocarcinoma and underwent chemotherapy. Obstructive jaundice developed and was treated with percutaneous transhepatic cholangiography drainage (PTCD). Subsequent bronchoscopy with contrast medium through the PTCD tube enabled identification of a BBF and the respon…

RATIONALE: Perioperative management of patients with bronchobiliary fistula (BBF) is an anesthetic challenge because they typically exhibit poor lung function preoperatively, require meticulous lung isolation intraoperatively and need postoperative respiratory support.
PATIENT CONCERNS: A 44-year-old man with a past surgical history of pancreaticoduodenectomy presented fluctuating fever, jaundice, dyspnea and yellowish sputum. Despite intravenous antibiotic treatment and repeated percutaneous drainage, patient showed gradual deterioration with hypoxemia, and uncontrolled pneumonia.

INTRODUCTION: The biliobronchial fistula of hydatic origin is a rare complication and the left localization is exceptional.
OBSERVATION: We report the case of a 26-year-old patient from the rural area who was referred to us for treatment of biliptysis. The thoracic and abdominal computed tomography diagnosed a left biliobronchial fistula was. Management consisted of primary endoscopic sphincterotomy, followed by left exclusive thoracotomy surgery to treat pulmonary, hepatic and diaphragmatic repair. The evolution was favorable with disappearance of the biliptysie.
CONCLUSION: The diagnosis o

INTRODUCTION: Biliobronchial fistula (BBF) is an abnormal communication between the biliary system and bronchial tree due to trauma, hemiheptectomy benign biliary stricture with cholangitis and necrotic hepatic infections such as hydatid disease. Intratoracic rupture of a hydatis cyst of the liver is a rare but severe complication with an incidence about 1% and mortality rate about 10%.
CASE REPORT: A 50-year-old male patient presented with cough, episodes of biliary expectoration and haemoptysis followed with fever, fatigue and dyspnea. The diagnosis was set by echosonography, Chest x-ray fi

A recently observed personal case of spontaneous cholecystospontaneous fistula is reported. Factors leading to the relative infrequency of this form in recent years are described, and its rarity is demonstrated by the fact that the case reported was the first to be noted in about 1000 operations on the bile ducts. A brief account of the genesis, diagnosis and treatment of the condition is given. The interest of the case lay in the unusual site of the orifice (epigastrium).
