Opsin activation as a cause of congenital night blindness.

Three different mutations of rhodopsin are known to cause autosomal dominant congenital night blindness in humans. Although the mutations have been studied for 10 years, the molecular mechanism of the disease is still a subject of controversy. We show here, using a transgenic Xenopus laevis model, that the photoreceptor cell desensitization that is a hallmark of the disease results from persistent signaling by constitutively active mutant opsins.

[Blindness after nitrous oxide anesthesia and internal gas tamponade].

The authors describe the case of a patient with bilateral retinal detachment. Immediately after surgery for the second eye under general ane … e was a central retinal artery occlusion, which resulted in irreparable ischemic retinal damage and blindness, explaining the severe and sudden visual loss. Nitrous oxide is regularly used but contraindicated if intraocular gas is present, due to its potential threat to visual function. We suggest that patients anesthetized with nitrous oxide carry a card o …

Vitamin A deficiency presenting as night blindness during pregnancy.

We describe a patient with a 6-year history of pancreatic malabsorption following surgical subtotal pancreatectomy. She presented at 33 weeks of pregnancy with night blindness as a result of vitamin A deficiency. She had had two successful pregnancies 9 and 8 years previously, giving birth to a healthy baby boy on each occasion. We suggest that patients with long-term malabsorption due to intestinal or pancreatic disease should have vitamin A status checked prior to and during pregnancy so that prompt supplementation can be commenced if necessary. The possibility that vitamin A deficiency may …

The Sivaganga eye survey: I. Blindness and cataract surgery.

PURPOSE: To assess the prevalence of visual acuity impairment, blindness, and cataract surgery among older adults in rural southern India. METHODS: Random selection of village- and urban-based clusters was used to identify a cross-sectional sample of persons 50 years of age or older from the Sivaganga district of Tamil Nadu. Subjects in 25 selected clusters were enumerated through a door-to-door survey and invited to examination sites for measurement of uncorrected, presenting, and best-corrected visual acuity and ocular examination in 1999. The principal cause was identified for eyes with pr…

Benign adult familial myoclonic epilepsy (BAFME) with night blindness.

This is the first report of benign adult familial myoclonic epilepsy (BAFME) with night blindness. Our cases of BAFME (mother, son, and daughter) demonstrated night blindness with a reduced b-wave response on electroretinography (ERG) suggesting an alteration in calcium-mediated neurotransmitter release from photoreceptors in response to light. Several familial epilepsies have been shown to be due to a channelopathy. On the other hand, the mutation of a calcium-channel gene in Xp11.23 was recently reported in incomplete X-linked congenital stationary night blindness (CSNB). Although the gene l…

Visual impairment and blindness in Europe and their prevention.

The European region currently differs in many aspects, such as political, socioeconomic, and geographical. After substantial political changes at the beginning of the 1990s, the majority of central and eastern European countries started to rebuild their healthcare systems. It is apparent that eastern Europe represents a highly diverse region where the difference among countries broadens year after year. In highly industrialised countries of Europe, the leading causes of childhood serious visual loss are lesions of the central nervous system, congenital anomalies and retinal disorders. In the m

[Onset of Behçet's disease as meningoencephalitis and sudden blindness].

CASE REPORT: A 41-year-old man presented a clinical picture characterized by lymphocytic meningoencephalitis, visual loss in both eyes and transverse sinus thrombosis. This picture was treated with prednisone and anticoagulation. Fundus examination showed complete occlusive arteritis, periphlebitis, peripheral ischemia and perfusion macular defects affecting both eyes. The picture was suggestive of Behçet's disease. Azathioprine was added to the treatment without improvement in visual acuity. Four months later oral aphthous ulcers developed, confirming the suspected diagnosis. DISCUSSION: Beh

Optic neuritis with transient total blindness during lactation(1).

BACKGROUND: Lactation-induced blindness is a rare but devastating puerperal complication. There are few reported cases and no consistent associated disease processes historically identified. This case illustrates lactation-associated optic neuritis as an early identifier of multiple sclerosis. CASE: A nulliparous woman underwent a term vaginal delivery complicated only by chorioamnionitis. She was treated with intravenous antibiotics, which included spontaneous bacterial endocarditis prophylaxis. Her postpartum course was uncomplicated, and she was discharged on postpartum day 2 with her infa…

[Blindness caused by an airbag in a minor accident].

PURPOSE: Installation of airbags has reduced the rate of fatal injuries in severe automobile accidents. We report, however, severe ocular injuries in a minor accident as the result of an airbag. CASE REPORT: A front passenger suffered a blunt ocular trauma of her right eye during a collision. The approaching speed was about 31 km/h. The maximum change of velocity in direction of the impact was 19 km/h. Color traces were found on the upper rim of the airbag, apparently from the patient's eye shadow. RESULTS: In the emergency room, visual acuity was reduced to light perception. There was endot

Prevalence of sleep/wake disorders in persons with blindness.

Blind individuals are not only handicapped by their loss of vision, but are also affected because the loss of sight may have a secondary imp … ls, and a higher incidence of inappropriate involuntary daily naps. In conclusion, individuals with blindness report a significant curtailment of total sleep time and hence resulting daytime somnolence, which impacts on daytime activities. A 'free-running' condition is also a common sleep/wake impairment that may compound the handicap of blindness.

Registration of new blindness in Singapore for 1985-1995.

Data on causes of blindness in Singapore from blind registration have been gathered yearly since 1950, when blind registration in Singapore was first started. The blind register is maintained by the Singapore Association of the Visually Handicapped (SAVH), based on reports from practising ophthalmologists in the government institutions. Ophthalmologists in the private sector (who also attend to the bulk of patients coming from overseas) do not register their blind patients. During the past four decades (from 1950 till 1990), analyses of our causes of blindness have been published by various lo…

Computer-based training for the treatment of partial blindness.

Partial blindness after brain injury has been considered non-treatable. To evaluate whether patients with visual-field defects can profit from computer-based visual restitution training (VRT), two independent clinical trials were conducted using patients with optic nerve (n = 19) or post-chiasmatic brain injury (n = 19). In post-chiasma patients, VRT led to a significant improvement (29.4%) over baseline in the ability to detect visual stimuli; in optic nerve patients, the effects were even more pronounced (73.6% improvement). Visual-field enlargements were confirmed by the observation of a vi…

[Cause of blindness in the province of Giang-Vietnam].

An Giang province is one of many provinces of Mekong Delta, South of Vietnam, with 2 million people and 85% population living in the countryside for agriculture. Main causes of blindness are leucoma secondary to trachoma and mycoses (16%) closed angle glaucoma (5%) and cataract (50%). Percentage of eyes requiring ocular surgery is 0.88% of total population. 1200 patients had surgery during year 1995; as 16,000 patients are waiting for surgery, we stress that this backlog must be reduced as soon as possible. On an other hand, we intend to develop corneal transplantation.

Tuberculous meningitis with blindness and perichiasmal involvement on MRI.

An 11-year-old boy with tuberculous meningitis developed blindness, a rare complication of this disease in the United States. Magnetic resonance imaging demonstrated perichiasmal enhancement, suggesting that arachnoiditis caused the visual loss. Serial neuroimaging over 7 months revealed a persistent inflammatory process in the chiasmatic cistern, hydrocephalus, and progressive cerebral infarctions.

Aetiology of severe visual impairment and blindness in microphthalmos.

Microphthalmos occupies a spectrum from a normal, but small globe, to a globe with multiple anterior and posterior segment abnormalities. This study examines 54 eyes of 27 patients who had bilateral microphthalmos and severe visual impairment or blindness. Congenital cataract was the commonest cause of severe visual impairment (44%), followed by presumed retinal or optic nerve dysplasia (30%) and chorioretinal coloboma (22%). Lensectomy was followed by phthisis bulbi in 3/23 cases and retinal detachment in 2/23 cases. There were no cases of angle closure glaucoma. The three clinical conditions…
